BACKGROUND: 'Dropped head' and 'bent spine' phenomena are recognized clinical presentations of neuromuscular disorders. Similar symptoms are known in patients with parkinsonian syndromes, but their pathophysiology remains unclear. One hypothesis is a relation between the movement disorder and the skeletal muscle pathology.
METHODS: We describe detailed histopathological data from 19 consecutive skeletal muscle biopsies in patients with idiopathic Parkinson's disease (PD) and concomitant 'dropped head' or 'bent spine' syndrome. A biochemical analysis of the respiratory chain complexes was also

Objective We herein report our experience with medically treated patients presenting with dropped head syndrome (DHS) due to isolated neck extensor myopathy (INEM) or primary hyperparathyroidism (PHPT)-related myopathy, focusing on the treatment response and the correlation between muscle pathology and INEM outcomes. Methods This single-center retrospective observational study reviewed 20 consecutive patients who presented with DHS as their primary complaint between March 2021 and June 2025. Sixteen patients were included in this study: 13 with INEM who received immunotherapy as the primary me

Dropped head syndrome is a common complication of various neurological disorders. Most commonly, dropped head syndrome is due to primary or secondary myopathy. However, neurodegenerative diseases and various other conditions can also be complicated by dropped head syndrome. Among the primary myopathies, dropped head occurs most commonly in association with mitochondrial disorders, congenital myasthenic syndrome, and axial myopathies. Among the secondary myopathies, dropped occurs most commonly in association with inflammatory myopathies. Myasthenia is the most common transmission disorder asso

BACKGROUND: In recent years, camptocormia and dropped head syndrome (DHS) have gained attention as particular forms of movement disorders. Camptocormia presents with involuntary forward flexion of the thoracolumbar spine that typically increases during walking or standing and may severely impede walking ability. DHS is characterized by weakness of the neck extensors and a consecutive inability to extend the neck; in severe cases the head is fixed in a "chin to chest position." Many diseases may underlie these conditions, and there have been some reports about radiation-induced camptocormia and

Dropped head syndrome (DHS) involves severe weakness of the neck extensor muscles causing the mandible to drop to the chest wall. Isolated neck extensor weakness is a rare complication of radiotherapy. This condition may result within a few weeks or months following radiotherapy (early-onset) or several years after radiotherapy (late-onset), with the latter more commonly encountered. Person-in-the-barrel syndrome is marked by bilateral brachial diplegia, intact cranial nerves, and preserved lower extremity strength. We describe the unique clinical profile of a patient with a six-week history o

Drop head syndrome (DHS), characterized by pronounced cervical ventroflexion, is a clinical syndrome that can be found associated with neuromuscular disorders, particularly myasthenia gravis. In this case series, we describe two dogs, an 8-year-old Basset Hound and a 3-year-old Zwergpinscher, presenting with DHS as the main clinical sign. In both cases, a presumptive diagnosis of myasthenia gravis was made and were empirically treated with pyridostigmine. Telephone follow-ups at 6 and 4 months after diagnosis, respectively, were consistent with clinical remission. These cases highlight the dia

BACKGROUND: Postoperative dysphagia has been reported to occur as the result of narrowing of the pharyngeal airway space (PAS) due to a change in the alignment of the cervical spine. This study aimed to investigate potential risk factors for postoperative PAS obstruction following corrective surgery for dropped head syndrome (DHS) by analyzing clinical and radiographic parameters. METHODS: This retrospective design included 42 patients (7 men, 35 women; mean age, 71.5 years) who underwent corrective surgery for DHS. The following measurements were obtained: narrowest PAS (nPAS), cervical verte

Dropped head syndrome, characterized by excessive flexion of the neck, frequently leads to significant impairment in quality of life. Among the various causes of this syndrome, some cases respond effectively to internal medicine. We report a case of a woman in her 70s who presented with dropped head syndrome and was finally diagnosed with hypothyroid myopathy limited to the extensor muscles of the neck. Cervical spine MRI at the initial examination indicated thyroid atrophy, increased subcutaneous fat, and a mild high signal in the right cervical extensor muscles on fat-suppressed T2-weighted

BACKGROUND: Dropped head syndrome (DHS) is caused by dysfunction of the cervical musculature. It is classified into two groups according to the cause: one is weakness of the neck extensors and the other is hypercontraction of the cervical flexors associated with Parkinson's disease and other disorders. Although some drugs have previously been reported as suspected causes of DHS, we are unaware of any reports in which oxaliplatin was suspected. In this report, we describe a case of DHS during adjuvant chemotherapy for gastric cancer, along with a review of the relevant literature.

BACKGROUND: Some myopathies can lead to dropped head or bent spine syndrome (DH/BS). The significance of this symptom has not been studied in inflammatory myopathies (IM).
OBJECTIVES: To assess the significance of DH/BS in patients with IM.
METHODS: Practitioners from five IM networks were invited to report patients with IM suffering from DH/BS (without other known cause than IM). IM patients without DH/BS, randomly selected in each participating centre, were included as controls at a ratio of 2 to 1.
RESULTS: 49 DH/BS-IM patients (DH: 57.1%, BS: 42.9%) were compared with 98 control-IM pati

BACKGROUND: Dropped head syndrome is commonly observed in neurodegenerative disorders such as dementia with Lewy bodies. Its underlying causes include neck extensor myopathy and cervical dystonia. However, spontaneous remission of dropped head syndrome is extremely rare.
CASE PRESENTATION: A 70-year-old woman initially presented with a dropped head. One year after her first visit, she developed gait disturbance, bilateral hand bradykinesia, and rigidity of the trunk and all four limbs. Levodopa had a limited effect on both the dropped head and parkinsonism, and was eventually discontinued due

BACKGROUND: Most of the previous studies about the surgical treatment of dropped head syndrome (DHS) are small case series, and their primary outcome measures were cervical alignment parameters. Therefore, little is known about the associations between pre- and postoperative global sagittal alignment in the whole spine and the clinical outcomes of the surgical treatment of DHS. In this study, we investigated the surgical outcomes of DHS, including correction of cervical and global spinal sagittal alignment.
METHODS: This study was a retrospective observational study. Fifteen patients with DHS

Mutations in the MYH7 gene are the source of an allelic series of diseases, including various cardiomyopathies and skeletal myopathies that usually manifest in adulthood. We observed a 1.5 y.o. male patient with congenital weaknesses of the axial muscles, "dropped head" syndrome, and dilated cardiomyopathy. The clinical evaluation included medical history, an echocardiogram, electromyography, and a histopathological study. The genetic evaluation included whole exome sequencing. Muscle biopsy samples from the proband were used for mRNA extraction. We revealed a novel genetic variant c.5655 + 5G

The dropped head syndrome (DHS) is a disabling condition caused by severe weakness of the neck extensor muscles causing progressive reducible kyphosis of the cervical spine and the inability to hold the head up. Weakness can occur in isolation or in association with a generalized neuromuscular disorder. Isolated cases are owed to the late onset of noninflammatory myopathy designated as INEM, where persistent chin to chest deformity may gradually cause or aggravate preexisting degenerative changes of the cervical spine and ultimately result in myelopathy. In review of the literature, we could f
