Genetic consequences of effective and suboptimal dosing with mutagenic drugs in a hamster model of SARS-CoV-2 infection.

Mutagenic antiviral drugs have shown promise against multiple viruses, but concerns have been raised about whether their use might promote the emergence of new and harmful viral variants. Recently, genetic signatures associated with molnupiravir use have been identified in the global SARS-COV-2 population. Here, we examine the consequences of using favipiravir and molnupiravir to treat SARS-CoV-2 infection in a hamster model, comparing viral genome sequence data collected from (1) untreated hamsters, and (2) from hamsters receiving effective and suboptimal doses of treatment. We identify a bro

A new Epseptimavirus bacteriophage vB_SalS-SIY1lw as a potential antimicrobial alternative to multidrug-resistant Salmonella Infantis.

Salmonella Infantis is an emerging pathogen highly associated with antibiotic-resistant issues and has contributed to increasing foodborne illness in recent years. Thus, finding novel antimicrobial agents is urgent for the solution. The objective was to characterize a newly isolated Epseptimavirus phage with antimicrobial activity against multidrug-resistant S. Infantis strains. Salmonella phage vB_SalS-SIY1lw (or SIY1lw) is a new member of the phages belonging to the Epseptimavirus genus. SIY1lw contained the receptor binding protein (ORF 23) and tail fiber protein (ORF 43) genes—both associa

Nucleoside analogue reverse transcriptase inhibitor options: a re-examination of the class.

The main options for dual nucleoside (or nucleotide) analogue reverse transcriptase inhibitors (nRTIs) as a component of initial antiretroviral therapy regimens are tenofovir/emtricitabine, zidovudine/lamivudine, and abacavir/lamivudine as fixed-dose combinations. Resistance to nRTIs can limit usefulness of many of the drugs in the class. Investigation of triple nRTI regimens has shown that zidovudine/lamivudine/abacavir does not provide benefits compared with dual nRTIs plus efavirenz and that others (tenofovir/lamivudine/abacavir and didanosine/lamivudine/abacavir) are associated with very h

Prevalence and factors associated with drug resistance mutations among HIV - 1 patients failing to achieve virological suppression on second line treatment in Tanzania.

Human Immunodeficiency Virus / Acquired Immunodeficiency Syndrome (HIV/AIDS) is a major public health problem affecting an estimated 38.4 million people globally, with 54% of them being from the Eastern and Southern African regions. Despite progress in the availability of multiple classes of antiretroviral drugs and early initiation of treatment with cART, many patients still fail to achieve viral suppression on first and second-line regimens due to drug resistance that is driven by continuous acquisition of drug resistance mutation (DRMs). DRMs are prevalent among adults failing treatment reg

Antiviral drugs for viruses other than human immunodeficiency virus.

Most viral diseases, with the exception of those caused by human immunodeficiency virus, are self-limited illnesses that do not require specific antiviral therapy. The currently available antiviral drugs target 3 main groups of viruses: herpes, hepatitis, and influenza viruses. With the exception of the antisense molecule fomivirsen, all antiherpes drugs inhibit viral replication by serving as competitive substrates for viral DNA polymerase. Drugs for the treatment of influenza inhibit the ion channel M(2) protein or the enzyme neuraminidase. Combination therapy with Interferon-α and ribavirin

The distinct contributions of fitness and genetic barrier to the development of antiviral drug resistance.

The selection of resistance to antiviral drugs depends on multiple parameters, including the genetic barrier that is often broadly defined as the number of mutations required to overcome drug selective pressure. In this review, it is intended to assess the roles of genetic barrier and viral fitness at various stages of the selection process. Several examples in the fields of HIV and HCV drug resistance show that the two parameters are distinct and independent. An analogy to the concept of kinetic versus thermodynamic control of chemical reactions supports a more narrow definition of the geneti

Quantifying antiviral activity optimizes drug combinations against hepatitis C virus infection.

With the introduction of direct-acting antivirals (DAAs), treatment against hepatitis C virus (HCV) has significantly improved. To manage and control this worldwide infectious disease better, the "best" multidrug treatment is demanded based on scientific evidence. However, there is no method available that systematically quantifies and compares the antiviral efficacy and drug-resistance profiles of drug combinations. Based on experimental anti-HCV profiles in a cell culture system, we quantified the instantaneous inhibitory potential (IIP), which is the logarithm of the reduction in viral repl

Inflammation burden score in multidrug-resistant HIV-1 infection.

OBJECTIVES: Four-class drug-resistant (4DR) people living with HIV (PLWH) are a fragile population with a high burden of disease. No data on their inflammation and T-cell exhaustion markers are currently available. METHODS: Inflammation, immune activation and microbial translocation biomarkers were measured through ELISA in 30 4DR-PLWH with HIV-1 RNA ≥ 50 copies/mL, 30 non-viremic 4DR-PLWH and 20 non-viremic non-4DR-PLWH. Groups were matched by age, gender and smoking habit. T-cell activation and exhaustion markers were assessed by flow cytometry in 4DR-PLWH. An inflammation burden score (IBS

Drug Susceptibility and Viral Fitness of HIV-1 with Integrase Strand Transfer Inhibitor Resistance Substitution Q148R or N155H in Combination with Nucleoside/Nucleotide Reverse Transcriptase Inhibitor Resistance Substitutions.

In clinical trials of coformulated elvitegravir (EVG), cobicistat (COBI), emtricitabine (FTC), and tenofovir disoproxil fumarate (TDF), emergent drug resistance predominantly involved the FTC resistance substitution M184V/I in reverse transcriptase (RT), with or without the tenofovir (TFV) resistance substitution K65R, accompanied by a primary EVG resistance substitution (E92Q, N155H, or Q148R) in integrase (IN). We previously reported that the RT-K65R, RT-M184V, and IN-E92Q substitutions lacked cross-class phenotypic resistance and replicative fitness compensation. As a follow-up, the in vitr

A spatio-temporal assessment of simian/human immunodeficiency virus (SHIV) evolution reveals a highly dynamic process within the host.

The process by which drug-resistant HIV-1 arises and spreads spatially within an infected individual is poorly understood. Studies have found variable results relating how HIV-1 in the blood differs from virus sampled in tissues, offering conflicting findings about whether HIV-1 throughout the body is homogeneously distributed. However, most of these studies sample only two compartments and few have data from multiple time points. To directly measure how drug resistance spreads within a host and to assess how spatial structure impacts its emergence, we examined serial sequences from four macaq

In vitro antiviral activity and cross-resistance profile of PL-100, a novel protease inhibitor of human immunodeficiency virus type 1.

Despite the success of highly active antiretroviral therapy, the current emergence and spread of drug-resistant variants of human immunodeficiency virus (HIV) stress the need for new inhibitors with distinct properties. We designed, produced, and screened a library of compounds based on an original l-lysine scaffold for their potentials as HIV type 1 (HIV-1) protease inhibitors (PI). One candidate compound, PL-100, emerged as a specific and noncytotoxic PI that exhibited potent inhibition of HIV-1 protease and viral replication in vitro (K(i), approximately 36 pM, and 50% effective concentrati

Bacteriophage for Mitigation of Multiple Drug Resistant Biofilm Forming Pathogens.

BACKGROUND: Microbial communities encased in exopolymeric substances (EPS) attached to suitable substrate as biofilms show heightened resistance to multiple drugs including antibiotics. One promising control strategy in dealing with the ever mounting problem of antibiotic resistance amongst biofilm forming bacterial pathogens is the use of biological agents. OBJECTIVE: This review focuses on the development of bacteriophages as means of disrupting biofilm forming pathogens and hence mitigation of multiple drug resistant organisms. RESULTS: Bacteriophages are obligatory viral intracellular pa

Ten years survey of primary HIV-1 resistance in Serbia: the occurrence of multiclass resistance.

In Serbia, the first cases of HIV infection were reported in 1985, whereas antiretroviral (ARV) therapy has been in use since 1987. With this study we aimed to assess the occurrence and pattern of HIV resistance mutations among newly diagnosed patients in the period 2002-2011. The study prospectively included 181 adult patients. Genotypic HIV-1 drug resistance testing was performed and drug resistance was scored according to the 2009 WHO list for surveillance of drug resistance mutations (SDRMs). A bioinformatic approach was used to estimate the duration of infection by calculating the percent

Advancements in development of novel class of HIV protease inhibitors.

BACKGROUND: Protease (PR) inhibitors (PIs) are widely regarded as the most significant agents in the treatment of human immunodeficiency virus (HIV) and have been instrumental in the success of highly active antiretroviral therapy (HAART). The PR enzyme is essential for the viral life cycle, as it cleaves various polyproteins into the individual components necessary for the formation of mature, infectious virions. METHODS: We systematically synthesized current evidence which revealed the multifaceted effects of PIs, which can function as entry inhibitors, reverse transcription inhibitors and

HIV type 1 integrase inhibitors: from basic research to clinical implications.

Similar to other retroviruses, productive infection with HIV-1 requires three key steps in the viral replication: (i) reverse transcription of viral genomic RNA into viral cDNA by the viral reverse transcriptase; (ii) integration of viral cDNA into host cell genome using the viral integrase; and (iii) cleavage of newly synthesized viral polypeptide by the viral protease into individual viral proteins during new virion assembly. Following their discovery, all three viral enzymes were considered as targets for antiretroviral drugs. However, while multiple reverse transcriptase and protease inhib

The emerging field of HCV drug resistance.

BACKGROUND: With 170 million people infected worldwide and an inadequate current standard of care, hepatitis C virus (HCV) infection represents a major unmet medical need. Multiple companies are working on the discovery and development of specific HCV antiviral drugs, including inhibitors of HCV polymerase, protease and NS5A. Because of the error-prone nature of viral RNA replication, resistance mutants will develop that could present a potentially significant challenge to developing antiviral treatment regimens. OBJECTIVE: Here, we review the major drug classes currently in preclinical and c

Cell-specific establishment of poliovirus resistance to an inhibitor targeting a cellular protein.

UNLABELLED: It is hypothesized that targeting stable cellular factors involved in viral replication instead of virus-specific proteins may raise the barrier for development of resistant mutants, which is especially important for highly adaptable small (+)RNA viruses. However, contrary to this assumption, the accumulated evidence shows that these viruses easily generate mutants resistant to the inhibitors of cellular proteins at least in some systems. We investigated here the development of poliovirus resistance to brefeldin A (BFA), an inhibitor of the cellular protein GBF1, a guanine nucleoti

Targeting HIV-1 integrase: A review of options, challenges and future directions.

HIV-1 infects CD4+ T-cells, causing immunodeficiency and, if untreated, progression to AIDS. Global antiretroviral therapy scale-up has reduced deaths and new infections by nearly half over the past decade, yet lifelong treatment and emerging drug resistance remain critical challenges. This review provides an overview of the HIV-1 lifecycle, with emphasis on the structure, function, and multiple roles of IN in viral replication. We discuss the development, mechanisms of action, clinical utility, and resistance profiles of current integrase inhibitors, underscoring the need for innovative appro

Predominance of positive epistasis among drug resistance-associated mutations in HIV-1 protease.

Drug-resistant mutations often have deleterious impacts on replication fitness, posing a fitness cost that can only be overcome by compensatory mutations. However, the role of fitness cost in the evolution of drug resistance has often been overlooked in clinical studies or in vitro selection experiments, as these observations only capture the outcome of drug selection. In this study, we systematically profile the fitness landscape of resistance-associated sites in HIV-1 protease using deep mutational scanning. We construct a mutant library covering combinations of mutations at 11 sites in HIV-
