Systemic and local tolerability of ophthalmic drug formulations. An update.

All eye drops raise problems of local tolerance, but with variable frequencies. They can induce pain on instillation, allergic reactions, delayed healing, punctate keratitis, disturbances of lacrimal secretion, disturbances of accommodation (especially the parasympathomimetics) and local pigmentation after prolonged use. Corticosteroids are associated with 2 major risks: chronic glaucoma and cataract, initially reversible if treatment is stopped. There is still a major risk of corneal herpes with corticosteroids. It is important to be aware of these local problems as they are responsible for p

ADA assays for high-dose biologics: redefining drug tolerance through clinical insights.

The validation of anti-drug antibody (ADA) assays is vital in biologics development, with regulatory bodies like EMA and FDA emphasizing drug tolerance. In patient care, drug tolerance assessments should reflect actual clinical use. We developed an ADA assay for a fully human monoclonal antibody used in oncology, addressing the challenges posed by high circulating drug levels and target biology. Various assay formats were tested using multiple positive controls and drug concentrations that mimic real-world exposure and drug-to-ADA ratios. Assessing different positive controls at various concen

Oral tolerance to prevent anti-drug antibody formation in protein replacement therapies.

Protein based therapeutics have successfully improved the quality of life for patients of monogenic disorders like hemophilia, Pompe and Fabry disease. However, a significant proportion of patients develop immune responses towards intravenously infused therapeutic protein, which can complicate or neutralize treatment and compromise patient safety. Strategies aimed at circumventing immune responses following therapeutic protein infusion can greatly improve therapeutic efficacy. In recent years, antigen-based oral tolerance induction has shown promising results in the prevention and treatment of

Effect of tolerance on the evolution of antibiotic resistance under drug combinations.

Drug combinations are widely used in clinical practice to prevent the evolution of resistance. However, little is known about the effect of tolerance, a different mode of survival, on the efficacy of drug combinations for preventing the evolution of resistance. In this work, we monitored Staphylococcus aureus strains evolving in patients under treatment. We detected the rapid emergence of tolerance mutations, followed by the emergence of resistance, despite the combination treatment. Evolution experiments on the clinical strains in vitro revealed a new way by which tolerance promotes the evolu

Drug-induced tolerance: the effects of antibiotic pre-exposure in Stenotrophomonas maltophilia.

Aim: To investigate if the prior use of nontargeted antibiotics induces cross-tolerance in Stenotrophomonas maltophilia. Methods: Antibiotic induction was performed to evaluate daptomycin and vancomycin as possible tolerance-inducing drugs measured by minimum bactericidal concentration/minimum inhibitory concentration (MIC) ratio, adapted disk-diffusion tests and time-kill curves. Results: After antibiotic exposure, three potentially tolerant strains were isolated, maintaining the same MIC value of levofloxacin, with minimum bactericidal concentration/MIC ratio slightly higher than the parenta

A case of very long-term appearing drug tolerance to intrathecal baclofen.

A 66-year-old man affected by familial spastic paraplegia since he was 22 developed drug tolerance to intrathecal baclofen after 16 years of treatment A stable dosage of 850 microg/day, achieved after the first two years, appeared to be progressively inadeguate to relief his spasticity. No other evidence of additional diseases or progression of his neurological disease were recognized. The daily dosage was then increased to 1200 microg/day without any decrease in spasticity or improvement in the patient's motor performance. Thus a slow and progressive decrease of the daily dosage was performer

Variability in intrinsic drug tolerance in Mycobacterium tuberculosis corresponds with phylogenetic lineage.

Drug tolerance allows bacteria to survive extended exposure to bactericidal drugs and is thought to play a role in drug resistance evolution. In Mycobacterium tuberculosis (Mtb), the causative agent of tuberculosis (TB), multidrug-resistant TB outbreaks are frequently caused by strains belonging to two phylogenetic lineages of the human-adapted strains of the Mtb complex, namely, lineages (L) 2 and L4. We hypothesized that members of L2 and L4 are more intrinsically drug tolerant and, as such, more readily evolve drug resistance. To explore this, we devised a high-throughput in vitro assay to

Efficacy and Tolerance of Synthetic Cannabidiol for Treatment of Drug Resistant Epilepsy.

Objective: Controlled and open label trials have demonstrated efficacy of cannabidiol for certain epileptic encephalopathies. However, plant derived cannabidiol products have been used almost exclusively. Efficacy of synthetically derived cannabidiol has not been studied before. The objective of this study was to evaluate tolerability and efficacy of synthetic cannabidiol in patients with pharmacoresistant epilepsy. Methods: In this prospective, open-label study (DRKS00013177), patients with pharmacoresistant epilepsy received synthetic cannabidiol in addition to their previously stable antico

Application of chimerism-based drug-induced tolerance to rat into mouse xenotransplantation.

The current critical shortage of human donor organs has stimulated the feasibility of the xenogenic transplantation, such as swine to primate. We have previously reported the induction of donor-specific tolerance in MHC-disparated recipient mice by using our cyclophosphamide (CP)-induced tolerance conditioning. In this study, we examined the efficacy of our CP-induced tolerance conditioning in xenogenic transplantation model. F344 rats and B10 mice were used as donors and recipients. Recipient mice were treated with donor spleen cells, CP, Busulfan and bone marrow cells, with or without prior

The proteasome regulator Rpn4 controls antifungal drug tolerance by coupling protein homeostasis with metabolic responses to drug stress.

Fungal pathogens overcome antifungal drug therapy by classic resistance mechanisms, such as increased efflux or changes to the drug target. However, even when a fungal strain is susceptible, trailing or persistent microbial growth in the presence of an antifungal drug can contribute to therapeutic failure. This trailing growth is caused by adaptive physiological changes that enable the growth of a subpopulation of fungal cells in high drug concentrations, in what is described as drug tolerance. Mechanistically, antifungal drug tolerance is incompletely understood. Here we report that the trans

Active transcriptome remodeling underlies antimony-induced drug tolerance in persister-like Leishmania parasites.

Persistence - characterized by the transient ability of subpopulations of Leishmania parasites to survive exposure to drugs - is a major driver of treatment failure and clinical relapses in leishmaniasis. Persisters are characterized by non-dividing or slow-growing state and increased drug tolerance. However, the molecular mechanisms governing formation of persisters remain poorly understood in Leishmania parasites. Here, we developed a model to explore persistence in Leishmania mexicana. Viable promastigote persister-like subpopulations were enriched using Ficoll density gradient centrifugati

Beyond Inhibition: Sublethal Rifampicin-Induced Molecular Adaptations Confer Phenotypic Drug Tolerance in Mycobacteria.

Tuberculosis (TB) remains a major global health threat, largely due to Mycobacterium tuberculosis (Mtb) resilience, which can be exacerbated by exposure to sublethal antibiotic concentrations arising from factors such as patient nonadherence and the granuloma structure which limits drug penetration. Within host granulomas, Mtb can exhibit both phenotypic tolerance and genotypic resistance, complicating curative treatments. This study aimed to determine whether sublethal rifampicin acts as a signaling molecule in Mycobacterium smegmatis (Msm) and the attenuated Mtb H37Ra strain, triggering phen

Chronic NT69L potently prevents drug-induced disruption of prepulse inhibition without causing tolerance.

NT69L is a neurotensin receptor agonist with antipsychotic-like activity. NT69L blocks apomorphine-induced climbing in rats with no effect on stereotypic behavior, attenuates d-amphetamine-induced hyperactivity, and blocks pharmacologically induced disruption of prepulse inhibition (PPI) of the startle response. Repeated administration of NT69L results in tolerance to some, but not to all of its effects. Because schizophrenic patients require long-term treatment, chronic (21-day) administration of NT69L was tested in PPI with comparisons to chronic haloperidol and clozapine treatment. Sprague-

Drug-tolerant persister cells: the deadly survivors in hematological malignancies.

Although the prognosis of patients with hematologic cancers has improved significantly owing to the advance of radiotherapy, chemotherapy, targeted therapies, immunotherapies, and hematopoietic stem cell transplantation therapy, disease relapse and refractory are still very prevalent and remain the major obstacles for long-term patient survival. Recent evidence suggests that drug-tolerant persister (DTP) cells are considered to be the cellular reservoir for tumor relapses and drug resistance formation. DTP cells refer to tumor cells that are acquired tolerance to anticancer drugs during exposu

A landscape analysis of Japanese tolerability assessments in oncology drug development.

PURPOSE: Drug loss in oncology therapies in Japan highlights the need to reduce operational and regulatory uncertainty in drug development. In oncology, regulatory requirements for Japanese Phase 1 studies that primarily assess tolerability in Japanese participants remain unclear. This lack of clarity may discourage sponsors from pursuing drug development in Japan, contributing to drug loss. We (1) characterized current practices of Japanese tolerability assessments through a landscape analysis and (2) identified factors associated with single-dose Japanese tolerability assessments. METHODS:

Targeting Upregulated cIAP2 in SOX10-Deficient Drug Tolerant Melanoma.

Drug tolerance and minimal residual disease (MRD) are likely to prelude acquired resistance to targeted therapy. Mechanisms that allow persister cells to survive in the presence of targeted therapy are being characterized but selective vulnerabilities for these subpopulations remain uncertain. We identified cellular inhibitor of apoptosis protein 2 (cIAP2) as being highly expressed in SOX10-deficient drug tolerant persister (DTP) melanoma cells. Here, we show that cIAP2 is sufficient to induce tolerance to MEK inhibitors, likely by decreasing the levels of cell death. Mechanistically, cIAP2 is
