Shoulder dystocia: a fetal-physician risk.

Trauma that occurs as a result of shoulder dystocia is an important cause of neonatal morbidity. If the occurrence of severe shoulder dystocia, resulting in fetal asphyxia and trauma, could be accurately predicted from maternal risk factors, then a cesarean section would be indicated to prevent the poor outcome. The information available in the obstetric literature, however, is contradictory regarding whether shoulder dystocia can be predicted. In the present study, the patients at greatest risk of shoulder dystocia (all 394 mothers delivering neonates with birth weights greater than or equal

Reverse Løvset maneuver for shoulder dystocia.

Shoulder dystocia is an obstetrical emergency associated with fetal morbidity and mortality. Mechanical obstruction and failure of fetal shoulder rotation prevent their descent into the pelvis. Current management strategies work by increasing the relative pelvic diameters, rotating the fetal shoulders into a more favorable pelvic diameter, or by reducing the fetal biacromial diameter. This study presents the reverse Løvset maneuver that was initially described in 1948 by the Norwegian obstetrician Jørgen Løvset. This is a powerful internal rotational maneuver that differs from the more widely

Shoulder Dystocia: Managing an Obstetric Emergency.

Shoulder dystocia is an obstetric emergency in which normal traction on the fetal head does not lead to delivery of the shoulders. This can cause neonatal brachial plexus injuries, hypoxia, and maternal trauma, including damage to the bladder, anal sphincter, and rectum, and postpartum hemorrhage. Although fetal macrosomia, prior shoulder dystocia, and preexisting or gestational diabetes mellitus increases the risk of shoulder dystocia, most cases occur without warning. Labor and delivery teams should always be prepared to recognize and treat this emergency. Training and simulation exercises i

Maternal Endogenous Forces and Shoulder Dystocia.

Childbirth is a complicated biomechanical process that many take for granted. However, the delivery forces generated by a mother (uterine contractions and maternal pushing) are strong and have a significant effect on the body and tissues of the fetus, especially during the second stage of labor. Although most infants are born without negative, force-related outcomes, in some infants the normal forces of labor cause an injury that can have either temporary or permanent sequelae. The biomechanical situation is further complicated when an infant's shoulder impacts the maternal pelvis, which provi

Shoulder dystocia: an Evidence-Based approach.

Shoulder Dystocia (SD) is the nightmare of obstetricians. Despite its low incidence, SD still represents a huge risk of morbidity for both the mother and fetus. Even though several studies showed the existence of both major and minor risk factors that may complicate a delivery, SD remains an unpreventable and unpredictable obstetric emergency. When it occurs, SD is difficult to manage due to the fact that there are not univocal algorithms for its management.Nevertheless, even if it is appropriately managed, SD is one of the most litigated cause in obstetrics, because it is frequently associate

Intrapartum interventions for preventing shoulder dystocia.

BACKGROUND: The early management of shoulder dystocia involves the administration of various manoeuvres which aim to relieve the dystocia by manipulating the fetal shoulders and increasing the functional size of the maternal pelvis. OBJECTIVES: To assess the effects of prophylactic manoeuvres in preventing shoulder dystocia. SEARCH STRATEGY: We searched the Cochrane Pregnancy and Childbirth Group's Trials Register (1 June 2006). SELECTION CRITERIA: Randomised controlled trials comparing the prophylactic implementation of manoeuvres and maternal positioning with routine or standard care. DA

[Shoulder dystocia from a legal standpoint].

Even though a shoulder dystocia occurs very seldom it plays an important part in obstetrical medical opinions since it is quite often associated with infant plexus brachialis injuries. In legal medical discussions it is necessary to determine whether there were certain antepartal risks, if diagnosis and therapy were carried out correctly and whether a shoulder dystocia resp. plexus brachialis injury could have been prevented. In general one is looking for answers to prevent both serious complications. In the past a great number of articles were published referring to these answers. Especially

Isolated and multiple causes of equine dystocia.

BACKGROUND: Dystocia is rare in horses, but is life-threatening for mares and foals. Therefore, veterinary research depends on up-to-date data to optimise equine dystocia management. In addition, knowledge of the prognosis of equine dystocia is necessary to advise animal owners. This retrospective study of equine dystocia aimed to enrich existing datasets with up-to-date information. For the first time, the focus was on the causes of dystocia that occurred alone or in combination. Over a period of 10 years, 72 cases of dystocia were analysed using a standardised, predetermined diagnosis code.

The Pathophysiology of Labor Dystocia: Theme with Variations.

Abnormally prolonged labor, or labor dystocia, is a common complication of parturition. It is the indication for about half of unplanned cesarean deliveries in low-risk nulliparous women. Reducing the rate of unplanned cesarean birth in the USA has been a public health priority over the last two decades with limited success. Labor dystocia is a complex disorder due to multiple causes with a common clinical outcome of slow cervical dilation and fetal descent. A better understanding of the pathophysiologic mechanisms of labor dystocia could lead to new clinical opportunities to increase the rate

Antenatal and intrapartum prediction of shoulder dystocia.

OBJECTIVES: To (1) develop algorithms to calculate the risk of shoulder dystocia at individual deliveries; (2) evaluate screening for shoulder dystocia. STUDY DESIGN: Retrospective analysis of 40284 consecutive term cephalic singleton pregnancies using a 'train and test' method. Four models were derived using logistic regression and tested (birthweight alone; birthweight and other independent antenatal variables; birthweight and all independent antenatal and intrapartum variables; and all independent variables excluding birthweight). RESULTS: Shoulder dystocia occurred in 240 deliveries (0.6

Uterine rupture as a cause of shoulder dystocia.

BACKGROUND: Shoulder dystocia and uterine rupture are complications that rarely are related. CASE: A 34-year-old white woman, gravida 4, para 2, therapeutic abortion 1, was admitted for labor induction. Fetal heart rate monitoring was normal until full dilation, when bradycardia developed and persisted. With the use of forceps, the vertex was delivered. Head retraction signaled the possibility of shoulder dystocia. Attempts at vaginal delivery failed, necessitating abdominal delivery. The fetal body was found anterior to the already contracted, anterolaterally ruptured uterus. An abdominally

The contribution of dystocia to the cesarean section rate.

OBJECTIVE: Our purpose was to examine the indications for performing cesarean sections in current obstetric practice and to define and measure the true contribution of dystocia ("difficult labor") to the overall cesarean section rate. STUDY DESIGN: A prospective audit was done of all cesarean sections performed during 1991 at The Queen Mother's Hospital, Glasgow, a teaching hospital and tertiary referral center. RESULTS: The cesarean section rate for 1991 was 16.3%. Dystocia was the primary indication in 16% of all cesarean sections in 1991 but was actually a contributing factor, directly or

Breech presentation and shoulder dystocia in childbirth.

This review covers the recent literature relevant to breech presentation and shoulder dystocia. Further evidence in favor of external cephalic version at term has emerged, and the possibility of its use in women with previous cesarean section has been suggested. Attention has been given to trial of labor in selected cases of term breech presentation. The evidence to guide the method of delivery for preterm breech presentations remains inadequate. No perinatal differences have been demonstrated among the outcome of breech second-born twins delivered by external cephalic version, breech extracti
