Percutaneous coronary intervention with bivalirudin anticoagulation, immediate sheath removal, and early ambulation: a feasibility study with implications for day-stay procedures.

We assessed the feasibility and safety of a strategy of transfemoral percutaneous coronary intervention (PCI) with bivalirudin anticoagulation, immediate sheath removal, early ambulation, and, where possible, same-day discharge in 100 consecutive patients. Ambulation was achieved by 2 hr 30 min in 85% of patients and same-day discharge in 26%. PCI was angiographically successful in 97%. In hospital, there were no deaths or Q-wave myocardial infarctions. One patient suffered a non-Q-wave infarction, another in-hospital surgical revascularization and one required blood transfusion for rectal ble

Early Neurological Improvement and Ambulation Recovery After Delayed Surgery in Surgically Selected Nonambulatory Metastatic Epidural Spinal Cord Compression: A Retrospective Cohort Study.

Delayed decompression for metastatic epidural spinal cord compression (MESCC) is a common real-world problem, yet short-interval recovery after patients have already remained nonambulatory for at least 48 h is poorly defined. We retrospectively evaluated 41 surgically selected patients with MRI-confirmed epidural MESCC (Bilsky grade 2-3) and preoperative nonambulatory neurological deficit (Frankel grades A-C) lasting at least 48 h. The primary outcome was early neurological improvement, defined as a gain of at least one Frankel grade by postoperative days 10-14. The secondary outcome was early

Characterization of risk factors for early ambulation in paraplegic dogs with absent pain perception undergoing decompressive surgery for thoracolumbar intervertebral disk extrusions.

BACKGROUND: Current literature warrants surgical decompression in paraplegic dogs with absent pain perception (APP), but the rate of ambulatory dogs with APP following thoracolumbar (TL) IVDE surgery in a clinical setting remains unknown. Furthermore, the outcome of paraplegic APP French Bulldogs (FBs) is anecdotally considered poor. The aims of this study were threefold within a large population of TL-IVDE paraplegic dogs with APP undergoing decompressive surgery: (1) to characterize early spontaneous pelvic limb movement and ambulation following surgery; (2) to identify risk factors for the

Immediate Weight-Bearing and Early Ambulation Rehabilitation After Femoral Neck System Fixation for Undisplaced Femoral Neck Fractures: A Prospective Randomized Controlled Trial.

OBJECTIVE: 11 beta-hydroxysteroid dehydrogenase type 1 (11 beta HSD1) converts inactive cortisone to active cortisol. 11 beta HSD1 activity is increased in GH deficiency and inhibited by GH and IGF-I in acromegaly. However it is not known whether these changes in cortisol metabolism exert significant effects during hydrocortisone therapy, and the effect has not been studied in patients taking cortisone acetate. We have studied the effect of GH induced 11 beta HSD1 inhibition in hypopituitary adults with severe GH deficiency to determine whether this inhibition has a different magnitude of effe

Vascular complications of early (3 h) vs standard (6 h) ambulation post-cardiac catheterization or percutaneous coronary intervention from the femoral artery.

BACKGROUND: The optimal time of ambulation post cardiac catheterization (CC) or percutaneous coronary intervention (PCI) done from the femoral artery is not well defined. The aim of this study was to determine whether early (3 hrs) ambulation post CC/PCI is as safe as standard (6 hrs) ambulation time. METHODS: This was a retrospective observational study comparing the vascular complications rate (bleeding, hematoma, pseudoaneurysm formation) among patients who underwent CC, alone or concomitant with PCI, from the femoral artery and who were ambulated after 3 or 6 hrs. RESULTS: The study popu

The Axillary Intra-Aortic Balloon Pump as a Bridge to Recovery Allows Early Ambulation in Long-Term Use: Case Series and Literature Review.

We evaluated the feasibility, tolerability, and efficacy of a strategy for transaxillary artery intra-aortic balloon pump placement for extended mechanical circulatory support of patients with postcardiotomy shock as a bridge to recovery. Patients requiring prolonged intra-aortic balloon pump support for postcardiotomy heart failure were identified. Intra-aortic balloon pump was tunneled through a 6-mm Dacron graft anastomosed to the right axillary artery. The intra-aortic balloon pump catheter is advanced under fluoroscopic guidance into the descending thoracic aorta. Three patients were tran

A patient with Marchiafava-Bignami disease type A transported by ambulance with impaired consciousness and malnutrition was successfully treated after early diagnosis by MRI.

Marchiafava-Bignami disease (MBD) is a very rare disorder characterized by demyelination and necrosis of the corpus callosum. A 53-year-old male was transported to the emergency room with impaired consciousness. On his arrival, he was quite emaciated. CT examination revealed no hemorrhagic lesions whereas MR images showed hyperintense areas throughout the corpus callosum, leading to a diagnosis of MBD. His impaired consciousness improved with treatment, including parenteral thiamine administration. When examining patients with impaired consciousness because of malnutrition, MBD should be taken

Immediate Return to Ambulation and Improved Functional Capacity for Rehabilitation in Complex Regional Pain Syndrome following Early Implantation of a Spinal Cord Stimulation System.

Complex regional pain syndrome (CRPS) is a neuropathic pain condition that is characterized by vasomotor, sensory, sudomotor, and motor symptoms. Spinal cord stimulation (SCS) has been successfully utilized for the treatment of pain refractory to conventional therapies. We present a case of a previously highly functioning 54-year-old female who developed a rarely reported case of idiopathic CRPS of the right ankle which spontaneously occurred four months after an uncomplicated anterior cervical disc fusion. This condition resulted in severe pain and functional impairment that was unresponsive

Is there a relationship between preferred posture and positioning in early life and the direction of subsequent asymmetrical postural deformity in non ambulant people with cerebral palsy?

BACKGROUND: It has been suggested that asymmetrical positioning of an infant with reduced mobility may lead to postural deformity becoming established over time. However, evidence to support or question this line of thinking is lacking. Therefore, the aim of this retrospective cohort study was to test the association between asymmetrical positioning in the first 12 months of life and the subsequent direction of postural deformity in non-ambulant people with cerebral palsy. METHODS: The direction of scoliosis, pelvic obliquity and windswept hip pattern and also the side of unilateral hip sublu

A pilot randomized controlled trial protocol of overground robotic exoskeleton-assisted early ambulation after cardiovascular and thoracic surgery in the acute care setting (REACT-CVTS).

Early ambulation following cardiovascular and thoracic surgery (CVTS) is associated with improved postoperative outcomes. However, barriers such as patient debility, high acuity, hemodynamic instability, cardiopulmonary complications, or resource limitations (e.g., intensive care unit setting, specialized equipment, or staff training) often prevent or delay early ambulation. Overground robotic exoskeletons (ORE) may facilitate earlier and higher-dose ambulation, yet their role in acute CVTS recovery remains uninvestigated. To evaluate the safety, feasibility, and preliminary outcomes of ORE-as

Early ambulation following lower limb burn and surgery is associated with improved long-term functional outcome and reduced hospital length of stay: A longitudinal cohort study.

INTRODUCTION: Ambulating early after lower limb burn surgery has been associated with a multitude of in-hospital and short-term benefits. However, long-term functional recovery following lower limb burns is poorly understood, especially if grafting surgery is required. OBJECTIVES: This study aimed to explore the association between early ambulation following lower limb burn injury and surgery, long-term lower limb functional recovery and secondly, hospital length of stay (LOS). METHODS: This retrospective longitudinal cohort study reported data from 1718 adult patients treated by a tertiary

Nail plate combination for Su type III periprosthetic distal femur fractures results in early ambulation and favorable clinical outcomes: A comparative case series and technical points.

INTRODUCTION: Periprosthetic distal femur fractures are deleterious injuries, found commonly in the elderly osteoporotic population, typically addressed with open reduction internal fixation (ORIF). Fractures that are very distal (Su type III) require special considerations due to limited distal bone stock for fixation. The utilization of the nail-plate combination (NPC) technique for these patients aims for an early return to ambulation by maximizing distal fixation and stability while allowing for immediate postoperative weight-bearing. METHODS: The technical steps for NPC in very distal pe
