Cranial Ultrasound Is an Important Tool in the Recognition of Life-Threatening Infratentorial Hemorrhage in Newborns.

Timely detection of severe infratentorial hemorrhage in neonates is crucial, especially in case of life-threatening brain stem compression and/or acute obstructive hydrocephalus, which need lifesaving neurosurgical intervention. Although the detection of infratentorial hemorrhage by ultrasound scanning is often considered as difficult, the use of additional acoustic windows and recognition of characteristic ultrasound features facilitate early diagnosis. In this case series, we report on newborns with severe, symptomatic infratentorial hemorrhage detected primarily by cranial ultrasound. We de

Risk factors for early intraventricular hemorrhage in low birth weight infants.

Because earlier studies suggested that preterm infants with germinal matrix hemorrhage or intraventricular hemorrhage or both (GMH/IVH) present within the first 12 postnatal hours are at greatest risk for the development of high-grade hemorrhage and neurodevelopmental disability, we examined the risk factors for this insult among 229 neonates of 600 to 1250 gm birth weight in a multicenter study. All had echoencephalography (ECHO) within the first 11 hours and serially for the next 20 days; risk factor data were collected prospectively. Forty-three infants had GMH/IVH within the first 5 to 11

[Present status and future development of neurophysiological examination in laboratory medicine].

In 2011, nine brain and six nerve and muscle physiological examinations were covered by the national health insurance in Japan. The greatest recent change was the digitalization of electroencephalography in the neurophysiological laboratory. This change is good for hospital affairs both economically and ecologically. Board certified doctors or technologists by the Academic Society in Neurophysiology are increasing each year and are improving the clinical level in daily laboratory work. One problem is that the level of neurophysiological laboratory examinations differs among hospitals in Japan,

Fetal magnetic resonance imaging of the brain: is ventriculomegaly the tip of the syndromal iceberg?

Ventriculomegaly discovered on prenatal ultrasound examinations is frequently a significant challenge both for the accurate identification of the etiology of ventriculomegaly as well as for adequate parental counseling. Fetal ventriculomegaly is usually easily depicted on prenatal ultrasound studies. It is, however, of utmost importance to differentiate between the various forms of ventriculomegaly. A ventriculomegaly may be "isolated" or part of a syndrome. In addition, ventriculomegaly may be a complication of an intrauterine infection or hemorrhage. A correct differentiation between the var

[Significance and margin of error of echoventriculography in children up to 3 years of age].

Comparative ventricular measurements were made on 43 small children and infants between the ages of 0 and 3 years, the methods used being those of pneumoencephalography and enchoventriculography. The number of cases that could be analyzed was 31. Good agreement of the results for the third ventricle was obtained in 64.5 per cent and for the lateral ventricular index, in 80.6 per cent of the cases. More or less satisfactory agreement of the two methods provides against making a clear decision in favor of one or the other method. Good agreement was observed between the results of repeated echove

Prophylactic indomethacin for prevention of intraventricular hemorrhage in premature infants.

The impact of early prophylactic use of intravenous indomethacin on the incidence and severity of periventricular-intraventricular hemorrhage and patent ductus arteriosus in 199 oxygen-requiring premature infants (less than or equal to 1300 g birth weight) was prospectively investigated. The trial was controlled, the infants were randomized, and the investigators were unaware of the group assignments. Patients with minimal (grade I) or no periventricular-intraventricular hemorrhage determined by prestudy echoencephalography were randomized within two birth weight subgroups (500 to 899 and 900

Hyperthermia as an early sign of intracranial hemorrhage in full-term neonates.

BACKGROUND: Intracranial hemorrhage (ICH) in full-term infants is uncommon and is a rare cause of neonatal fever. This study was conducted to estimate the incidences of ICH and fever in a hospital-based population, and to determine if the clinical features of neonatal ICH with and without hyperthermia differ. METHODS: We selected 315 afebrile neonates who received cranial ultrasonography screening from 2003 to 2004 as the control group, and 153 neonates diagnosed with fever from 1998 to 2004 as the study group. During the same period, 28 full-term neonates with birth weights >2500g and ICH in

The association of periventricular echodensity with cerebral palsy in preterm infants.

The purpose of this study was to investigate the risk factors of development of cerebral palsy (CP) in preterm infants who had periventricular echogenic abnormalities with or without cyst formation. During a period of 66 months, 332 newborns of 34 weeks gestation or less born at our hospital received serial brain ultrasound scans. Sixty-six of the 332 infants, 36 males and 30 females, were detected increased periventricular echodensity (PVE) during admission period. Subsequently, serial brain ultrasound scans were done for the 66 preterm infants at our follow-up clinic. Their neurodevelopment

Schizencephaly: antenatal and postnatal assessment with colour-flow Doppler imaging.

Schizencephaly, a disorder of neuronal migration, is rarely discovered in utero. Three cases, detected in utero and followed through the early neonatal period, were assessed with colour-flow Doppler imaging in an attempt to determine whether the clefts were associated with any demonstrable vascular disturbance. The Doppler spectral wave-forms of the fetal and neonatal internal carotid and middle cerebral arteries were normal in two of the cases, whereas the right middle cerebral artery was persistently occluded in the third. Although Doppler imaging was not performed during the first trimester

[Peroperative echography in pediatric neurosurgery. Experience in a series of 40 procedures].

Forty neurosurgical operations (supra-tentorial: 27; infra-tentorial: 10; intra-spinal: 3) have been performed under real time ultrasonography control. The supra-tentorial anatomical Landmarks visualized on the screen were the base of the skull, the cranial vault, the falx cerebri and the ventricles. The infra-tentorial landmarks were the petrous bone, the tentorium and the brainstem. The dura-mater, the subarachnoid space and the medulla were easily detected in the spinal canal after laminectomy. Intra operative real time ultra sonography performed on the dura-mater or on the cortex allows th

Pediatric encephalic ultrasonography: the essentials.

Nowadays, cranial ultrasonography (US) of the newborn represents the first imaging method in brain damage study and its possible outcomes. This exam is performed using the natural fontanelles, especially the anterior one. It is fast, non-invasive and does not produce any side effect. Ultrasonographic examination is usually performed in cases of prematurity, especially in children with birth weight less than 1500 g, because important informations about the possible presence of pathologies such as cerebral hemorrhage and hypoxic-ischemic encephalopathy are given. This approach can be useful also

[Skull fracture and cephalhematoma in a newborn--a case report].

Skull fractures and cephalhematoma in newborns belong to the group of birth injuries of the head and neck, accounting for 11.4 to 15% of the total number of birth injuries. We presented a fracture of the parietal bones in a newborn, associated with parietal cephalhematoma. The newborn had transient and mild neurological symptoms: vomiting and changes in muscle tone. Clinical examination confirmed a positive "ping-pong" phenomen, craniogram confirmed the fracture of parietal bones and shown cephalhematoma, as well as ultrasound (US) examinations of the brain showed peri-interventricular bleedin

[Transcranial sonography in diagnosis of movement disorders].

Conventional methods of neuroimaging, such as computed tomography and magnetic resonance imaging, are mostly useful in exclusion of movement disorders with secondary aetiology. Recently, the application of transcranial sonography has been introduced to the diagnosis of extrapyramidal diseases. This valuable technique, with proven usefulness in the field of cerebrovascular events, may reveal some brain parenchymal structures. In this review the most important findings of ultrasonographic abnormalities in the brain, particularly hyperechogenicity of substantia nigra in Parkinson disease, are pre

Malformation of the fetal brain in thanatophoric dysplasia: US and MRI findings.

We present a case in which the unusual cerebral malformations of thanatophoric dysplasia (TD) were identified on a 21-week fetal US and confirmed by antenatal MRI, postmortem imaging and autopsy. TD is the most common lethal skeletal dysplasia and is characterized by short long bones, which are often bowed (type 1), a small thorax, and skull deformities. There is also a recognised constellation of abnormalities of the brain primarily affecting the temporal lobes that, although well described in the postmortem setting, are not widely recognized in fetal imaging. Familiarity with this appearance

[Ultrasound diagnosis of periventricular leukomalacia].

Periventricular leucomalacia (PVL) is the most common form of hypoxemic brain damage in infants. In PVL, there is usually a preceding history of asphyxia or hypoxia. This results in hypoxic-ischaemic damage to the region of the brain around the lateral ventricles, a particularly vulnerable area as it is in a border zone between arterial supplies. PVL is usually first recognised as an echodense heterogenic flare in the anterior and posterior periventricular areas on brain ultrasound scan. This must clearly be differentiated from the typical halo seen in newborns. The formation of pseudocystic p
