Ultrasonography for Band-Induced Small-Bowel Obstruction Associated With an Omphalomesenteric Duct Cyst in a Child: A Case Report.

Omphalomesenteric duct cysts are uncommon congenital anomalies and may present with nonspecific abdominal symptoms in children. We report a 6-year-old girl who presented with acute periumbilical pain and recurrent vomiting. Ultrasonography revealed an infraumbilical cystic lesion, dilated small-bowel loops, and a band-like echogenic structure that appeared to extend from the inferior aspect of the cyst toward an adjacent dilated bowel loop. Computed tomography confirmed small-bowel obstruction and provided a broader cross-sectional view of the infraumbilical cyst and the posterior course of th

Diagnosis of abdominal tuberculosis: sonographic findings in patients with early disease.

OBJECTIVE: The diagnosis of abdominal tuberculosis is often difficult, because clinical manifestations and results of laboratory studies are nonspecific. If sonographic findings are sufficiently characteristic for diagnosis, sonography would be useful, especially in India, where abdominal tuberculosis is common and more expensive imaging techniques are not easily available. Accordingly, we performed sonography to establish the sonographic findings in cases of early tuberculosis in 56 patients with abdominal tuberculosis who had normal barium studies of the small bowel. SUBJECTS AND METHODS: F

Omental calcification, necrotizing enterocolitis, and undescended testes after fetal abdomino-amniotic shunting performed for the management of meconium peritonitis: A case report.

Prenatal ultrasonography (US) in a 39 year-old woman revealed massive fetal ascites. A fetal abdomino-amniotic shunting procedure was performed. Subsequently, plain radiographs demonstrated diffuse gaseous distention of the bowel and multiple punctate calcifications in the left upper abdomen. Postnatal US examination showed multiple echogenic foci in the liver and the left upper abdomen, bowel wall thickening in the right-sided abdomen, and undescended testes. There was no intra-abdominal free air or loculated fluid collections. Medical management was instituted secondary to the clinical suspi

Congenital jejunal and ileal atresia: natural prenatal sonographic history and association with neonatal outcome.

OBJECTIVE: The purpose of this study was to describe the prenatal sonographic features and natural course of congenital jejunal and ileal atresia and correlate the findings with neonatal outcomes. METHODS: We identified all neonates with surgically confirmed jejunal or ileal atresia that had prenatal sonography and neonatal surgery in our center from January 1, 1995, to April 1, 2005. Sonography reports and images were reviewed, without knowledge of neonatal outcomes, for features of intestinal obstruction. Obstetric and neonatal outcomes were evaluated. RESULTS: Fifteen (60%) of 25 offsprin

Utility of minor ultrasonographic markers in the prediction of abnormal fetal karyotype at a prenatal diagnostic center.

OBJECTIVE: This study was undertaken to assess the value of minor ultrasonographic markers in predicting significant karyotypic abnormalities. STUDY DESIGN: A total of 2743 fetuses (14-24 weeks' gestation) prospectively underwent a detailed ultrasonographic survey before genetic amniocentesis. Criteria for 8 minor ultrasonographic markers were established. Odds ratios for significant karyotypic abnormalities in the presence of minor ultrasonographic markers were calculated with the chi(2) and Fisher exact tests. RESULTS: Of the fetuses, 14.6% had a single minor ultrasonographic marker, 2.1%

[Ultrasonography of acute diverticulitis of the colon].

Ultrasonographic findings in two patients with acute diverticulitis of the left colon are described. Echographic identification of diverticula and thickened (> 4 mm) hypoechoic bowel wall were common findings. The final diagnosis was confirmed by contrast enema and colonoscopy. Patients had clinical signs and symptoms of acute diverticulitis. Diverticula were visualized as echogenic shadowing foci in thickened bowel wall (1 case) or as outpouches of the intestinal wall in the pericolonic soft tissues (1 case). One patient had edema of the pericolic fat. Additional studies are needed to confirm

The echographic characteristics of fatty tissues and tumors.

Fatty tissues and tumors have been described as characteristically cyst-like on ultrasound examination, with relatively few internal echoes. The authors offer several examples of lipomatous masses and tissues which are markedly echogenic. In vitro studies suggest that this is an inherent property of body fat and is not due to coexistent fibrous tissue or tumor vascularity. The echogenicity of fat has important implications for diagnostic ultrasound: (a) the presence of fat in tissues and masses may be demonstrated by ultrasound, and (b) because the high-level internal echoes blend with the ech

Ultrasound pitfalls and artifacts related to six common fetal findings.

Routine use of ultrasound (US) in antenatal screening has increased over the past decade among both low- and high-risk pregnancies. Recognition of common US imaging pitfalls and artifacts associated with frequently encountered fetal anomalies on obstetric US is imperative to avoid misdiagnosis. The purpose of this article is to present practical tips on how to accurately diagnose 6 fetal anomalies including choroid plexus cyst, mild ventriculomegaly, echogenic intracardiac focus, prominent thymus, mild renal pelviectasis, and echogenic bowel. A suspected fetal anomaly should always be visualiz

Sonographic appearances of inverted Meckel diverticulum with intussusception.

This study illustrates the sonographic findings of inverted Meckel diverticulum acting as a lead point of an intussusception in five patients. In four patients, the inverted diverticulum was seen as a segment of blind-ending, thick-walled bowel projecting for a variable distance from the apex of the intussusceptum. The larger diverticula had a characteristic bulbous shape. The central serosal surface of the inverted diverticulum was filled with fluid in one patient, with fluid and fat in another, and with echogenic fat only in the other two. The presence of fat was confirmed by CT in one patie

Cystic fibrosis presenting as fetal anemia. A case report.

BACKGROUND: Cystic fibrosis in the fetus may present with meconium ileus, seen as dilated, hyperechogenic bowel on the prenatal ultrasound examination. Rarely does in utero bowel perforation occur with resulting peritoneal echogenicity, ascites or lack of bowel dilation. CASE: In a case of cystic fibrosis in a fetus, all three findings above occurred, as did anemia, most likely secondary to bleeding from perforated bowel. The anemia was detected following the finding of significantly elevated peak flow velocity in the middle cerebral artery (MCA). CONCLUSION: Fetuses with cystic fibrosis can

Inflammatory fibroid polyp presenting with intestinal invagination: sonographic and correlative imaging findings.

We report a case of a large inflammatory fibroid polyp of the small bowel presenting with intussusception in a 22-year-old man. Sonography demonstrated a solid, mobile, homogeneous, echogenic mass surrounded by the typical mural layers of an invaginated ileum. CT demonstrated a well-defined intraluminal solid mass with an attenuation of 17 HU. The pathologic diagnosis after segmental ileal resection was ileal inflammatory fibroid polyp.

Submucosal hyper-echogenicity on intestinal ultrasound is associated with fat deposition and predicts treatment non-response in patients with ulcerative colitis.

BACKGROUND AND AIMS: The submucosa is the most responsive bowel wall layer on intestinal ultrasound (IUS) when assessing treatment response in ulcerative colitis (UC). Submucosal thickening with hyper-echogenicity is observed. This study aimed to quantify echogenicity and understand transmural changes in UC. METHODS: In total, 118 patients were studied in two cohorts. Cohort 1 included colectomy patients: 19 UC patients and 52 controls without inflammatory bowel disease. Cohort 2 included 47 UC patients in a prospective cohort starting anti-inflammatory treatment. In Cohort 1, submucosal infl

Prenatal ultrasonographic gastrointestinal abnormalities in fetuses with gastroschisis do not correlate with postnatal outcomes.

PURPOSE: In the setting of gastroschisis, the clinical significance of prenatal ultrasound findings of secondary changes in bowel appearance remains unknown. The purpose of this study was to correlate prenatal identification of additional gastrointestinal sonographic abnormalities with postnatal clinical outcome. METHODS: A retrospective review was conducted on 64 fetuses with a prenatal diagnosis of gastroschisis treated at the Children's Hospital of Philadelphia from 2000 to 2007. Postnatal outcomes were compared between newborns with additional sonographic gastrointestinal abnormalities an

From ruptured ovarian cyst to ileo-ileal knotting: a case of diagnostic dilemma.

Ileo-ileal knotting is a rare, life-threatening cause of small bowel strangulation. Preoperative diagnosis is challenging, and hemorrhagic ascites in reproductive-age women may mimic gynecologic emergencies. A 33-year-old Ethiopian female presented with sudden abdominal pain, vomiting, obstipation, and hemorrhagic shock. Ultrasonography showed dilated small bowel loops and echogenic free fluid, suggesting ruptured ovarian cyst. Due to instability, computed tomography was omitted. Emergency laparotomy revealed 2000 ml of hemorrhagic ascites with normal ovaries and no gynecologic bleeding. An il

Bayesian analysis for cystic fibrosis risks in prenatal and carrier screening.

PURPOSE: Risk assessment is an essential component of genetic counseling and testing, and Bayesian analysis plays a central role in complex risk calculations. We previously developed generalizable Bayesian methods to calculate the autosomal recessive disease risk of a fetus when one or no mutation is detected, and another, independent risk factor is present. Our methods are particularly useful for calculating the CF disease risk for a fetus with echogenic bowel. In genetics practice, however, there are other scenarios for which our previous methods are inadequate. METHODS AND RESULTS: We prov

Tube within tube: Ascaris in bowel and biliary-tract.

Ascaris lumbricoides is one of the most common human helminthic diseases worldwide. On ultrasound, it is seen as linear non-shadowing echogenic structures with target appearance in cross section, and the live worm may show writhing movements in real time. On barium meal follow through, it appears as radiolucent tubular filling defects within the bowel lumen. Though not sensitive, direct real-time visualization of Ascaris on ultrasound is quick, non-invasive, and definitive.

Multiple gastrointestinal atresias: sonography of associated biliary abnormalities.

The pre-operative finding of biliary ductal dilatation in the setting of multiple gastrointestinal atresias has not been previously reported. This paper illustrates the sonographic findings in two neonates with multiple gastrointestinal atresias from the prepyloric region to the rectum. Biliary ductal dilatation was seen in both patients and highly echogenic, mobile intraluminal bowel debris secondary to calcified meconium was evident in one. Post-operative biliary gas was demonstrated on sonography in one case.
