Graduate education in general surgery and its related specialties and subspecialties in the United States.

Each year, approximately 1,000 graduating medical students enter training in general surgery and its related specialties and subspecialties in the United States. Traditionally, residents who want to practice vascular surgery, plastic surgery, thoracic surgery, and other specialties and subspecialties derived from general surgery have been required to complete five years of training in general surgery before embarking on further training. However, three phenomena have recently emerged that are changing the picture of surgical training: (1) proliferation of fellowships in subspecialties of gener

A follow-up study of recent special education graduates of learning disabilities programs.

This study investigated the postschool vocational and community adjustment of recent special education graduates from a southwestern metropolitan school district. Sixty-four students with learning disabilities (LD) who had attended self-contained classes at 12 high schools were randomly selected. A questionnaire, administered either via the telephone or in person, was the source of data. Thirty-eight of the LD sample were in competitive employment. In general, the LD students received very few community services. For the most part, they received no agency support after graduation. Most of the

Pandemic to endemic: a longitudinal multinational study on COVID-19's effects on graduate medical education.

BACKGROUND: This study applied a resilience engineering framework to understand how international graduate medical educations (GME) programs responded to the disruptions caused by the COVID-19 pandemic. Beyond describing early pandemic disruptions, we offer a multi-year, multi-country analysis of how training functions adapted, which elements durably changed, and what program design features may be associated with recovery. METHODS: We conducted a retrospective longitudinal study of all 180 ACGME International (ACGME-I)-accredited programs across seven countries. Annual surveys were completed

A systematic review of intellectual and developmental disability curriculum in international pre-graduate health professional education.

BACKGROUND: Despite the increasing global population of individuals with intellectual and developmental disabilities (IDD), this population remains especially vulnerable to health disparities through several factors such as a lack of access to sufficient medical care and poor determinants of health. To add, numerous studies have shown that healthcare professionals are still insufficiently prepared to support this population of patients. This review synthesizes the literature on current pre-graduate IDD training programs across healthcare professions with the goal of informing the creation of e

Lurking in the shadows: time as an actor in the narratives of pregnancy within graduate medical education.

Previous studies suggest that time factors heavily into trainees' decision-making around issues of pregnancy within graduate medical education (GME). Trainees are often dissatisfied with parental leave policies and tension exists as they navigate training interruptions. However, our understanding is limited because prior studies have conceptualized only two main actors involved in this negotiation, program directors (PDs) and trainees. This study aimed to understand the role of a third ever present character that has been overlooked - Time. We recruited 13 pairs of residents/fellows who had be

Humour in clinical-educational interactions between graduate student clinicians and people with aphasia.

BACKGROUND: During clinical interactions, clinicians and people with aphasia (PWA) use humour and laughter for a range of purposes, most of which contribute to friendly interactions in which the participants appear to develop a positive regard for one another. Moreover, humour is a vital component of facework, or the processes interactants engage in to protect their own and one another's well-respected, public personas. AIMS: To examine the ways in which speech-language pathology graduate student clinicians enlist humour during one-on-one therapy sessions for PWA. METHODS & PROCEDURES: Three

Integrating Artificial Intelligence Competencies Across the Six ACGME Core Competencies: A Proposed Framework for Graduate Medical Education.

Although Artificial Intelligence (AI) is increasingly embedded in clinical practice and Graduate Medical Education (GME), AI-specific competencies are not yet explicitly represented as a distinct domain within the Accreditation Council for Graduate Medical Education's (ACGME) six core competency framework. The ACGME's ongoing consideration of AI in its Common Program Requirements revision process signals an emerging recognition of the need to address this gap. Over 1200 FDA-authorized AI medical devices, unregulated generative AI tools in clinical workflows, and a proliferating state legislati

Beyond the Leash: Exploring the Public Health Dimension of Facility Dog Handler Experiences in Military Graduate Education.

Facility dogs are increasingly incorporated throughout higher-education institutions as integrative conduits for supporting students by reducing stress, anxiety, and emotional exhaustion. While these programs are designed to enhance student well-being, little is known about the experiences of the handlers who train, care for, and work with these dogs-specifically within military-affiliated programs. The handlers' role necessitates a unique dynamic of caregiving, emotional labor, and professional responsibility and management that can create role conflict and additional stress among those gradu

Mapping Diversity, Equity, and Inclusion Statements in Graduate Medical Education: Protocol for a Scoping Review Across Specialties.

BACKGROUND: Persistent disparities in diversity, equity, and inclusion (DEI) remain across US residency programs despite recent accreditation requirements. In this context, DEI disparities refer to inequities in the representation, recruitment, advancement, and educational experiences of individuals from underrepresented and marginalized groups. Numerous specialty-specific studies have evaluated DEI content on residency program websites, but these studies differ in their methodologies, assessed content, and reporting practices. There is currently no comprehensive synthesis of how DEI content i

Residency training in physiatry during a time of change: funding of graduate medical education and other issues.

Decision makers at the federal and state level are considering, and some states have enacted, a reduction in total United States residency positions, a shift in emphasis from specialist to generalist training, a need for programs to join together in training consortia to determine local residency position allocation strategy, a reduction in funding of international medical graduates, and a reduction in funding beyond the first certificate or a total of five years. A 5-page, 24-item questionnaire was sent to all physiatry residency training directors. The objective was to discern a descriptive
