Food-specific IgG-based elimination diet decreased IL-6, TNF-α, and CGRP and improved symptoms in adults with migraine.

BACKGROUND: Food-specific IgG antibodies have been proposed to be biomarkers to identify food that triggers an inflammation response. We aimed to evaluate the effect of a food-specific IgG-based elimination diet by assessing the changes in the symptoms of migraine and its comorbidities, inflammatory cytokines, neuropeptides, and neurotransmitters, and their correlation. METHODS: This was a sham-controlled randomized trial. A total of 98 patients with migraine who had at least one positive food-specific IgG antibody were randomly assigned to either the true diet group (IgG-positive foods were

One-food versus six-food elimination diet therapy for the treatment of eosinophilic oesophagitis: a multicentre, randomised, open-label trial.

BACKGROUND: Empirical elimination diets are effective for achieving histological remission in eosinophilic oesophagitis, but randomised trials comparing diet therapies are lacking. We aimed to compare a six-food elimination diet (6FED) with a one-food elimination diet (1FED) for the treatment of adults with eosinophilic oesophagitis. METHODS: We conducted a multicentre, randomised, open-label trial across ten sites of the Consortium of Eosinophilic Gastrointestinal Disease Researchers in the USA. Adults aged 18-60 years with active, symptomatic eosinophilic oesophagitis were centrally randoml

Diagnosis of Food Protein-Induced Enteropathy Based on Gastrointestinal Mucosal Pathology before and after Elimination Diet Therapy: A Case Report.

We describe the case of a 1-year-old girl with food protein-induced enteropathy (FPE) that was difficult to diagnose. She was referred to our hospital with a 3-month history of diarrhea, vomiting, and weight loss. Although her diarrhea improved after a few days of fasting, oral intake of elemental diets, formula milk, or rice porridge resulted in repeated relapses. The serum IgE level was 1028 IU/mL, and radioallergosorbent tests were positive for milk, casein, alpha-lactalbumin, and other allergens. A histopathology of the duodenal mucosa revealed loss of mucosal villous structure, crypt hype

Pediatric eosinophilic esophagitis effectively treated with a short-term 6-food-group elimination diet and reintroduction therapy: A case report.

RATIONALE: Eosinophilic esophagitis (EoE) is an inflammatory disease diagnosed based on clinical symptoms and pathological findings. EoE is treated with proton pump inhibitors (PPIs), topical steroids, and elimination diet-reintroduction therapy. After remission is achieved with the elimination diet, foods can be reintroduced sequentially to identify specific food triggers; however, this reintroduction method was not previously standardized. PATIENT CONCERNS: A 12-year-old girl presented to our hospital with a 3-year history of epigastric pain. Esophagogastroduodenoscopy revealed linear furro

Clinical, histopathological features and efficacy of elimination diet and proton-pump inhibitor therapy in achieving histological remission in Asian children with eosinophilic gastritis.

AIM: Paediatric eosinophilic gastritis (EG) is a rare disorder and existing literature on diagnostic criteria and management remains lacking. We aim to describe the clinical spectrum and assess the efficacy of dietary elimination and proton-pump inhibitor (PPI) therapy, with particular emphasis on histologic remission in children with primary EG. METHODS: We performed a retrospective study of patients aged 0-18 years diagnosed with EG at a single centre in Singapore from 2013 to 2021. EG was diagnosed based on histological criteria of infiltration of >30 eosinophils per high-power film (HPF)

[Acquired IGE Mediated Milk Protein Allergy in Children with Atopic Diseases, after Cow`s Milk Elimination Diet - A Case Series Presentation].

Approximately 30% - 40% of children with moderate-to-severe atopic dermatitis (AD) have IgE-mediated food allergy (FA). Although food avoidance may improve AD, avoidance diets do not cure AD and may even have detrimental effects such as progression to immediate-type allergy including anaphylactic reaction. An avoidance diet is only indicated in patients clearly identified as food allergic by an appropriate diagnostic food test and oral food challenge test. Eosinophilic esophagitis (EOE) patients have a history of atopic disease. EOE symptoms are characterized depending on the age of the patien

Effects of Online, Asynchronous Education Modules on Migraine Severity and Elimination Diet Use Among Higher Education Students: An Observational, Pilot Feasibility Study.

Background/Objectives: Migraine is a debilitating neurologic disorder with diet-related triggers. No studies exist on education on migraine in conjunction with an elimination diet as a non-pharmacologic management approach. Methods: Higher education students who self-reported migraine were enrolled in this observational, pilot feasibility study. At baseline, participants completed questionnaires on demographics, migraine disability, and their understanding of migraine and an elimination diet. After one month of self-paced, asynchronous, online modules, participants were reassessed on their und

Dietary approaches to treat MS-related fatigue: comparing the modified Paleolithic (Wahls Elimination) and low saturated fat (Swank) diets on perceived fatigue in persons with relapsing-remitting multiple sclerosis: study protocol for a randomized controlled trial.

BACKGROUND: Fatigue is one of the most disabling symptoms of multiple sclerosis (MS) and contributes to diminishing quality of life. Although currently available interventions have had limited success in relieving MS-related fatigue, clinically significant reductions in perceived fatigue severity have been reported in a multimodal intervention pilot study that included a Paleolithic diet in addition to stress reduction, exercise, and electrical muscle stimulation. An optimal dietary approach to reducing MS-related fatigue has not been identified. To establish the specific effects of diet on MS

Evaluating the behavior, growth performance, immune parameters, and intestinal morphology of weaned piglets after simulated transport and heat stress when antibiotics are eliminated from the diet or replaced with L-glutamine.

Study objectives were to evaluate the effects of post-weaning transport during heat stress (HS) and thermoneutral (TN) conditions when dietary antibiotics are removed or replaced with a nutraceutical. Sixty mixed sex piglets from 10 sows ( = 6 piglets/sow) were weaned (18.8 ± 0.8 d of age) and then herded up ramps into 1 of 2 simulated transport trailers in either TN (28.8 ± 0.2°C) or HS (cyclical 32 to 37°C) conditions where they remained for 12 h. During the 12 h of simulated transport, fans were used to simulate air movement through the trailer, feed and water were withheld, and rectal temp

From Diet to Dysbiosis: The Microbiome's Relationship to Eosinophilic Esophagitis.

PURPOSE OF THE REVIEW: Eosinophilic esophagitis is a chronic, type 2 inflammatory disease of the esophagus. Diet elimination trials suggest that EoE is triggered by food antigens, however, incomplete response rates present gaps in our understanding. This review aims to synthesize current evidence surrounding the diet-microbiome-immune axis in EoE and propose a conceptual framework for how dietary interventions may influence disease beyond antigen elimination. RECENT FINDINGS: Many studies on the EoE microbiome highlight descriptive differences in cross-sectional cohorts, although findings rem

Dietary Therapy for Eosinophilic Esophagitis: Elimination and Reintroduction.

Eosinophilic esophagitis (EoE) is a food antigen-mediated disorder of the esophagus characterized by eosinophil predominant inflammation and symptoms of esophageal dysfunction. Dietary antigen elimination induces clinical and histological remission in patients with EoE. The most restrictive of elimination diets (the elemental diet) removes all possible food antigens while empiric elimination diets remove all (or a subset) of food antigens most commonly reported to cause esophageal eosinophilia and food allergies (milk, egg, wheat, soy, peanuts, tree nuts, fish, or legumes). Elimination diets a

Dietary Therapy in Eosinophilic Esophagitis (EoE): A Practical Approach to Elimination.

PURPOSE OF REVIEW: To provide an overview of current dietary elimination strategies for eosinophilic esophagitis (EoE) and propose a practical, patient-centered approach to guide dietary therapy. RECENT FINDINGS: Empiric elimination diets, including one- (1FED), two- (2FED), four- (4FED), and six-food (6FED) elimination strategies, have demonstrated varying efficacy, with broader elimination generally associated with higher histologic remission rates. Elemental diets remain the most effective but are often impractical. Allergy testing-directed diets have limited utility due to poor predictive

Rethinking Restrictive Diets in IBD: Toward an Inclusion-Focused Nutritional Paradigm.

PURPOSE OF REVIEW: Patient-initiated elimination diets are widely adopted by those with inflammatory bowel disease (IBD), yet the evidence supporting their long-term use remains limited and their risks are increasingly recognized. Some common elimination diets utilized by patients include the specific carbohydrate diet (SCD), low-FODMAP diet, and gluten-free and dairy-free diets. This narrative review critically examines the role of self-initiated restrictive dietary strategies in IBD management, evaluating the evidence for and against common elimination-based approaches, and contrasts these w

Dietary Approaches in Eosinophilic Esophagitis.

Eosinophilic esophagitis (EoE) is a chronic, immune-mediated, non-IgE-mediated food allergy and a leading cause of dysphagia and food impaction in adults, with rising prevalence worldwide. Diagnosis requires ≥15 eosinophils per high-power field on esophageal biopsy after exclusion of other causes of gastrointestinal eosinophilia. Food allergens, principally milk, wheat, eggs, legumes and seafood, drive the inflammatory process, and treatment options include proton pump inhibitors, topical corticosteroids, biologic agents and endoscopic dilation for stricturing disease. Among these, dietary the

Development of a Practical Guide to Implement and Monitor Diet Therapy for Eosinophilic Esophagitis.

Dietary therapy for short- and long-term management of eosinophilic esophagitis is an effective yet poorly understood and underutilized treatment strategy. Despite several prospective trials demonstrating the efficacy of dietary therapies, successful clinical implementation is hampered by the need for a multidisciplinary approach including dietitian support and provider expertise. The availability of these resources is not readily available to most gastroenterologists. Without standardized guidance on starting or completing the diet for gastrointestinal providers and/or consulting dietitians,

Dietary strategies for chronic spontaneous urticaria: an evidence-based review.

Although the relationship between diet and chronic spontaneous urticaria (CSU) remains elusive, several patients seek dietary modifications as they are easy and cost-effective. Adequate patient education and counseling are crucial as modified diets may be beneficial for a subset of antihistamine refractory CSU patients, and no modality currently exists to identify these patients. Elimination of food items based exclusively on patient history may lead to unnecessary restrictions in most cases resulting in nutritional deficiencies and impaired quality of life. Several dietary strategies have bee

Carotenemia and hepatomegaly in an atopic child on an exclusion diet for a food allergy.

Eczema is a frequent childhood manifestation and a few atopic children are allergic to certain foods or aeroallergens. Anxious parents of atopic children often have a fear of topical steroid-related side-effects, and some may try a range of elimination diets to avoid allergies. Elimination diets increase the risk of anaphylaxis on re-exposure to previously tolerated foods from the loss of oral tolerance. Unbalanced diets together with an inadvertent excessive consumption of fruits and vegetables may lead to carotenemia from the carotenoids in the plant foods. Carotenemia is benign but unusual
