Public access defibrillator locations and details in Forsyth County, North Carolina: Results of a telephone survey canvas of large businesses to facilitate out-of-hospital cardiac arrest treatment.

BACKGROUND: Limited data are available on locations of public access defibrillation programs across communities in the United States, despite their widespread presence. Our goal was to determine publicly available AED locations of large businesses in a mixed urban-rural county. We then compared our survey results to a NC state-mandated AED registry and the county's emergency medical dispatch center AED registry. METHODS: We conducted structured phone surveys of all large businesses (>19 employees) and select small businesses (healthcare, government, childcare, educational, and religious organ

Prehospital time indicators before and after the implementation of an electronic information management system (EIMS): a cross-sectional study.

BACKGROUND: The prehospital emergency system faces significant challenges, including a lack of coordination, primarily due to poor communication of information. An electronic information management system (EIMS) was introduced in Iran to improve coordination. OBJECTIVES: This study aims to assess the impact of the system on the time indicators in prehospital emergency services in Kermanshah, Iran. METHODS: In this retrospective cross-sectional study conducted in 2022, time indicators were compared for 900 PCRs: 450 from the paper-based system (2016–2017) and 450 from the electronic PCR system

Image transmission and video live-streaming in emergency medical communication centre: a narrative review of its feasability, use, and impact.

INTRODUCTION: Emergency Medical Communication Centres (EMCC) are responsible for assessing calls, providing pre-arrival instructions, and dispatching resources. Telephone-based triage is limited by the lack of visual information, which may lead to under-triage or over-triage. Telemedicine, including image transmission and video live-streaming has emerged as a complementary tool, but its actual impact in EMCC remains uncertain. We aimed to summarise the existing literature on the feasibility, use and impact of telemedicine using image transmission or video live-streaming in EMCCs. METHODS: We

Contemporary Prehospital Emergency Medical Services Response Times for Suspected Stroke in the United States.

BACKGROUND AND PURPOSE: There are no contemporary national-level data on Emergency Medical Services (EMS) response times for suspected stroke in the United States (US). Because effective stroke treatment is time-dependent, we characterized response times for suspected stroke, and examined whether they met guideline recommendations. METHODS: Using the National EMS Information System dataset, we included 911 calls for patients ≥ 18 years with an EMS provider impression of stroke. We examined variation in the total EMS response time by dispatch notification of stroke, age, sex, race, region, tim

From Single Impact Sites to Dispersed Corridors: A Disaster Report of Emergency Medical Services Command Adaptation to Cluster-Munition Pattern Missile Strikes in Israel.

This Disaster Report describes a series of missile impact events managed by Magen David Adom (MDA), Israel's national Emergency Medical Services (EMS) organization, during the March 2026 missile campaign. The report focuses on a recurring operational distinction between compact single-warhead impact events and dispersed cluster-munition pattern events. Unlike routine mass-casualty incident recognition, MDA dispatch did not need to infer the possibility of a dispersed event only from accumulating emergency calls. While missiles are in flight, the Home Front Command (HFC) broadcasts real-time th

Reorganisation of the Basic Life Support Segment in a Physician-Led Emergency Medical Service: A Retrospective Evaluation of Operational and Clinical Characteristics.

Background: Increasing demand for emergency medical services (EMS) challenges prehospital resource availability, particularly for time-critical emergencies requiring advanced life support (ALS). Although tiered BLS and ALS response models are established in many non-physician-led EMS systems, evidence regarding qualification-based dispatch stratification within physician-led EMS systems remains limited. This study evaluated the clinical and operational characteristics of an expanded BLS dispatch segment in Berlin EMS. Methods: A retrospective observational study analysed 2,132,246 EMS missions

An analysis of the relationship between the applied medical rescue actions and the return of spontaneous circulation in adults with out-of-hospital sudden cardiac arrest.

Sudden cardiac arrest (SCA) is a significant medical and social issue, the main cause of death in Europe and the United States. The aim of the research was to evaluate the effectiveness of emergency medical procedures applied by emergency medical teams in prehospital care in the context of return of spontaneous circulation (ROSC).The case-control study was based on the medical documentation of the Rescue Service in Katowice (responsible for monitoring 2.7 million inhabitants of the region) referring to 2016. The research involved exclusively adults (ie, individuals older than 18 years) with out

Design and rationale of the TANGO2 trial: A randomized controlled non-inferiority trial of bystander compression-only CPR versus standard CPR in out-of-hospital cardiac arrest.

RATIONALE: Bystander cardiopulmonary resuscitation (CPR) before emergency medical services arrival is associated with improved survival after out-of-hospital cardiac arrest (OHCA). Compression-only CPR (CO-CPR) is simpler to teach and perform, eliminates interruptions in chest compressions for rescue breaths, and has been associated with higher bystander CPR rates. For adults with OHCA of presumed medical etiology, clinical equipoise remains regarding the optimal CPR strategy for previously trained bystanders. PRIMARY HYPOTHESIS: Bystander CO-CPR is non-inferior to standard CPR (S-CPR) for 30

Recognition of visual symptoms in stroke: a challenge to patients, bystanders, and Emergency Medical Services.

BACKGROUND: Identification of visual symptoms as a sign of acute stroke can be challenging for both first line healthcare professionals and lay persons. Failed recognition of visual symptoms by medical dispatchers at the Emergency Medical Dispatch Center (EMDC-112) or personnel at the Out-of-Hours Health Service (OOHS) may delay stroke revascularization. We aimed to identify correct system response to visual symptoms in emergency calls. METHODS: Phone calls from patient or bystander to the EMDC-112 or OOHS, which included visual symptoms on patients later verified with stroke/Transient ischem

Prehospital suicide mortality in the Lake Van basin: rural-urban disparities from three-year EMS dispatch surveillance in eastern Türkiye.

BACKGROUND: Eastern Türkiye records high suicide mortality; however, prehospital emergency medical services (EMS) dispatch records from this region have not been systematically evaluated as a surveillance source. We aimed to describe EMS-attended suicide and self-harm cases in Van Province and to quantify the association between rural residence and prehospital suicide mortality. METHODS: We conducted a retrospective population-based surveillance study of all 'suicide'-coded records in the Van Province 112 EMS database (ASOS) from January 2023 to December 2025. After excluding duplicates and m

Influence of different relationships of bystanders to out-of-hospital cardiac arrest patients on the effectiveness of dispatcher-assisted CPR.

BACKGROUND: Dispatcher-assisted cardiopulmonary resuscitation (DA-CPR) significantly improves CPR performance by bystanders, increasing the survival of patients. However, emotional stress among family members witnessing out-of-hospital cardiac arrest (OHCA) may hinder their ability to effectively perform CPR, leading to delayed and poorer quality CPR. The influence of the relationships of callers to patients (i.e., family members, friends, and strangers) on DA-CPR outcomes remains insufficiently explored. MATERIALS AND METHODS: This retrospective observational study analyzed the data of nontr

Recognition of stroke symptoms indicative of anterior circulation large-vessel occlusion via telephone and video calls: a simulation study.

BACKGROUND: Identifying suspected anterior circulation large-vessel occlusion (aLVO) strokes during emergency calls could enhance dispatch efficiency, particularly in rural areas. However, data on emergency medical dispatchers' (EMDs) ability to recognize aLVO symptoms remain limited. This simulation study aimed to evaluate the feasibility of identifying side-specific arm paresis, side-specific conjugate eye deviation (CED), and aphasia during emergency calls by instructing layperson callers to perform brief, standardized examination steps. Two communication methods were compared: (1) telephon

Analysis of multiple casualty incidents - a prospective cohort study.

BACKGROUND: Our aim was to report the rate and causes for multiple casualty incidents (MCI) to analyse the prehospital part of responding to MCIs, report mortality and find areas for improvement. METHODS: A prospective cohort study conducted in an urban emergency medical service (EMS) between 1.3.1998 and 28.2.2004. RESULTS: Fifty-nine MCIs involving 263 patients (167 walking, 96 non-walking) occurred. The incidence of MCIs was 1.8/100,000 inhabitants year(-1). Traffic accidents were the most common cause followed by residential fires, intoxications and stabbings or shootings. Early MCI alar

To see or not to see--better dispatcher-assisted CPR with video-calls? A qualitative study based on simulated trials.

BACKGROUND: Video communication through mobile telephone is now available in many parts of the world. We ask how mobile phone video-calls compares with traditional phone calls for dispatcher-assisted cardiopulmonary resuscitation (T-CPR). METHODS: Primary data was collected through individual interviews with six dispatchers after their participation in simulated cardiac arrest. They had 10 scenarios each, during which they guided rescuers on resuscitation. During half of the scenarios they used video-calls, and traditional phone calls for the rest. Concepts from modern systems theory were use

Effect of the score predicting imminent delivery on the management of unexpected out-of-hospital obstetrical deliveries: a cluster randomized clinical trial.

BACKGROUND AND IMPORTANCE: Early identification of imminent deliveries is crucial for guiding the decision to dispatch emergency medical team to the prehospital setting. OBJECTIVE: To study whether the use of the score predicting imminent delivery (SPID) at the emergency call center reduces the risk of prehospital delivery occurring without the presence of a physician-staffed mobile ICU team. DESIGN, SETTING, AND PARTICIPANTS: Prospective multicenter cluster randomized controlled trial in 19 call centers in France. Calls from pregnant woman in labor with at least 33 weeks of amenorrhea were

Prehospital Care of Severely Intoxicated Patients by a Dutch Physician-Staffed Helicopter Emergency Medical Services: A Retrospective Study.

OBJECTIVE: Intoxicated patients are often encountered by emergency medical services (eg, in cases of recreational drug use, accidental ingestion, or inhalation of toxic substances or [attempted] suicide). Earlier research showed that a physician-staffed helicopter emergency medical service (P-HEMS) is regularly dispatched for intoxicated patients. However, it is still unclear if there is added value of P-HEMS compared with standard care provided by an ambulance crew. The aim of this study was to analyze the contribution of additional expertise and equipment that P-HEMS brings to the prehospita
