Prodromal complaints and 30-day survival after emergency medical services-witnessed out-of-hospital cardiac arrest.

BACKGROUND: Out-of-hospital cardiac arrest (OHCA) is a frequent and lethal condition with a yearly incidence of approximately 5000 in Denmark. Thirty-day survival is associated with the patient's prodromal complaints prior to cardiac arrest. This paper examines the odds of 30-day survival dependent on the reported prodromal complaints among OHCAs witnessed by the emergency medical services (EMS). METHODS: EMS-witnessed OHCAs in the Capital Region of Denmark from 2016-2018 were included. Calls to the emergency number 1-1-2 and the medical helpline for out-of-hours were analyzed according to th

Oligoanalgesia in Patients With an Initial Glasgow Coma Scale Score ≥8 in a Physician-Staffed Helicopter Emergency Medical Service: A Multicentric Secondary Data Analysis of >100,000 Out-of-Hospital Emergency Missions.

BACKGROUND: Oligoanalgesia, as well as adverse events related to the initiated pain therapy, is prevalent in out-of-hospital emergency medicine, even when a physician is present. We sought to identify factors involved in insufficient pain therapy of patients presenting with an initial Glasgow Coma Scale (GCS) score of ≥8 in the out-of-hospital phase, when therapy is provided by a physician-staffed helicopter emergency medical service (p-HEMS). METHODS: This was a multicenter, secondary data analysis of conscious patients treated in primary p-HEMS missions between January 1, 2005, and December

Service availability and readiness to provide comprehensive emergency obstetric and newborn care services in post-conflict at North Wollo Zone hospitals, Northeast Ethiopia: mixed survey.

INTRODUCTION: Countries with humanitarian crises and fragile conditions contribute to 61% of the global burden of maternal mortality. Emergency Obstetric and Newborn Care (EmONC) services reduce direct obstetric complications, which cause approximately 70-80% of maternal deaths and 10% to 15% of neonatal deaths. Therefore, this study was aimed to assess the service availability and readiness to provide comprehensive emergency obstetric and newborn care services in post-conflict at North Wollo Zone hospitals, Northeast Ethiopia. METHODS: A facility-based mixed cross-sectional study design was

Out-of-hospital deaths in Mongolia: a nationwide cohort study on the proportion, causes, and potential impact of emergency and critical care services.

BACKGROUND: Little is known about the proportion and causes of out-of-hospital deaths in Mongolia. In this study, we aimed to determine the proportion and causes of out-of-hospital deaths in Mongolia during a six-month observation period before the COVID-19 pandemic. METHODS: In a retrospective study, the Mongolian National Death Registry was screened for all deaths occurring from 01 to 06/2020. The proportion and causes of out-of-hospital deaths, causes of out-of-hospital deaths likely treatable by emergency/critical care interventions, as well as sex, regional and seasonal differences in th

Ambulatory care sensitive conditions at out-of-hospital emergence services in Croatia: a longitudinal study based on routinely collected data.

Conditions for which a hospital and emergency utilization can be considered avoidable are often referred as ambulatory care sensitive conditions (ACSCs). Until now, there has been no published research related to ACSCs in Croatia. This study was undertaken with the aim of determining the trends relating to ACSCs in out-of-hospital ES from 1995-2012. The study is based on data from the Croatian Health Service Yearbooks. Five chronic and three acute conditions were chosen: diabetes, hypertension, congestive heart failure, angina pectoris, asthma and COPD, bacterial pneumonia, urinary tract infec

Emergency Medical Services Outcomes Project (EMSOP) II: developing the foundation and conceptual models for out-of-hospital outcomes research.

Development of methodologically acceptable outcomes models for emergency medical services (EMS) is long overdue. In this article, the Emergency Medical Services Outcomes Project proposes a conceptual framework that will provide a foundation for future EMS outcomes research. The "Episode of Care Model" and the "Out-of-Hospital Unit of Service Model" are presented. The Episode of Care Model is useful in conditions in which interventions and outcomes, especially survival and major physiologic dysfunction, are linked in a time-dependent manner. Conditions such as severe trauma, anaphylaxis, airway

[Unusual attendance at Hospital Emergency Services for asthma and chronic obstructive pulmonary disease and SO2 air pollution in Cartagena (Spain)].

BACKGROUND: The objective of this investigation is to determine the influence of air pollution (SO2 and particulate matter) on the existence of days of unusual attendance at emergency services for asthma and chronic obstructive pulmonary lung disease (COPD). A registry of attendances for these diseases and the daily mean concentrations of SO2 and particles were used (1989-1991). RESULTS: Unusual attendance days are establish by the fortnightly movable mean of the period that has that day as central one, under the hypothesis of a Poisson distribution with equal mean as the calculated one. Emergency medical services agency-level variation in non-initiation of resuscitation and termination of resuscitation in out-of-hospital cardiac arrest.

BACKGROUND: Variations in the decisions to initiate and terminate resuscitation for out-of-hospital cardiac arrest (OHCA) likely differ across emergency medical services (EMS) systems. We evaluated EMS-agency variation in non-initiation of resuscitation for EMS-assessed OHCA and applying termination of resuscitation (TOR) for those meeting the Universal TOR rule. METHODS: We conducted a retrospective cohort study of patients with non-traumatic OHCA using the 2018-2024 ESO Data Collaborative. Our outcomes were non-initiation of resuscitation and TOR. For each outcome, we quantified the extent

Factors Influencing Hospitalization Among Older Adults in Tokyo's Emergency Medical Services: The Role of Cerebral Disease and Seasonal Variations.

BACKGROUND: The global acceleration of population aging, especially in developed countries, has led to increased demands on emergency medical services. In Tokyo, Japan's largest city, over half of all emergency transports involve older adults aged 65 and older, a figure that continues to rise annually. These patients often present with complex medical conditions, require intensive prehospital interventions, and have higher hospitalization rates compared to younger populations. Despite these trends, there is a scarcity of studies examining the characteristics of older adults patients transporte

Regional analgesia in the emergency department for hip fractures: survey of current UK practice and its impact on services in a teaching hospital.

INTRODUCTION: While the benefits of regional analgesia (RA) for preoperative pain relief in hip fracture (HF) in elderly patients are well recognised, this service is yet to be established in many UK emergency departments (EDs). We set out to discover how widely RA is adopted in the UK EDs. METHODS: In April-July 2010, we conducted a postal survey of 218 UK EDs, followed up with fax reminders for non-respondents. RESULTS: A total of 147/218 (67%) EDs completed the survey. Sixty five (44%) respondents reported use of RA blocks for HF. The commonest stated reasons for not using RA were lack of

Assessing the efficacy of the new protocol for chest compressions before definitive cardiac arrest in emergency medical service-witnessed adult out-of-hospital cardiac arrests.

AIMS: Japanese emergency medical services (EMS) personnel providing advance life support confirm the absence of a carotid pulse before initiating chest compressions (CCs) in adult out-of-hospital cardiac arrest (OHCA). This study aims to investigate the efficacy of a new protocol facilitating early CCs before definitive cardiac arrest in enhancing the outcomes of OHCA. METHODS: The 2011 new protocol facilitated EMS to initiate CCs when the carotid pulse was weak and/or <50/min in comatose adult patients with respiratory arrest (apnoea or agonal breathing) and loss of the radial pulse. During

The Potential Role of Nonclinical Patient Support Service Staff in Reducing Emergency Department Visits and Hospitalizations of Urban and Rural Patients of Health Centers Funded by the Health Resources and Services Administration.

PURPOSE: Payers such as Medicaid are concerned about high rates of potentially avoidable acute care utilization by beneficiaries called high-need, high-cost (HNHC) and frequently seen by Health Resources and Services Administration-funded health centers. Yet, health center HNHC patients have comparatively lower acute care utilization than patients of other primary care providers, possibly due to better access to nonclinical staff who provide patient support services, such as care coordination and self-management education. Furthermore, rural areas face provider shortages that may impact staffi

Validation of the CaRdiac Arrest Survival Score (CRASS) for predicting good neurological outcome after out-of-hospital cardiac arrest in an Asian emergency medical service system.

BACKGROUND: Survival with favorable neurological outcomes is an important indicator of successful resuscitation in out-of-hospital cardiac arrest (OHCA). We sought to validate the CaRdiac Arrest Survival Score (CRASS), derived using data from the German Resuscitation Registry, in predicting the likelihood of good neurological outcomes after OHCA in Singapore. METHODS: We conducted a retrospective population-based validation study among EMS-attended OHCA patients (≥18 years) in Singapore, using data from the prospective Pan-Asian Resuscitation Outcomes Study registry.
