Barriers and facilitators to using an app-based tool for suicide safety planning in a psychiatric emergency department: A qualitative descriptive study using the Theoretical Domains Framework and COM-B model.

BACKGROUND: Emergency departments (EDs) play a crucial role in suicide prevention, with evidenced-based safety planning interventions (SPI) playing an important role. Traditional safety plans, which are often paper based, come with challenges (i.e., not easily accessible, or easy to misplace). Digital safety planning tools offer an alternative mode of intervention delivery, but clinical implementation remains limited. Using behavioural change frameworks, we aim to provide insights into the factors influencing app adoption. These insights will be used as a knowledge base to design behavior chan

More Than Suicide: Mortality After Emergency Psychiatric Care and Implications for Practice.

BACKGROUND: Emergency departments (EDs) provide vital mental health services. ED patients with behavioral health presentations, particularly suicidal ideation, are at increased risk of death by suicide, medical illness, trauma, or overdose. Causes of death for patients who receive specialty emergency psychiatric services remain undescribed. OBJECTIVE: We describe the incidence and cause of death after care in a psychiatric emergency service (PES). METHODS: Mortality data were obtained for all adult patients treated in a safety net ED from April 2016 to June 2020. Causes of death were categor

Perspectives on Coordinating Health Services for Individuals with Serious Mental Illness - A Qualitative Study.

PURPOSE: Individuals with serious mental illness (SMI) might require coordinated health services to meet their healthcare needs. The overall aim of this study was to describe the perspectives of professionals (registered nurses, medical doctors, social educators, and social workers) on care coordination and measures to ensure proper and coordinated follow-up of the healthcare needs of individuals with SMI. More specifically, we investigated which measures are taken by employees in municipal health and care services to prevent the deterioration of health conditions and which measures are taken

[Bad or mad? Thinking in terms of 'bad or mad' and the (possible) prosecution of offenses committed by the mentally ill].

BACKGROUND: Psychiatrists working for the outreach emergency services of mental health institutions are regularly asked to assess whether a suspect being held at a police station should be charged and later prosecuted or be admitted to a mental institution. In other words, is the suspect 'suited for detention'. Often the representative of the judicial system and the psychiatrist from the emergency services then discuss whether the suspect would be better served by care under the mental healthcare system or under the criminal judicial system. Compulsorily admission to a mental health institutio

Effects of mandatory screening labs in directing the disposition of the apparently healthy psychiatric patient in the emergency department.

OBJECTIVE: To determine whether mandatory psychiatric admission laboratory tests yield results that change the disposition of a patient with primary psychiatric complaint from admission to a psychiatric service to admission to a medical service. METHODS: This was a single center retrospective cohort chart review study approved by the facility Institutional Review Board in which we used a records database maintained by the emergency department's social workers to access the records of every patient that presented to our emergency department with a psychiatric chief complaint between the dates

Mental health response to community disasters: a systematic review.

IMPORTANCE: Exposure to a disaster is common, and one-third or more of individuals severely exposed may develop posttraumatic stress disorder or other disorders. A systematic approach to the delivery of timely and appropriate disaster mental health services may facilitate their integration into the emergency medical response. OBJECTIVE: To review and summarize the evidence for how best to identify individuals in need of disaster mental health services and triage them to appropriate care. EVIDENCE REVIEW: Search of the peer-reviewed English-language literature on disaster mental health respon

On-scene crisis intervention: psychological guidelines and communication strategies for first responders.

Effective emergency mental health intervention for victims of crime, natural disaster or terrorism begins the moment the first responders arrive. This article describes a range of on-scene crisis intervention options, including verbal communication, body language, behavioral strategies, and interpersonal style. The correct intervention in the first few moments and hours of a crisis can profoundly influence the recovery course of victims and survivors of catastrophic events.

Psychiatric Emergency or General Emergency: Evolution or Involution? A Qualitative Study With Mental Health and Emergency Professionals.

The treatment of individuals with psychiatric disorders who visit the Emergency Department (ED) remains a significant issue within healthcare organisations. Over the past decades, various organisational solutions have been proposed, ranging from dedicated Emergency Departments to liaison mechanisms involving mental health nurses within EDs or direct access to acute units. On one hand, there are clinical and organisational needs pushing towards the creation of dedicated pathways; on the other hand, there are concerns that such solutions may be counterproductive and dangerous in terms of health

A study on recidivism in the psychiatric emergency room.

Our goal in this retrospective study was to assess empirical risk factors for repeat visits to the psychiatric emergency room. This information may be useful for targeted prevention and cost-effective service planning. Over a 7-month period, 400 (18%) of 2212 patients were repeat visitors, accounting for 36% of all visits. A diagnosis of a psychotic disorder at the first visit was a risk factor for a repeat visit, especially in young patients. Substance abuse, as suggested by positive urine toxicology, decreased the likelihood of recidivism, but positive toxicology screens in young schizophren

Health services use in women with a history of bulimia nervosa or binge eating disorder.

OBJECTIVE: The current study examined health services use during the past 12 months in a sample of young women with a history of an adolescent eating disorder (bulimia nervosa [BN] or binge eating disorder [BED]). METHOD: A community sample of 1,582 young women (mean age = 21.5 years) was classified, based on a screening interview (and, for eating disorder diagnosis, confirmatory diagnostic interview), into one of three groups: BN or BED (n = 67), other psychiatric disorder (n = 443), and no adolescent psychiatric disorder (n = 1,072). RESULTS: A history of BN/BED in adolescence was associat

Beyond the Emergency Department: Behavioral Health Urgent Care as a Child and Adolescent Mental Health Crisis Care Solution.

Pediatric mental health-related emergency department (ED) visits have increased, though EDs are often ill-equipped to deliver timely, developmentally appropriate care, leading to repeated utilization. In response, Northwell Health launched a pediatric Behavioral Health Urgent Care (BHUC) in 2017 as an intermediate level of care providing same-day assessment, crisis intervention, care coordination, and short-term bridging services for youth not requiring ED care. Evaluation showed effective ED diversion, improved follow-up and care coordination, and high parent satisfaction, supporting BHUCs as

Psychiatric Emergency Visits of Autistic Adults With or Without Documented Borderline Personality Disorder.

Autism and borderline personality disorder (BPD) are each associated with increased psychiatric emergency department (PsyED) use, yet the service impact of coexisting autism-BPD remains unclear. We conducted a retrospective chart review of 1027 PsyED visits by 345 autistic adults (2018-2020) at an urban Canadian psychiatric hospital to compare service patterns between those with and without a BPD diagnosis. Sociodemographics, presenting reasons, restraint use, and disposition were analyzed using mixed-effects logistic regression. BPD was documented in 33.5% of visits and in 11.0% of unique aut

Battered women: a medical problem requiring detection.

A one month study of the surgical and psychiatric service of a university hospital emergency room identified thirty-seven women coming for treatment as a result of physical abuse. Thirty-three of the thirty-seven women presented at the surgical services are represented 3.8 per cent of surgical admissions in the emergency room. These women came from all ages, social and ethnic groups. They usually presented with contusions or other injuries to the head and neck. The majority would not accept psychiatric treatment but were willing to discuss the problem with the emergency room physician if encou

The longitudinal dispositions of people diagnosed with adjustment or severe stress disorders.

BACKGROUND: Adjustment and stress-related disorders are prevalent among psychiatric service users. Despite their prevalence, little is known about their prognosis. To reduce that gap, the present article documents the service use and diagnostic outcomes of people with adjustment or stress-related disorders presenting at Singapore's largest psychiatric emergency department. METHODS: Administrative data from 2014 to 2021 was retrieved to follow a group of 683 service users whose first-ever psychiatric presentation in 2014 warranted a diagnosis of adjustment or stress-related disorder. People we

Mitral valve prolapse and panic attacks.

Patients complaining of recurrent spontaneous panic attacks (RSPA) are frequently seen in emergency rooms and psychiatric outpatient services. The symptomatology of RSPA suggests some underlying cardiac pathology. Recent publications in the psychiatric literature have reported a high incidence of mitral valve prolapse (MVP) in patients complaining of RSPA. In our study we report a 39% incidence of MVP in a psychiatric population with RSPA. The importance of this finding is yet to be determined.
