Aortic Valve Infective Endocarditis Complicated by Annular Abscess: Antibiotics in the Abscess Cavity.

OBJECTIVES: Infective endocarditis of the aortic valve complicated by annular abscess is a challenging problem and often requires patch reconstruction after surgical debridement of the abscess cavity. Filling the remaining cavity with antibiotics is advocated to prevent recurrent endocarditis. This study aimed at evaluating the role of local antibiotics in patients with aortic valve infective endocarditis complicated by annular abscess. METHODS: Between January 2012 and December 2021, all consecutive patients with aortic valve infective endocarditis complicated by annular abscess undergoing c

When Stroke Speaks First: A Case Report on the Hidden Face of Streptococcus mitis Endocarditis.

Multiple cerebral infarctions and intraparenchymal hemorrhages are typical complications of infective endocarditis, which can be easily misinterpreted as an indeterminate cardiac mass through non-invasive diagnostics. We present a case of a 60-year-old man with acute dysarthria, jaw clonic movement, and gaze deviation, resulting from multiple cerebral infarctions caused by infective subacute endocarditis by Streptococcus mitis, confirmed by histopathological examination after surgical heart valve resection. By means of this case, we remind clinicians of the importance of investigating infectiv

Non-ischemic rupture of the posteromedial papillary muscle due to Streptococcus agalactiae endocarditis.

UNLABELLED: Papillary muscle rupture is usually caused by myocardial infarction although rare cases of non-ischemic etiology have also been described. Among these, infective endocarditis represents an important cause. Herein, we report a case due to Streptococcus agalactiae involving the posteromedial papillary muscle. LEARNING OBJECTIVE: Non-ischemic papillary muscle rupture should be suspected when there is no evidence of atherosclerotic coronary artery disease. In the febrile patient, infective endocarditis should be considered in the differential diagnosis.

Contemporary Role of Positron Emission Tomography (PET) in Endocarditis: A Narrative Review.

Endocarditis, a serious infectious disease, remains a diagnostic challenge in contemporary clinical practice. The advent of advanced imaging modalities has contributed significantly to the improved understanding and management of this complex disease. 18F-fluorodeoxyglucose (18F-FDG) positron emission tomography (PET) imaging has shown remarkable potential in improving the diagnostic accuracy of endocarditis. In the update of the Modified Duke Criteria, in 2023, The International Society for Cardiovascular Infectious Diseases (ISCVID) Working Group recognized specific 18F-FDG PET/CT findings a

Procedures associated with infective endocarditis in adults. A case control study.

OBJECT: To assess the relative risk of infective endocarditis associated with various procedures and the protective efficacy of antibiotic prophylaxis by a case-control study. BACKGROUND: Recommendations for the prevention of infective endocarditis are based on the hypothesis of a relationship between procedures and infective endocarditis which is supported by anecdotal reports and data from experimental models. METHODS: Cases met the Von Reyn's diagnostic criteria modified with echocardiographic and macroscopic findings Controls were recruited from cardiology or medicinal wards. Cases (n =

Isolated pulmonary valve infective endocarditis in isolated congenital supravalvular pulmonary stenosis: a case report.

BACKGROUND: Isolated pulmonary valve stenosis, of which supravalvular pulmonary stenosis is a subcategory, rarely occurs in live births. Isolated pulmonary valve infective endocarditis is uncommon and typically arises in settings of recurrent transient bacteremia, immunodeficiency, or congenital heart disease. Any infective endocarditis carries high morbidity with complications commonly including congestive heart failure, acute renal failure, and septic emboli among others. Consistent with previously published cases of pulmonary valve infective endocarditis, these cases can be challenging to d

[Infectious endocarditis in non-addict patients without predisposing heart disease. Differential features].

INTRODUCTION AND AIMS: Although uncommonly, infective endocarditis in non-addict patients may involve people without predisponente heart disease. The aim of our study was to assess the clinical and prognostic features of this type of endocarditis and to compare them with those of the more common type of endocarditis with underlying lesion. METHODS: With this aim, we have reviewed 71 consecutive cases of non-addict infective endocarditis diagnosed in our hospital in the last 7 years; there was no preexisting cardiac lesion in 9 patients (13% of all endocarditis and 21% of native valve endocard

Leuconostoc species endocarditis in an intravenous drug user.

Infective endocarditis is a potentially lethal infection, which predominantly affects the atrioventricular valves. Rapid identification and management is critical to reduce morbidity and mortality in this patient population. Herein, we present a case of a 24-year-old man with Leuconostoc species infective endocarditis of the aortic valve. Disease course was complicated by several septic emboli to the brain, central retinal artery, and spleen. This case serves to remind clinicians that Leuconostoc species, which are typically not pathogenic to human species, can cause infective endocarditis in

[Left ventricular-right atrial shunt due to infective endocarditis--report of a case].

Acquired left ventricular-right atrial shunt is a very rare cardiac disease. Infective endocarditis, cardiac operative procedures, and thoracic trauma were reported as origins. We report a case of a patient with left ventricular-right atrial shunt due to infective endocarditis. A 53-year-old male who had aortic regurgitation due to infective endocarditis developed suddenly severe congestive heart failure. Two-dimensional and pulsed doppler echocardiography demonstrated left ventricular-right atrial shunt. Emergency operation was done. The fistula was found through the atrioventricular membrano

Isolated Pulmonary Valve Infective Endocarditis With Persistent Staphylococcus aureus Bacteremia and Rapid Clearance With Ertapenem Plus Cefazolin.

Infective endocarditis is an infection of the heart's native or prosthetic valves, often caused by bacteria such as Staphylococcus aureus. Although infective endocarditis most commonly affects the left heart, cases of right-sided infective endocarditis, involving structures like the tricuspid or pulmonary valves, are also noted. Isolated native pulmonary valve infective endocarditis is exceptionally rare. After suspicion, the diagnosis relies on clinical symptoms and signs, imaging, and microbiological evidence. We report an unusual case of isolated pulmonary valve infective endocarditis in a

Capnocytophaga canimorsus and infective endocarditis-making a dog's dinner of the aortic valve: a case report.

BACKGROUND: Capnocytophaga canimorsus, a bacterium found in the oral cavities of healthy cats and dogs, is rarely reported as a cause of infective endocarditis. In this report we describe such a case in a young, male dog owner who presented acutely unwell in heart failure. CASE SUMMARY: A 47-year-old male presented with a subacute onset of fever, night sweats, weight loss, dyspnoea, and peripheral oedema. On clinical examination typical features of infective endocarditis, heart failure, and aortic regurgitation were found. The patient had no conventional risk factors for infective endocarditi

Infective endocarditis in children with congenital heart disease: comparison of selected features in patients with surgical correction or palliation and those without.

The diagnostic and prognostic features of 44 episodes of infective endocarditis in 42 children with congenital heart disease were reviewed. Endocarditis occurred in 18 patients who had not had surgical correction or palliation of the defect (non-operated group). There were 26 episodes in 24 patients who had been treated surgically (operated group) (16 open and eight closed cardiac operations). Endocarditis occurred soon after open heart surgery in eight patients and as a late complication in the other 16. It recurred in two patients (operated group). Invasive monitoring and low cardiac output

[Trend and contributing factors of in-hospital deaths in patients with infective endocarditis over the last twenty years].

OBJECTIVES: To evaluate the changes in the clinical background to infective endocarditis and identify the contributing factors to in-hospital deaths over the last 20 years. METHODS: Seventy-five patients (mean age 48.2 +/- 24.0 years) with infective endocarditis treated between January 1984 and December 2003 at our hospital were evaluated retrospectively. The patients were divided into two groups (first decade, n = 26 and second decade, n = 49). RESULTS: The infection route was unknown in 65% of the patients, but the oral route was the most common known route (16.0%). Congenital heart diseas

First report of Sneathia sanguinegens together with Mycoplasma hominis in postpartum prosthetic valve infective endocarditis: a case report.

BACKGROUND: The presence of more than one bacterial agent is relatively rare in infective endocarditis, although more common in prosthetic cases. Molecular diagnosis from a removed heart tissue is considered a quick and effective way to diagnose fastidious or intracellular agents. CASE PRESENTATION: Here we describe the case of postpartum polymicrobial prosthetic valve endocarditis in a young woman. Sneathia sanguinegens and Mycoplasma hominis were simultaneously detected from the heart valve sample using broad range 16S rRNA polymerase chain reaction (PCR) followed by sequencing while cultur

Acute Mitral Regurgitation of Unknown Etiology Associated with Disseminated Intravascular Coagulation Eventually Diagnosed as Enterococcus Faecalis Infective Endocarditis by Mitral Valve Surgery.

BACKGROUND Infective endocarditis is prevalent worldwide and the modified Duke criteria have been used universally to diagnose this condition. However, making the correct diagnosis is rather difficult because the clinical presentation and findings of blood tests are non-specific. CASE REPORT A 70-year-old female complaining of dyspnea for 5 days with acute mitral regurgitation was transferred to our hospital. She had acute heart and respiratory failure and disseminated intravascular coagulation. Although infective endocarditis was suspected, repeated blood cultures and transesophageal echocard
