There are a variety of techniques available to investigate endolymph dynamics, primarily seeking to understand the cause of endolymphatic hydrops. Here we have taken the novel approach of injecting, via a glass micropipette, fluorescein isothiocyanate-dextran (FITC-dex) and artificial endolymph into scala media of anaesthetized guinea pigs, with subsequent imaging of the inner ear using Light Sheet Fluorescence Microscopy (LSFM) as a means to obtain highly resolved 3D visualization of fluid movements. Our results demonstrate endolymph movement into the utricle, semicircular canals and endolymp

Gentamycin or kanamycin have no effect on endolymph formation in experimental induced endolymphatic hydrops. The increase of endolymphatic volume after obliteration of endolymphatic duct and sac is not influenced by aminoglycosides as well as the decrease of DC-potential and the increase of sodium-activity in endolymphatic hydrops. The way of action of intratympanal injected gentamycin seems to be in a partial chemical induced labyrinthectomy.

To determine whether stria vascularis volume correlated with cochlear duct volume, 1,180 human temporal bones were examined. Fifty-eight ears in 44 persons showed the presence of endolymphatic hydrops. Strial atrophy was found in 143 ears in 83 persons, in five of which it was severe. Eight cases of endolymphatic hydrops occurred in the presence of mild to moderate strial atrophy. A decrease in cochlear duct volume usually accompanied severe strial atrophy. However, a case in which cochlear duct volume was normal in the presence of severe strial atrophy was seen. Cochlear duct volume does not

By means of human temporal bone slides we want to demonstrate that interruption of the so-called longitudinal endolymphcirculation causes a hydrops ahead of the block. This fact, proven by animal experiments, we observed in human specimens. Predominant locations are the anatomic narrows of the endolymphatic system: Isthmus of the endolymphatic duct, utriculoendolymphatic valve Bast, ductus reuniens. The different possible pathohistological data are: bony obliteration of the ductus endolymphaticus, embolic closure within the isthmic portion of the endolymphatic duct, blockage of the utriculoend

Although the endolymphatic sac (ES) is thought to be a primary site for endolymph volume regulation, we have limited knowledge of how it responds to volume and pressure changes. In a prior publication, we demonstrated changes of K(+), Na(+) and endolymphatic sac potential (ESP) resulting from volume injections into, and withdrawals from, scala media of the cochlea. In the present study, we compared the influence of injections into and withdrawals from scala tympani of the cochlea on the endolymphatic sac. It is assumed that similar pressure changes are induced in endolymph and perilymph of bot

CONCLUSION: Blockage of the endolymphatic duct is a significant finding in Meniere's disease. The position of the utriculo-endolymphatic valve (UEV) and blockage of the ductus reuniens in the temporal bones were not found to be directly indicative of Meniere's disease.
OBJECTIVE: Comparison of blockage of the longitudinal flow of endolymph between ears affected by Meniere's disease and normal ears.
METHODS: We examined 21 temporal bones from 13 subjects who had Meniere's disease and 21 normal temporal bones from 12 controls.
RESULTS: The endolymphatic duct was blocked in five (23%) ears aff

Thirty-nine guinea pigs were used for four groups of experiments: 1. sacculotomy only, 2. sacculotomy and simultaneous obliteration of the endolymphatic duct, 3. sacculotomy followed by obliteration of the endolymphatic duct, and 4. obliteration of the endolymphatic duct followed by sacculotomy. Sacculotomy alone caused only minimal cochlear pathology, whereas sacculotomy on hydropic ears produced severe atrophy of the organ of Corti and cochlear neurons as well as connective cells of the limbus. There was histological evidence that Reissner's membrane in hydropic ears was ruptured by the sacc

Ménière's disease is an inner ear disorder characterized by vertigo attacks, fluctuating and progressive hearing loss, tinnitus, and aural fullness in the affected ear. The pathophysiology of Ménière's disease remains elusive. Theories so far are anatomical variation in the size or position of the endolymphatic sac and duct, viral inflammation or autoimmune involvement of the sac, or a genetically determined abnormality of endolymph control. Animal studies on blocking the ductus reuniens and endolymphatic duct have produced hydrops in the cochlea, saccule, and utricle. Cone beam computed tomog

PURPOSE OF REVIEW: Visualization of endolymphatic hydrops using MRI has become a cutting-edge method not only for diagnosing Meniere's disease but also for pathophysiological elucidation of the disease. Here, we review recent advances in imaging analysis of the inner ear in Meniere's disease, which could provide additional information over previous findings.
RECENT FINDINGS: In addition to identification of endolymphatic hydrops on MRI, high-intensity signals in the perilymph, which represent alterations in vascular permeability in the blood-perilymph barrier (BPB), might be a key to elucidat

Endolymphatic sac-related surgeries are a series of effective procedures to treat patients with Ménière's disease. However, there is controversy since they were proposed in 1927. Although most studies have shown the effectiveness of endolymphatic decompression and endolymphatic shunting, the mechanisms of them are still unclear. Some studies suggested that these procedures work because the endolymphatic hydrops relieves after surgery. However, the new one proposed recently, endolymphatic duct blockage, seems to contradict to the relief theory. Therefore, this article reviewed the mechanisms of

The endolymphatic sacs are described in temporal bone specimens from a 31-year-old man with bilateral Mondini disorder. The ducts and sacs are thin-walled, cyst-like structures with complete absence of loose vascular perisac tissue, and are directly apposed to the bone of the vestibular aqueduct. Histological evidence of severe bone erosion is present in these specimens and is most marked in the intermediate and distal portions of the vestibular aqueduct. It is also present in the foveal region of the posterior temporal bone surface underlying the sac. Erosion of the bony wall of the paravesti

Fistulization at various parts of the membranous labyrinth has been currently in use as one of the treatments for the symptoms of Meniere's disease. The present report concerns our experience with membranous fistulae in experimental animal ears with and without manifestation of endolymphatic hydrops. The materials presented are mostly new; however, our previous data are also incorporated in order to bring this issue into better perspective. The first part describes the nonproduction aspect of endolymphatic hydrops in the rat, chinchilla, and squirrel monkey after obliteration of the endolympha

The external shunt operation on the endolymphatic sac is based on the concept that it will drain excess endolymph and thus arrest the progression of endolymphatic hydrops. We performed histological studies on 46 temporal bones from 13 cases of bilateral and 20 cases of unilateral Meniere's disease to evaluate the status of the pathways of longitudinal flow of endolymph to the sac. The endolymphatic ducts were blocked in 8 specimens (17%), the endolymphatic sinuses in 9 (19.5%), the utricular ducts in 12 (26%), the saccular ducts in 7 (15%), and the ductus reuniens in 27 (59%). These blockages

The etiology underlying endolymphatic hydrops is scarcely understood. It is generally accepted, however, that the primary cause of endolymphatic hydrops is the malfunction and malabsorption of the endolymphatic sac. This has already been proven by animal experiments as well as by histopathological studies of human temporal bone. In this study, we attempted to induce endolymphatic hydrops by utilizing both mechanical and immunological methods. An obliterative procedure was performed on the endolymphatic sac and duct of 14 guinea pigs, 12 of which showed extensive endolymphatic hydrops after a v

After obliteration of endolymphatic sac and duct, no significant alteration in d.c. potential, K+ activity or protein content during the development of endolymphatic hydrops could be observed. The K+ activity of endolymphatic sac was only one-ninth of that in the cochlear part of the endolymph. After injection of Thorotrast into the endolymphatic sac, aggregated particles were found in the cochlear endolymph 2 days later. Ethacrynic acid (60 mg/kg) caused a decrease in K+ activity in the cochlear endolymph, but an increase in the endolymphatic sac. Intercellular edema was observed both in stri

Recently Meniere's disease is believed to show a pathological features of endolymphatic hydrops, but the etiology of this disease has not yet been ascertained. Many investigators tried to create animal models with endolymphatic hydrops by obliterating endolymphatic sac and duct. However, these methods have not been adequate to explain the mechanism of development of Meniere's disease, because it gives mechanical damage to destroy endolymphatic sac and duct. On the other hand, Meniere's disease is thought to develop even as a result of stress. The present paper reported the conduction of an acu

The protein content and the d.c. potential of the endolymph differs in the various parts of the endolymphatic space (cochlea, utricle, semicircular canals and endolymphatic sac) as also does the ion composition (chloride, potassium, sodium). 12 months after obliteration of the endolymphatic sac and duct in guinea pigs the d.c. potential falls, whereas, the sodium activity increases. The endolymphatic hydrops is not caused by an increased colloid osmotic pressure. The increased water-binding capacity of the cochlear endolymph is correlated with the increased Na+ activity.

Endolymphatic shunt surgery has been suggested as a means to relieve vertigo caused by syphilis in patients unresponsive to conventional medical therapy. We reviewed results of endolymphatic shunt surgery on vertigo and hearing in six patients with syphilis. We found no relief of vertigo in four patients who qualified for this measure and no improvement in hearing. Histologic review of temporal bones from three patients with syphilitic hydrops revealed syphilitic involvement of the endolymphatic duct system. This may explain why endolymphatic shunt surgery may not be beneficial.

Perilymphatic and endolymphatic pressures were consecutively measured by means of a servomechanical nulling pressure system through the round window and basilar membrane one to 2.5 months after obliteration of the left endolymphatic sac and duct in seven guinea pigs. The position of the pipette was monitored by the simultaneous recording of the direct current potential through the micropipette used for the pressure measurement. (1) The perilymphatic and endolymphatic pressures of the control ears were equal. (2) The histological evaluation showed that the ears operated on had a significantly g
