BACKGROUND: Outcome of endolymphatic sac shunt surgery crucially depends on endolymphatic sac morphology. The intraoperative finding of a fibrotic sac, e.g., will lead to therapy-failure. Aim of the present study was to determine possible developmental changes in endolymphatic sac morphology which could imply, that a shunt operation should not be undertaken in older age.
METHOD: 46 histological temporal bone serial sections were examined. All sections were taken from the "Wittmaack Temporal Bone Collection". All specimens were obtained from patients with morphologically healthy ears, especial

The long-term efficacy of endolymphatic sac procedures for control of vertigo in Menière's disease has been controversial. We evaluated results of sac shunt surgery for 234 patients having at least 10 years followup (mean, 13.5 years). All patients had persistent vestibular symptoms despite medical therapy. All underwent endolymphatic subarachnoid shunt as their original operation. Data were collected by chart review and questionnaire regarding: (1) the number of additional surgical procedures to control vertigo, (2) remaining dizziness, and (3) level of disability. One hundred forty-seven of

The external shunt operation on the endolymphatic sac is based on the concept that it will drain excess endolymph and thus arrest the progression of endolymphatic hydrops. We performed histological studies on 46 temporal bones from 13 cases of bilateral and 20 cases of unilateral Meniere's disease to evaluate the status of the pathways of longitudinal flow of endolymph to the sac. The endolymphatic ducts were blocked in 8 specimens (17%), the endolymphatic sinuses in 9 (19.5%), the utricular ducts in 12 (26%), the saccular ducts in 7 (15%), and the ductus reuniens in 27 (59%). These blockages

There continues to be considerable confusion concerning the diagnosis and treatment of perilymphatic fistulas (PLFs) and Meniere's disease. This paper reports an 8-year experience with patients whose symptoms were compatible with Meniere's disease but who had some other element that raised the possibility of their having a PLF. This review is a retrospective report on 64 patients who had fluctuating hearing loss, vertigo, tinnitus, and aural fullness typical of Meniere's disease. However, they also had a positive fistula test and/or symptoms beginning immediately after head trauma. They all un

Conservative shunting procedures, i.e. ESS, ultrasonic irradiation and cryosurgery, are based on the assumption that there is increased volume and/or pressure of endolymph in Menière's disease. Since there is no reliable clinical test to detect endolymphatic hydrops, shunting procedures on cases without hydrops are doomed to failure. Surgery on the endolymphatic sac was not associated with fibrosis or obliteration of its lumen. Silastic shunt tubes were well tolerated by the body, and the shunt between the sac and the subarachnoid space seemed to remain open. The results of experimental surger

Endolymphatic sac-related surgeries are a series of effective procedures to treat patients with Ménière's disease. However, there is controversy since they were proposed in 1927. Although most studies have shown the effectiveness of endolymphatic decompression and endolymphatic shunting, the mechanisms of them are still unclear. Some studies suggested that these procedures work because the endolymphatic hydrops relieves after surgery. However, the new one proposed recently, endolymphatic duct blockage, seems to contradict to the relief theory. Therefore, this article reviewed the mechanisms of

Ménière's disease is characterized by attacks of vertigo and by sensorineural hearing loss, tinnitus, and ear fullness; these cochlear symptoms are unilateral in the majority of cases. Medical treatment tends to control vertigo and to cure the possible causes of endolymphatic hydrops (autoimmune disease, syphilis, metabolic and endocrine derangements, etc). In case of failure of medical treatment, surgery is considered. Endolymphatic mastoid shunt is indicated when hearing is normal or fluctuating. In all other cases, vestibular neurectomy or chemical vestibular labyrinthectomy, by means of in

While endolymphatic shunt procedures are conceptually logical, there are characteristics of underlying pathological conditions and host response that severely limit the probabilities of achieving functional shunts.

In 1985 the American Academy of Otolaryngology-Head and Neck Surgery Committee on Hearing and Equilibrium established revised guidelines for reporting treatment results for Menière's disease. Since then little information regarding the newly adapted criteria and their effects on the evaluation process has appeared. Thus we compared the results of different surgical procedures for Menière's disease using both the 1985 and 1972 guidelines. One hundred nine surgical procedures from 1969 to 1985 were reviewed. Six different surgical procedures were evaluated: the Cody-Tack, cochleosacculotomy, end

Endolymphatic sac tumors (ELSTs) are rare neuroectodermal neoplasms arising within the posterior petrous bone. We present a case of a 21-year-old man who presented with a 6-month history of intermittent morning headaches, fatigue, diplopia, and gait ataxia. Imaging and surgical pathology identified an adenocarcinoma of the endolymphatic sac compressing the cerebellum and brain stem. The tumor and multiple metastases were treated with surgery, radiation, and radiosurgery. Following insertion of a ventriculoperitoneal shunt for hydrocephalus, he developed symptomatic tension pneumocephalus secon

Surgical intervention has been offered to patients with Meniere's disease who have failed medical treatment and have disabling symptoms. Surgical options have included labyrinthectomy (mechanical and chemical), vestibular neurectomy, and endolymphatic sac surgery with or without shunting. We present a modification of endolymphatic sac decompression surgery that includes wide decompression of the sigmoid sinus, posterior cranial fossa dura, and endolymphatic sac (sac-vein decompression). Thirty-five patients underwent 37 primary procedures with 2 years of follow-up. Patients were evaluated acco

A series of fifteen patients with intractable vertigo from Meniere's disease were treated by endolymphatic sac surgery. Thirteen of the fifteen patients were followed up for more than 6 months with an average of 16 months. Eight of the 13 patients relieved from vertigo and 3 improved. While in half of the patients the tinnitus and pressure feeling were diminished or vanished, only one patient demonstrated a 13 dB hearing loss. The surgical indication for Ménière's disease, the location of the endolymphatic sac and the evaluation of the endolymphatic shunt operation were discussed.

The senior author has performed shunt procedures routinely for medically refractory cases of Meniere's disease starting in 1964 and ending in 1984. Five hundred and sixty-eight consecutive patients were operated on and followed from 1970 to 1984. Combining the cases of C. G. Jackson, there have been a total of 676 procedures done at the Otology Group. Initially endolymphatic-subarachnoid (ES) shunts were used, then endolymphatic-mastoid shunts (EM), and most recently, the Denver valve has been employed. No significant difference between these procedures was noted in the control of vertigo or s

The effects of Thomsen's 1981 report of a controlled double-blind study on the surgical treatment of Meniere's disease were studied by reviewing the current literature and Thomsen's subsequent reports. The current surgical literature was found to contain reasonable support for the continued use of endolymphatic shunt procedures despite the criticism of such procedures by Thomsen. Surgical therapy for Meniere's disease continues to be divided into auditory sparing or conservative procedures and auditory ablative or destructive procedures. Conservation procedures are important because approximat

Delayed endolymphatic hydrops, an entity which can be differentiated from Meniere's disease, was first reported simultaneously by Nadol et al. (1975) and Wolfson & Leiberman (1975) and specified by Schuknecht (1978). They diagnosed it from the anamnesis of patients and confirmed the hydropic condition of the labyrinth solely by evidence obtained from surgical procedures such as labyrinthectomy. The aim of the present study was to emphasize the usefulness of the furosemide test for distinguishing this entity before surgery by detecting the hydropic condition in the labyrinth. In these 3 years,

Any medical or surgical treatment of Meniere's disease is always suspect because the natural history is so variable and the pathologic physiology is not well-known. Even the occurrence rate in one and both ears is not known. It is certainly true that endolymphatic hydrops, caused by the failure of absorption of endolymph in the endolymphatic sac, is the principal pathology in the classical, fully developed Meniere's disease. While the endolymphatic shunt operation is correct in theory, in most ears, it does not work because the surgeon does not always locate the lumen of the sac and when locat

The electrocochleographic findings of 3 unusual cases from about 350 performed on patients with various hearing loss are presented; they demonstrate the value of electrocochleography as a clinical tool in otological investigation. The first case was an electrocochleographically detected acute impairment of the first neuron, the second a hereditary hearing loss with endolymphatic hydrops, and the last shows the effect of an endolymphatic shunt on a hydropic hearing loss.

Surgical therapy for disabling vertigo usually comes in at a late stage in the course of treatment for this patient group. The surgical procedures can be divided into two categories: non-destructive and destructive surgery. The former constitutes primarily procedures directed against the endolymphatic sac such as endolymphatic sac shunt procedures or decompression procedures. Peri-endolymphatic shunt procedures have also been proposed by some surgeons. None of these latter procedures have, however, gained widespread clinical acceptance. Destructive surgery is aimed against the end-organ itself

In 1981 we published the results of a double-blind, placebo-controlled study in which the efficacy of a regular endolymphatic sac mastoid shunt was compared with a purely placebo operation (mastoidectomy) in controlling the symptoms of 30 patients with typical Meniere's disease. Minor differences could be demonstrated between the shunt and the sham operation, but the greatest difference was between the preoperative and postoperative scores, and both groups improved significantly. It was concluded that the impact of the various endolymphatic sac shunts on the symptoms of patients with Meniere's

OBJECTIVE: This study aimed to describe the rationales for and preliminary results of three new types of surgery for the treatment of intractable Meniere's disease, all involving insertion of a capillary tube into the endolymphatic duct. This study also aimed to compare the contrasting surgical strategies of endolymphatic sac enhancement versus sac supplantation.
STUDY DESIGN AND SETTING: The study design was a retrospective review of 129 surgeries conducted by the author at Chang Gung Memorial Hospital since 1993: 51 cases of Huang/Gibson inner ear shunt implantation, 52 cases of intraductal
