Clinical outcomes of underwater endoscopic mucosal resection for esophageal subepithelial tumors.

Esophageal submucosal tumors are increasingly identified through routine upper endoscopy, often performed as part of health screenings. Although many of these lesions are benign, some have malignant potential, necessitating accurate diagnosis and appropriate resection. Underwater endoscopic mucosal resection (UEMR) has gained attention as a novel technique that enables mucosal resection without submucosal injection, offering advantages in terms of safety and efficacy. However, data regarding its use for esophageal submucosal tumors are limited. This retrospective single-center study included 3

Surgical management of recurrent sinonasal mucosal melanoma: endoscopic or transfacial resection.

Sinonasal mucosal melanoma (SNMM) is associated with poor prognosis. Local recurrence is common and represents a major problem in the therapy. Wide resection surgery is usually applied. However, given the almost futile prognosis, optimal symptom control may be preferable to wide resection at all costs. The aim of our study was to analyze the outcome in patients with recurrent SNMM treated by transfacial radical surgery or by a less invasive endoscopically controlled approach. Patients with recurrent grade III or IV staged SNMM who presented to our ENT department between 2000 and 2010 were eith

Recurrence after endoscopic piecemeal mucosal resection for large sessile colorectal polyps.

AIM: To evaluate the safety and outcomes of endoscopic piecemeal mucosal resection (EPMR) for large sessile colorectal polyps. METHODS: The patients enrolled in this study were 47 patients with 50 large sessile polyps (diameter, 2 cm or greater) who underwent EPMR using a submucosal saline injection technique between December 2002 and October 2005. All medical records, including characteristics of the patients and polyps, complications, and recurrences, were retrospectively reviewed. The first follow-up endoscopic examination was performed at 3-6 mo after initial endoscopic resection, and the

Laparoscopy-assisted endoscopic mucosal resection in the colon: a preliminary report.

Endocopic mucosal resection of flat villous tumours or giant polyps (> 3 cm) may give rise to local complications such as haemorrhage or perforation because of the very thin wall of the colon, above all in the right half. Our aim was to evaluate whether laparoscopy-assisted endoscopic excision of flat villous tumours or giant polyps (> 3 cm) can be safely performed, avoiding critical septic complications and can also help in the selection of patients to be submitted to colonic resection without increasing morbidity or mortality. The procedure is a new minimally invasive therapeutic approach in

Endoscopic mucosal resection of early esophageal carcinoma--experience of 9 cases.

BACKGROUND: Early esophageal mucosal carcinoma (M1 and M2) can be treated by ablation, or by endoscopic resection such as endoscopic mucosal resection (EMR) and endoscopic submucosal dissection. Endoscopic resection enables pathologic examination of resected specimens. We hereby report our experiences with early esophageal cancer and its endoscopically observed types, chromoendoscopy with Lugol's iodine and EMR results. METHODS: Between May 2003 and July 2007, 9 patients with early esophageal carcinoma underwent EMR. The diagnosis was made by conventional endoscopy (waiting for the relaxed ph

Endoscopic mucosal resection of Barrett's oesophagus containing dysplasia or intramucosal cancer.

Barrett's oesophagus is premalignant. Oesophagectomy is traditionally regarded as the standard treatment option in the presence of high grade intraepithelial neoplasia or intramucosal cancer. However, oesophagectomy is associated with high rates of mortality and morbidity. Endoscopic ablative therapies are limited by the lack of tissue for histological assessment, and the ablation may be incomplete. Endoscopic mucosal resection is an alternative to surgery in the management of high grade intraepithelial neoplasia and intramucosal cancer. It is less invasive than surgery and, unlike ablative tr

Hydroxypropyl methylcellulose: A better submucosal fluid cushion for endoscopic mucosal resection.

BACKGROUND: Formation of a submucosal fluid cushion has become integral to endoscopic mucosal and polyp resection. Solutions available for injection into the submucosa of the GI tract create short-lasting submucosal fluid cushions or are costly. The feasibility, durability, and histologic response to submucosal fluid cushions created by injection of hydroxypropyl methylcellulose were studied in the esophagus of pigs. METHODS: Thirty-six esophageal submucosal fluid cushions created with hydroxypropyl methylcellulose were studied in 12 animals divided in 2 groups of 6 animals each. In Group I,

[Endoscopic mucosal resection of premalignant lesions of the upper gastrointestinal tract].

BACKGROUND: Surgical therapy of early malignancies of the upper gastrointestinal tract is associated with substantial morbidity and mortality, especially in elderly and co-morbid patients. In Japan endoscopic mucosal resection (EMR) has been proven to be safe and efficacious in this indication. PATIENTS AND METHODS: 22 patients (68 +/- 14 years, 9 females) with high-grade dysplasia of the esophagus (n = 5), early carcinoma of the esophagus (T1N0M0, n = 11) or early gastric cancer (T1N0M0, n = 6) proven by high-resolution videoendoscopy (plus chromoendoscopy in most cases), miniprobe-endosonog

Underwater endoscopic mucosal resection of a follicular lymphoma: A case report.

INTRODUCTION: Endoscopic resection of a follicular lymphoma (FL) presenting as a gastric subepithelial tumor (SET), along with periodic follow up can be a treatment option because gastrointestinal FL cells tend to reside in the primary site, which may explain its indolent nature. PATIENT CONCERNS: A gastric lesion was found incidentally during a screening endoscopy in 73-year-old woman without any gastrointestinal symptom. DIAGNOSIS: The patient was diagnosed with a grade I FL that was 1.4 cm large, at the greater curvature of lower-body. INTERVENTION: We performed underwater endoscopic muc

Transverse colon schwannoma treated by endoscopic mucosal resection: A case report.

Gastrointestinal (GI) schwannoma is one of the rarest tumors of the GI tract. We herein describe the case of a 70-year-old female patient who presented for surveillance colonoscopy. The examination detected a 1-cm polyp that was removed by snare cautery polypectomy. Immune and histochemical staining revealed spindle cells that were positive for S-100 and vimentin, but negative for CD34 and smooth muscle actin, consistent with GI schwannoma. This case is noteworthy as GI schwannomas usually present in the stomach, making the finding of a colonic schwannoma of clinical interest. Furthermore, the

Pyogenic granuloma of the duodenum treated successfully by endoscopic mucosal resection.

Pyogenic granuloma is a lobular capillary hemangioma that occurs mostly on the skin and the mucosal surfaces of the oral cavity and tongue. Only a few cases in other parts of the digestive tract have been reported. Gastrointestinal pyogenic granuloma is a rare cause of hemorrhage in the digestive tract, but should be considered in the differential diagnosis of patients with gastrointestinal bleeding. We report the case of a 62-year-old anemic woman found to have a pyogenic granuloma of the duodenum, which was treated adequately by endoscopic mucosal resection.

Histopathology of Barrett's esophagus after ablation and endoscopic mucosal resection therapy.

This review focuses on the histopathological evaluation of endoscopic mucosal resection (EMR) specimens in Barrett's esophagus, and on the histopathological, biological, and molecular properties of postablation Barrett's esophagus. EMR may be used for both diagnostic and therapeutic purposes. Diagnostic accuracy regarding the grade and stage of neoplasms is improved with the use of EMR, but the value of this technique for treatment is more controversial because of the high prevalence rate of positive margins and the rate of metachronous lesions found elsewhere in the esophagus during follow-up
