Plate-Counting Techniques for Average Bioburden Below the Limit of Detection.

Current methods for estimating average bioburden on medical device products, following ISO 11737-1:2018 with low bioburden or high limit of detection, present challenges for bioburden-based sterilization methods (e.g., radiation sterilization by ISO 11137-2:2013 or ISO 13004:2022). The inability to accurately estimate low bioburden counts per device can result in an overestimation of device bioburden and an artificially decreased challenge during validation. It can also force products to be sterilized at higher doses if the overestimation of bioburden prevents use of a lower dose than would ot

The analytical evaluation threshold for inorganic metal extractables and leachables analysis of medical devices.

Chemical characterization of medical devices uses the analytical evaluation threshold (AET) to determine reportable organic extractables, as these chemicals may be of toxicological concern and should be addressed via toxicological risk assessment. The AET is not applicable to metal extractables due to the exclusion of toxicity data on inorganics from the dataset used to derive dose-based threshold (DBT) values. This results in minimal guidance for reporting metal extractables. Herein, an AET for metals, or mAET, is proposed as a reporting threshold for individual metal extractables. The mAET c

A new quantitative method for determining patient risk for reusable medical device categorization based on using and interpreting Kremer's cleaning classification system.

BACKGROUND: The cleaning of reusable medical devices involves inherent challenges that can impact on the effectiveness of the cleaning process; consequently, the subsequent safety of patients. Fluid dynamics play a critical role in determining the flow and distribution of cleaning agents where the design of the device can either facilitate or hinder this important process. Complex geometries, narrow channels, or irregular surfaces can impede effective flushing of contaminants leading to incomplete cleaning that creates a greater likelihood for patient contamination risks. METHODS: Device feat

Recovery of Surgical Equipment Sterile Processing During a Floodwater Boil Advisory.

Excessive rainfall and dam failures resulted in floodwater contaminating our public water supply. The endotoxin risk in the contaminated water created challenges in recovery of sterile processing for our surgical equipment. Recovery plans should include a potable water source and a method to connect it to the required location. We share our solution of plumbing our sterile processing equipment to tanker-transported potable water sources. (Disaster Med Public Health Preparedness. 2018; 12: 415-418).

Nucleic acid contamination in sexual health clinics.

PURPOSE OF REVIEW: This review explores the potential for nucleic acid contamination with gonorrhoea or chlamydia within healthcare settings, the implications that this has for the transmission of infection and the potential for contamination to cause false positive test results in patients. RECENT FINDINGS: Contamination with Chlamydia trachomatis and Neisseria gonorrhoeae nucleic acid on hard surfaces and examination equipment is common particularly in areas where urine samples are being taken. The quantity of material which can be detected is low and unlikely to lead to transmission of inf

Addressing Medical Device Extractables and Leachables via Non-Target Analysis (NTA); The Analytical Evaluation Threshold (AET) and Quantitation.

Substances leached from a medical device during its clinical use are important due to the patient health-related effects they may have. Thus, medical devices are profiled for leachables (and/or extractables as probable leachables) by screening extracts or leachates of the medical device for released organic substances via a non-target analysis (NTA) employing chromatographic methods coupled with mass spectrometric detection. Chromatographic mass spectral response factors for extractables and leachables vary significantly from compound to compound, complicating the application of assessment str

Current issues in reprocessing of medical and surgical instruments.

By the very nature of reusability, processing for reusable medical devices has been and continues to be risk prone. Healthcare systems have historically been slow to embed best practice quality control elements into everyday reprocessing workflow. The identification of weak points and an openness to change are necessary to allow for a proactive culture of safety. Infection preventionists can play key roles in establishing and maintaining related quality control and oversight.

Disposable Ureteroscopes in Urology: Current State and Future Prospects.

Ureteroscopy is the most common surgical modality for stone treatment. Reusable flexible ureteroscopes are delicate instruments that require expensive maintenance and repairs. Multiple single use ureteroscopes have been developed recently to combat the expensive and time-intensive sterilization and repair of ureteroscopes. Although multiple studies have looked at different aspects of reusable and single use ureteroscopes, there is significant heterogeneity in performance measures and cost between the 2 categories, and neither has a clear advantage. Both can be used successfully, and individual

Sampling methods for flexible endoscopes without a working channel: a scoping review.

BACKGROUND: A diagnostic flexible laryngoscopy using a flexible endoscope (FE) without a working channel can become contaminated when inserted through the nose to inspect the throat. Microbiological surveillance is necessary to ensure adequate reprocessing. A lack of knowledge exists about the most accurate way to assess microbiological contamination on the surface of FEs without a working channel. A scoping review of research on sampling techniques for FEs without a working channel was done to identify frequently used sampling techniques and to determine the best way to assess microbiological
