Clinical Classification of Acquired Concomitant Esotropia.

BACKGROUND: Classification and management of acquired concomitant esotropia is controversial. We sought to establish a simple clinical classification in order to determine in which cases further investigations in search of underlying pathologies are necessary. PATIENTS AND METHODS: Observational retrospective study of the files of 175 consecutive patients examined in our unit between 2009 and 2018 for acute convergent strabismus. One hundred and nine patients were selected, after exclusion of infantile, incomitant, or mechanical esotropias, residual esotropias, and patients examined on a sing

Treatment of acute acquired concomitant esotropia.

BACKGROUND: The treatment efficacy of botulinum toxin bilateral medial rectus injections for acute acquired concomitant esotropia (AACE) in adult is not clear. We characterize the effects of botulinum toxin injection in the treatment of AACE, especially in patients over 14 years old, and compared it with surgical treatment. METHODS: In this prospective, nonrandomized, controlled clinical study, patients with AACE in our hospital from March 2017 to March 2020 elected to receive bilateral medial rectus injections of botulinum toxin or to undergo extraocular muscle surgery. Ocular position and s

A Case Series: Exercise-Induced Esotropia.

The authors present three patients with idiopathic exercise-induced esotropia. The history, physical findings, and testing that led to the diagnosis are discussed. To the authors' knowledge, this is the largest series of patients with this rare condition, with this series representing half of reported patients in the literature. It is hoped that a discussion of common features of the condition will guide further exploration of the etiology.

20 unusual presentations of accommodative esotropia.

PURPOSE: This paper will discuss several unusual presentations of accommodative esotropia. METHODS: A total of 20 patients with unusual case histories who presented with accommodative esotropia were studied retrospectively. These patients were separated into 3 categories. The first contained children who had their onset of accommodative esotropia after a traumatic event such as head trauma or ocular trauma. The second involved infants between 3 and 5 months of age who presented with accommodative esotropia. The third showed the onset of accommodative esotropia associated with diabetic ketoaci

Unilateral esotropia after enucleation in infancy.

Five patients developed esotropia in the remaining eye after unilateral enucleation in the first few weeks to months of life. Esotropia was associated with a face turn toward the opposite side and abduction nystagmus with a null point in extreme adduction. Our experience with these patients supports the reflexogenic theory for the development of a type of congenital-infantile esotropia. An intact globe-ocular muscle relationship, even in a blind eye, may have a stabilizing effect on the fellow eye in the first few weeks to months of life, and this should be considered before enucleation is don

What to expect from primary inferior oblique overaction after esotropia surgery : Primary inferior oblique overaction in esotropia.

BACKGROUND: Overelevation in adduction is common in patients with primary esotropia. This study evaluates the variation in ocular motility pattern in patients with primary inferior oblique (IO) muscle overaction after esotropia surgery. METHODS: The medical records of consecutive patients who underwent surgery for infantile, partially accommodative, and basic esotropia over eleven years and had at least one year of follow-up were reviewed. Patients with primary inferior oblique muscle overaction (IOOA) presented at baseline or during follow-up were selected and divided according to the first

Effect of binasal occlusion in children with esotropia.

BACKGROUND: Esotropia is a common pediatric ophthalmologic disorder that, if left untreated, can have a significant impact on the binocular visual function and appearance of the child. Binasal occlusion (BNO) is a non-surgical measure to alleviate the impact of esotropia. But there is no consistent theoretical basis for BNO and its therapeutic efficacy has been controversial. This study aimed to investigate the effect of BNO in the treatment of children with esotropia. METHODS: We performed a retrospective review of children with multiple types of esotropia who still had symptoms of obvious e

A rare cause of acquired esotropia: Leprosy.

Leprosy is an infective chronic granulomatous disease involving the skin and peripheral nerves caused by Mycobacterium leprae. Complications such as lagophthalmos, corneal opacity and uveitis are common, but cranial nerve involvement is rarely seen. The fifth and seventh cranial nerves are most commonly involved. We report a case of acquired esotropia due to sixth nerve palsy following a rare cranial nerve involvement by leprosy.

Best age for surgery for infantile esotropia.

Infantile esotropia (IE) is defined as an esotropia before the age of 6 months, with a large angle, latent nystagmus, dissociated vertical deviation, limitation of abduction, and reduced binocular vision, without neurological disorder. Prematurity, low birth weight, and low Apgar scores are significant risk factors for IE. US standard age of first surgery is 12-18 months, in Europe 2-3 years. The only study to date with prospectively assigned early- and late-surgery groups and evaluation according to intention-to-treat, was the European Early vs. Late Infantile Strabismus Surgery Study (ELISSS
