Swarm intelligence.

The standard view of how things work is that an outside force impacts a group of individuals and causes outcomes they are interested in. The outside force may not affect all individuals to the same extent, but we can summarize the effect by taking the average. Effective influence is thought to come from the top, not from the group that is being led. The alternative considered here is that a substantial degree of intelligence resided in the individuals or elements that someone wants to study or change. And these elements of the system interact with each other. This phenomenon goes by many names

Should the practice of medicine be a deontological or utilitarian enterprise?

There is currently an unrecognised conflict between the utilitarian nature of the overall NHS and the basic deontology of the doctor-patient interaction. This conflict leads to mistrust and misunderstanding between managers and clinicians. This misunderstanding is bad for both doctors and managers, and also leads to waste of time and resources, and poorer services to patients. The utilitarian thinkers (mainly managers and politicians) tend to value finite, short term, evidence based technical interventions, delivered according to specifications and contracts. They appear happy to break care up

Realist evaluation of maternity waiting home intervention models in Inhambane, Mozambique: protocol for a comparative embedded case study, the Mozambique-Canada Maternal Health Project.

INTRODUCTION: This is a study protocol that tests and refines realist theories regarding the uptake and scale-up of the linked maternity waiting home (hereafter MWH) and facility birth intervention in the Mozambican context. The theories were developed through a realist review of MWH-facility birth literature from low-income and middle-income countries. The aim of the proposed study is to contribute to a contextually refined understanding of the causal chains underlying MWH-facility birth adoption by pregnant women and their families, communities, the health system and donors. METHODS AND ANA

The GenAI Learning Partner: An Evolving Framework for Personalized and Reflective Artificial Intelligence Use in Occupational Therapy Education.

The integration of generative artificial intelligence (genAI) into health professions education remains limited, particularly regarding its potential to support deeper learning, ethical reasoning, and personalization. Current discourse often focuses on institutional risk or faculty productivity, with less emphasis on genAI's role in student learning. This study presents a theory-informed model for using genAI to scaffold engagement and professional growth in a graduate-level occupational therapy research methods course. Drawing from personalized learning theory and constructivist pedagogy, we

Performance-based payment models and value-based health care: a semi-systematic review through theoretical lenses.

OBJECTIVES: To identify and critically examine the theoretical lenses applied in the literature to conceptualize performance-based and value-based payment models, and to analyze how these frameworks explain their behavioral, organizational, and system-level effects. METHODS: A semi-systematic review was conducted in July 2025 across Web of Science, PubMed/MEDLINE, Embase, LILACS, and CINAHL, using a structured search strategy and transparent selection procedures following PRISMA 2020 reporting principles. Eligible empirical and theoretical studies on value- or performance-based payment were e

Dynamic interactions of facilitators and barriers in palliative care in China: a qualitative study.

BACKGROUND: China is experiencing rapid population ageing and increasing demand for palliative care. However, development remains uneven because of fragmented policies, workforce shortages, limited public understanding, and sociocultural barriers. Existing studies have typically described these factors separately, offering limited insight into how they interact within complex health systems. This study aimed to examine the dynamic interactions among facilitators and barriers shaping palliative care development in China, informed by Wu-Xing theory. METHODS: We conducted a multi-method qualitat

Elective non-therapeutic intensive care and the four principles of medical ethics.

The chronic worldwide lack of organs for transplantation and the continuing improvement of strategies for in situ organ preservation have led to renewed interest in elective non-therapeutic ventilation of potential organ donors. Two types of situation may be eligible for elective intensive care: patients definitely evolving towards brain death and patients suitable as controlled non-heart beating organ donors after life-supporting therapies have been assessed as futile and withdrawn. Assessment of the ethical acceptability and the risks of these strategies is essential. We here offer such an e

Correlated Subjects: Relational Ethics and Veterinary Legal Accountability in Animal-Assisted Interventions.

The ethical and legal governance of Animal-Assisted Interventions (AAIs) remains conceptually and normatively fragmented. Although animals engaged in therapeutic, educational, and assistive activities make valuable contributions to human well-being, they continue to be defined by law as property or welfare objects, despite their meaningful yet limited forms of relational participation within structured human-controlled environments. This perspective obscures their context-dependent responsiveness and their institutional embeddedness. The present paper addresses this gap by adopting a normative

Job Stress and Individual Creativity: A Screen-Based Partial Least Squares Structural Equation Modeling Protocol for AI-Enabled Workplaces.

The increasing use of artificial intelligence (AI) in organizations is reshaping employees' job demands, leadership perceptions, and opportunities for creativity. Drawing on the Job Demands-Resources (JD-R) model and Social Learning Theory, this study examines whether employees' perceptions of ethical leadership mediate the relationship between job stress and individual creativity, and whether organizational AI embeddedness amplifies this relationship. Using partial least squares structural equation modeling (PLS-SEM) with 5,000 bootstrap resamples, the findings show that job stress negatively

What factors are important to whom in what context, when adults are prescribed hearing aids for hearing loss? A realist review protocol.

INTRODUCTION: Hearing aids are the gold standard treatment to help manage hearing loss. However, not everyone who needs them has them, and of those who do, a significant proportion of people do not use them at all, or use them infrequently. Despite literature reviews listing key barriers and enablers to the uptake and use of hearing aids, there is little evidence to describe how this varies by population and context. This review will describe what factors are important to whom in what context when considering the provision of hearing aids for hearing loss in adults. METHODS AND ANALYSIS: The

Ethical challenges of emerging anatomical technologies in UK public engagement.

Anatomical technologies that enhance opportunities for teaching, research, and public engagement are becoming increasingly popular in the UK. This narrative review critically examines the ethical dimensions of these emerging anatomical technologies for use in anatomical outreach activities, from a UK legal and cultural perspective. While innovations such as 3D printing, imaging, virtual and augmented reality, and "synthetic donors" expand opportunities for anatomical teaching, and by extension anatomical outreach, they also complicate established ethical standards that the anatomy community ha

From the dead to the unborn: is there an ethical duty to save life?

In this paper I examine the question whether physicians have a legal and ethical duty to sustain pregnancies of women who die during the first or second trimester by the delivery of their fetuses. One ground for such a duty, on which I am focusing, is the duty of "special relationship" between the mother and the fetus. In my paper, I claim that the special relations the pregnant woman and the fetus have do provide such a moral duty. This moral duty derives from the special and intimate relationship between the mother and the fetus, which has two considerations that support it: the uniqueness o

Why moral judgements change across variations of trolley-like problems.

In the standard 'trolley problem', respondents must decide whether to save a condemned group of individuals by sacrificing a safe bystander. Although respondents often are willing to sacrifice the bystander in some circumstances (e.g., by pulling a lever), they are loath to sacrifice the bystander in others (e.g., by pushing the bystander off a footbridge). This difference in responding has been explained via a Dual Process theory of moral judgements (DPT). DPT, however, is a classic boxes-and-arrows model that only makes directional predictions. Developing an Ethical Perspectives Scale.

Many theories hold that ethical perspectives inform moral judgments, but few such theories have corresponding individual difference scales. The present research aimed to develop an Ethical Perspectives Scale (EPS) reflecting specifically the five-perspective Markkula framework: utilitarianism; rights; fairness/justice; common good; and virtue. The authors wrote and progressively revised five sets of three items, each set intended to represent one and only one Markkula perspective, before obtaining responses from the present convenience sample (n = 621; 463 female, 157 male, 1 unspecified; Mage

Effects of an embodied micro-film intervention on ethical sensitivity, compassion, and ethical decision-making in nursing students: A mixed-methods study.

BACKGROUND: Bridging the "knowing-doing gap" remains a central challenge in nursing ethics education. Traditional didactic methods often position students as disembodied observers, limiting the transformation of ethical knowledge into practical competence. OBJECTIVE: To address the "knowing-doing gap" in ethics education, this study aimed to develop and evaluate an innovative micro-film intervention, guided by embodied cognition theory, and to assess its impact on key components of nursing ethical competence. DESIGN: A mixed methods study employing a two-arm cluster-assigned quasi-experiment

Reducing and managing faecal incontinence in people with advanced dementia who are resident in care homes: protocol for a realist synthesis.

INTRODUCTION: Faecal incontinence (FI) is the involuntary loss of liquid or solid stool that is a social or hygienic problem. The prevalence of FI in residents of care homes is high, but it is not an inevitable consequence of old age or dementia. There is good evidence on risk factors, but few studies provide evidence about effective interventions. There is a need to understand how, why, and in what circumstances particular programmes to reduce and manage FI are effective (or not) for people with dementia. The purpose of this review is to identify which (elements of the) interventions could po

Deliberation at the hub of medical education: beyond virtue ethics and codes of practice.

Although both codes of practice and virtue ethics are integral to the ethos and history of "medical professionalism", the two trends appear mutually incompatible. Hence, in the first part of the paper we explore and explicate this apparent conflict and seek a direction for medical education. The theoretical and empirical literature indicates that moral deliberation may transcend the incompatibilities between the formal and the virtuous, may enhance moral and other aspects of personal sensitivity, may help design and improve other parts of the curricula, and may foster self-awareness and clarif
