AIM: To present the Croatian system of ethical review of clinical trials and assessment outcomes of the applications reviewed by the Croatian Central Ethics Committee.
METHODS: Clinical trial applications reviewed by the Croatian Central Ethics Committee, which has the legal mandate to review clinical trials of medicinal products and medical devices, were retrospectively analyzed from May 2004 to the end of 2008 according to the number, research area, and type of opinion issued. Applications from 2008 were analyzed separately according to the study phase, participants (adult trials vs pediatr

PURPOSE OF INVESTIGATION: To evaluate how many women required the so-called "emergency contraception" at our outpatient service and what the actual role is of this kind of pharmacological administration in interfering with ovulation and pregnancy, paying particular attention to the ethical and medico-legal aspects of this subject.
METHODS: During the period from 1 December 1998 to 30 November 2003, emergency contraception was prescribed to a total of 1,160 women. With regard to the contraceptives used, in most cases (1,132, 97.6%) a combined oral estrogen-progestogen pill (ethinyloestradiol 0

Developments in the realms of medical innovation and geriatric clinical intervention impact our understanding of the nature of late life, the possibilities for health in advanced age, medical decision making, and family responsibility in ways that could not have been predicted 15 years ago. This essay begins to map new forms of biomedicalization in the U.S. and to underscore their emergence in a new ethical field. We suggest that a new kind of ethical knowledge is emerging through "routine" clinical care, and we offer examples from the following interventions: cardiac procedures, kidney dialys

Pneumocystis carinii pneumonia (PCP) remains a common and morbid infection among patients with the acquired immunodeficiency syndrome (AIDS). Most patients who die of PCP do so because of respiratory failure. The survival after intubation and mechanical ventilation for PCP and respiratory failure has gone through three eras: Era I (1981-85), when the survival rate to hospital discharge was about 10%; Era II (1986-88), when the hospital survival rate rose to about 40%; and Era III (1989-present), when the hospital survival rate fell again to about 25%. Patients with CD4 counts of less than 50,

Ethics education in biomedical research in India is frequently limited to regulatory compliance, with inadequate emphasis on ethical reasoning, contextual vulnerability, and deliberative review processes. As part of the Silver Jubilee of the Institutional Ethics Committee of St John's Medical College Hospital, a one-day preconference workshop titled "Responsible Researchers and Reviewers: Ethics of Good Clinical Practice" was conducted during the 13th International Conference of Ethics Education. The workshop employed case-based, interdisciplinary discussions focusing on vulnerability, ethics

As Clinical Ethics Consultation (CEC) becomes more institutionalized, concerns about its legitimacy and efficacy have grown. We defend a realist orientation, according to which CEC has normative force only if its recommendations answer to stance-independent reasons, not to individual preferences and institutional policies. Public justification can and should proceed through familiar means (e.g., shared mid-level norms and transparent procedures) so that stakeholders can scrutinize the reasoning. We show how neutrality, admirable as a procedural virtue, hollows CEC when treated as a master idea

Health care professionals and patients impacted by incarceration face unique medical, legal, and ethical issues. The frequency and nature of ethics consultations for these patients are underexplored. This study aimed to characterize the primary ethical issue and contextual features of ethics consultations involving patients impacted by incarceration. We conducted a qualitative concept content analysis of ethics consultations involving patients impacted by incarceration and calculated descriptive statistics of demographics to compare these patients with the broader population of patients impact

In November 2018 the American Society for Bioethics and Humanities (ASBH) administered the first Healthcare Ethics Consultant Certification examination to 138 candidates, 136 of whom (98.5 percent) passed and were "certified" as "healthcare ethics consultants." I believe this certification process is both premature and inadequate. Certification for ethics consultants is premature because, as Kornfeld and Prager state repeatedly in their article in this issue of The Journal of Clinical Ethics, "The Clinician as Clinical Ethics Consultant: An Empirical Method of Study," there is a lack of "empir

In response to national trends calling for increasing accountability and an emerging dialogue within bioethics, we describe an effort to credential clinical ethicists at a major academic medical center. This effort is placed within the historical context of prior calls for credentialing and certification and efforts currently underway within organized bioethics to engage this issue. The specific details, and conceptual rationale, behind the NewYork-Presbyterian Hospital's graduated credentialing plan are shared as is their evolution and ratification within the context of institutional policy. The recent Ebola outbreak in West Africa began in the spring of 2014 and has since caused the deaths of over 6,000 people. Since there are no approved treatments or prevention modalities specifically targeted at Ebola Virus Disease (EVD), debate has focused on whether unproven interventions should be offered to Ebola patients outside of clinical trials. Those engaged in the debate have responded rapidly to a complex and evolving crisis, however, and this debate has not provided much opportunity for in-depth analysis. Additionally, the existing literature on access to unproven therapies has foc

With cannabis medicines now obtaining legal status in many international jurisdictions (generally on the authorisation of a medical professional), a rapid increase in consumer demand for access to cannabis as a therapeutic option in the treatment and management of a range of indications is being noted. Despite this accessibility, knowledge on optimal use is lacking. Further drug development and clinical trials at regulatory standards are necessary both if a better understanding of the efficacy of cannabis medicines, optimal product formulation and indication-specific dosing is needed and to en

PURPOSE: The rapid advancement of automatic speech recognition (ASR) and natural language processing technologies has created significant opportunities for clinical applications within speech and language disorders, yet these capabilities remain largely confined to high-resource languages and populations. As research communities work to address these inequities through inclusive speech data collection, the intersection of clinical vulnerability, linguistic diversity, and emerging speech and language technologies creates ethical considerations that are rarely addressed by existing guidelines. BACKGROUND: The Clinical Ethics Residency for Nurses was offered selectively to nurses affiliated with two academic medical centers to increase confidence in ethical decision-making.
RESEARCH QUESTION/AIM: To discover how effective the participants perceived the program and if their goals of participation had been met.
RESEARCH DESIGN: A total of 65 end-of-course essays (from three cohorts) were analyzed using modified directed content analysis. In-depth and recursive readings of the essays by faculty were guided by six questions that had been posed to graduates.

BACKGROUND: Regenerative Medicine (RM) techniques aimed at the musculoskeletal system are increasingly translated to clinical trials and patient care. This revolutionary era in science raises novel ethical challenges. One of these challenges concerns the appropriate choice of the comparator in (randomized controlled) trials, including the ethically contentious use of sham procedures. To date, only general guidelines regarding the choice of the comparator exist.
OBJECTIVE: To provide specific guidelines for clinical trial comparator choice in musculoskeletal RM.
METHODS: In this manuscript, w

Ethics consultation's (EC) role in supporting healthcare professionals, patients, and families confronted by eating disorders (ED) is under-described in the literature. We used qualitative conceptual content analysis to characterize the ethical issues and contextual features of EC involving patients with Anorexia Nervosa (AN), Bulimia Nervosa (BN), Binge-Eating Disorder (BED), Avoidant Restrictive Food Intake Disorder (ARFID), or Other Specified Feeding or Eating Disorder (OSFED) and compared adult vs. pediatric settings at a single institution from 2015 to 2024. We identified 40 consultations

Clinical work--psychotherapy, assessment, consultation, or research--with emotionally ill children and adolescents is challenging, complex, and exceedingly rewarding. Psychiatrists must remain mindful of children's vulnerabilities but also remain respectful of their rights, regardless of any legal barriers or developmental limitations that exist. To varying degrees, youths are afforded the rights of integrity, autonomy, informed consent or assent, protection of health care information, and participation in research. All physicians who work with children and adolescents are obliged to possess t

Investigator-initiated vaccine clinical trials are an important branch of Investigator-Initiated Trials (IIT), focusing on scientific issues such as vaccine protective efficacy, rare adverse reactions, optimization of vaccination strategies, and exploration of immune mechanisms. By systematically sorting out the development of ethical requirements related to IIT in China, this paper compares in depth the common requirements of investigator-initiated vaccine clinical trials and registered vaccine clinical trials in terms of informed consent and ethical review, and specifically analyzes the spec
