Factors affecting the implementation of guideline-based prophylactic antiemetic therapy for chemotherapy-induced nausea and vomiting in Japan: a protocol for a hospital-based qualitative study.

INTRODUCTION: Chemotherapy-induced nausea and vomiting (CINV) decrease patients' quality of life and negatively impact treatment outcomes. Although standard prophylactic antiemetic therapy for acute CINV recommended by guidelines is effective, poor guideline implementation is a worldwide problem. In Japan, prophylactic antiemetic therapy is relatively well implemented for chemotherapy associated with high emetogenic risk, while implementation gaps are observed for that with low emetogenic risk.Although most reports on factors influencing appropriate antiemetic prescription focus on physicians'

Diagnostic pathways for obstructive sleep apnoea in the Australian community: observations from pharmacy-based CPAP providers.

PURPOSE: Models for the diagnosis of obstructive sleep apnoea (OSA) are evolving in many countries to meet public demand. In Australia, the unregulated sleep industry does not provide a framework to govern or review emerging pathways in the community. The objectives of this study were to (1) describe current diagnostic pathways for OSA in Australia as reported by continuous positive airway pressure (CPAP) providers in community pharmacies and (2) obtain stakeholder feedback on pathways and identify potential areas for practice improvement. METHODS: Semi-structured telephone interviews were co

[Consequences for public hospital of the law of December 20th 1988 (Huriet law), concerning the protection of persons used for biomedical researches].

Does the public hospital have the capability of harbouring this research? In any case, it is an important mission entrusted to it by the Huriet law. But even before the law has come into effect (the date of applicability was initially set for January 1st 1990, postponed until June 1st 1990, and then further pushed back to December 31st 1990 due to the lack of publication of corresponding application decrees), its text has prompted many questions and much debate among numerous health sectors. We are limited in the extent to which we may estimate, within the context of the public hospital, the s

Transversal incision of the vagina favors the remaining of the tape in the middle-third urethra compared to longitudinal incision during transobturator sling procedures for stress urinary incontinence.

BACKGROUND: To describe a new type of incision of the vagina during transobturator sling procedure and to evaluate by ultrasound the tape position at 3, 6 and 12 months after surgery. We conducted a prospective study including 51 patients with urinary stress incontinence who underwent sling procedure using the transversal vaginal incision. Tape position was evaluated by ultrasound at 3, 6 and 12 months after surgery and expressed as a percentage of the urethral length (the proximal third of the urethral length 0-39 %, the middle third 40-60 %, and the distal third 60-100 %). Why MASLD Trials Underselect the MENA Region Sites and What Sponsors and Regulators Must Change.

Metabolic dysfunction associated steatotic liver disease (MASLD) and metabolic dysfunction associated steatohepatitis (MASH) are accelerating global priorities, yet pivotal clinical trial activity remains concentrated in North America and Europe despite substantial burden in the Middle East and North Africa (MENA). Building on prior work quantifying MENA underrepresentation, we provide a policy and operational analysis informed by contemporary epidemiological, regulatory, and clinical trial literature and practical experience in trial delivery. We argue that underselection is driven less by sc

Re-imagining poppers regulation in Canada: Perspectives from pharmacists, pharmacy students and public health practitioners.

Alkyl nitrites, commonly known as poppers, remain widely used in Canada despite regulatory prohibitions that restrict their sale as prescription-only substances. This incongruence between policy and practice has contributed to unregulated supply chains, product contamination, and missed opportunities for harm reduction - particularly among gay, bisexual, and other men who have sex with men, who use poppers at higher rates. This commentary examines the implications of reclassifying poppers as Schedule II drugs, permitting pharmacist-mediated, non-prescription access, drawing on perspectives fro

Systematic Review of Pharmacist-Administered Paediatric Vaccinations in Low- and Lower-Middle-Income Countries.

BACKGROUND: An estimated 25 million children globally miss routine vaccinations each year, with 14.3 million classified as zero-dose and the majority residing in low- and lower-middle-income countries (LMICs). Pharmacies are recognised as accessible healthcare delivery points, yet their role in paediatric immunisation in LMICs is not well defined. This literature review sought to evaluate pharmacist-provided vaccination services for paediatric patients in LMICs. METHODS: A systematic review was conducted following Cochrane and PRISMA guidelines. Five databases and grey literature were searche

Digital twins in precision pharmacotherapy: emerging applications, challenges, and future directions.

Digital twin technology, defined as dynamic digital models that represent individual patients, is emerging as a promising paradigm in precision pharmacotherapy. The integration of pharmacokinetic and pharmacodynamic (PK/PD) modeling, clinical data, genomic information, and real-time patient monitoring enables digital twins to shift drug therapy away from population-based averages toward individualized, adaptive decisions. This narrative review explores conceptual frameworks, emerging applications, methodological approaches, clinical value, limitations, and future directions of digital twins in

Top Ten Tips Palliative Care Clinicians Should Know About Intentionally Interprofessional Collaboration.

Although high-quality and holistic specialty palliative care is delivered by an interprofessional team, little guidance is available to optimize approaches to and sustainment of such teamwork. This article supports individuals to practice at the top of their education, clinical training, and scope of practice while maximizing the functionality of the palliative care team as a whole. We intentionally use the term interprofessional rather than interdisciplinary to clarify that we are focused on collaboration of team members who represent multiple professions or occupations that require specializ

KInetics of Procalcitonin to Reduce Unnecessary aNtibiotic use (KIPRUN): protocol for a multicentre, randomised, superiority trial to compare the efficacy and safety of procalcitonin kinetics-guided and absolute procalcitonin value-guided antibiotic initiation in critically ill patients.

INTRODUCTION: Appropriate antibiotic (AB) therapy remains a challenge in intensive care units (ICUs). Guidelines recommend against using procalcitonin (PCT) to guide the initiation of AB therapy, although with low certainty and a very low level of evidence. Recent studies suggest that changes in PCT levels may be more accurate than a single measurement; however, this has never been tested in a randomised trial. METHODS AND ANALYSIS: In this multicentre, randomised controlled trial (RCT), we aim to compare the efficacy and safety of two different PCT protocols for initiating AB therapy. Hemody

Clofazimine use for nontuberculous mycobacterial infections: 10-year experience in Belgium.

BACKGROUND: Clofazimine is increasingly recommended for nontuberculous mycobacterial (NTM) infections, despite limited evidence. In Belgium, until July 2025 access to clofazimine - outside of procurement by the national tuberculosis program - was restricted and associated with financial and administrative hurdles, creating barriers for patients. METHODS: This retrospective, multicentre study documents clofazimine use from January 2015 to December 2024 through pharmacy and medical record review at Belgium's two largest tertiary care hospitals. Collected data included indications, treatment dur

Parent-delivered Hand-arm Bimanual Intensive Training Including Lower Extremity for young children with bilateral cerebral palsy in Vietnam: protocol for a randomised controlled trial.

INTRODUCTION: Cerebral palsy (CP) is a leading cause of long-term disability for children in Vietnam and across the world, affecting both gross and fine motor skills as well as daily independence. Intensive therapy has been shown to significantly improve motor outcomes. Moreover, parental involvement in therapy contributes positively to child development. Hand-Arm Bimanual Intensive Therapy Including Lower Extremities (HABIT-ILE) is a structured intervention that targets motor function of the upper and lower extremities through intensive, goal-directed practice. This study aims to evaluate the

Study protocol of the HD-MED study aiming to personalize drug treatment in Huntington's disease: a longitudinal, observational study to assess medication use and efficacy in relation to pharmacogenetics.

BACKGROUND: Huntington's disease (HD) is a hereditary, neurodegenerative disorder characterized by motor, cognitive, and psychiatric symptoms. Currently, HD can only be managed symptomatically, including a large variety of prescribed drugs. Many HD patients experience negative medication effects (e.g. side effects or non-response). Pharmacogenetic (PGx) studies show how genetic variation affects both medication efficacy and toxicity and holds the potential to improve these outcomes of drug treatment. PRIMARY OBJECTIVE: To classify the effect of the PGx profile of CYP2C19 and CYP2D6 in HD gene

Design and rationale of DUTCH-AF: a prospective nationwide registry programme and observational study on long-term oral antithrombotic treatment in patients with atrial fibrillation.

INTRODUCTION: Anticoagulation therapy is pivotal in the management of stroke prevention in atrial fibrillation (AF). Prospective registries, containing longitudinal data are lacking with detailed information on anticoagulant therapy, treatment adherence and AF-related adverse events in practice-based patient cohorts, in particular for non-vitamin K oral anticoagulants (NOAC). With the creation of DUTCH-AF, a nationwide longitudinal AF registry, we aim to provide clinical data and answer questions on the (anticoagulant) management over time and of the clinical course of patients with newly diag
