[Patulous eustachian tube following otitis media].

Loss of weight, dehydration, pregnancy, fatigue, and otitis media are among the factors proposed as causes of a patulous eustachian tube, but true details remain obscure. We studied patients who developed a patulous eustachian tube following otitis media and discuss the relationship between these 2 conditions. Subjects were 12 patients diagnosed with otitis media at our department who later developed a patulous eustachian tube. The initial middle ear disease progressed from acute otitis media to otitis media with effusion in 2, acute otitis media in or acute mastoiditis in 1 each, and otitis m…

Unusual eustachian tube mass: glomus tympanicum.

A case of recurrent glomus tympanicum presenting with epistaxis is described. CT and MR imaging revealed a homogeneously enhancing mass extending along the entire course of the eustachian tube, with a portion protruding into the nasopharynx. Glomus tumors tend to spread along the path of least resistance and may extend into the eustachian tube. The unique imaging appearance should place a glomus tumor high on the list of differential diagnoses.

[Patulous eustachian tube--diagnosis and therapy].

The patulous Eustachian tube (tuba aperta) is a benign condition that may be overlooked or misdiagnosed. It is a distressing condition for the patient with such symptoms as autophony and a sensation of fullness in the ear. One third of the women with the diagnosis are pregnant or taking oestrogens. The diagnosis is made by the history of fullness or blockage and hearing one's own voice and breath in the ear. The drum is usually atrophic and moves with respiration but this may not be visible. The usual past history is of weight loss. The patulous Eustachian tube is frequently associated with pe…

Patulous eustachian tube identification using tympanometry.

The patulous, or nonclosing, eustachian tube is believed to affect as many as 7 percent of all adults, causing physical and psychological difficulties. This paper provides a brief review of the literature regarding the patulous eustachian tube (PET), its symptoms, precipitating conditions, incidence, diagnosis, and current medical management. Several case studies are also presented to illustrate the use of tympanometry in PET identification.

Surgical treatment of patulous eustachian tube.

Thirteen patients (16 ears) with patulous eustachian tubes were treated with pterygoid hamulotomy combined with transposition (eight ears) or transection (eight ears) of the tendon of the tensor veli palatini muscle. Tubal function was tested preoperatively and postoperatively by sonotubometry. Prior to surgery, only one ear showed normal tubal function on swallowing at the time of the test. The follow-up period ranged from two months to five years. Nine ears yielded normal sonotubometric results during the average follow-up time of two years, while two tubes opened on swallowing, remained ope…

Eustachian tube blockage and Meniere's disease.

On the basis of our clinical experience, it has been noted that an unusually large proportion of patients with Meniere's disease have intermittent eustachian tube blockage. A review of the tympanograms of 81 patients with Meniere's disease, representing 107 ears, showed that 25 (30.9%) of the patients and 35 (32.7%) of the ears had abnormal negative middle ear pressure. The maximum compliance peak was in at a pressure point in excess of --100 mm H2O. Further investigation of these patients has led us to conclude that intermittent eustachian tube blockage frequently accompanies Meniere's diseas…

Revisiting Eustachian Tube Anatomy and Physiology.

The Eustachian tube (ET) it is a complex structure connecting the middle ear cavity with the nasopharynx. Its function is to maintain middle ear ventilation to facilitate transmission of sound from the tympanic membrane to the cochlea. It is located medial to the parapharyngeal space. The ET and tympanomastoid cavities form an anatomic and functional unit that cannot easily be divided, and it plays a fundamental role in the etiology of different clinical entities. However, the underlying pathophysiological mechanisms are not yet fully understood, and further research is needed to address the g…

Paper patching for patulous eustachian tube.

BACKGROUND: Paper patching, a method that places cigarette paper over the most mobile quadrants of the tympanic membrane, is one of the treatment options for patulous eustachian tube (PET). AIMS/OBJECTIVES: The objective of this study was to compare the outcomes of two different treatment strategies for PET. MATERIAL AND METHODS: Twenty-three patients underwent paper patching of the tympanic membrane and 16 patients were treated with nasal saline irrigation with or without ipratropium bromide nasal spray. Medical records were reviewed for resolution of PET symptoms as categorical variables (…

Eustachian tube duplication: a unique anomaly.

Very few eustachian tube anomalies have been published in the literature and have consisted of descriptions of diverticula, hypoplasia/aplasia, fistula, or aberrant associated musculature. We present a girl with a novel anomaly consisting of a eustachian tube duplication that originates in the nasopharynx and exits posterior to a microtic and atretic ear. We review the literature on eustachian tube anomalies and also consider the derivation of this anomaly.

The patulous eustachian tube: management options.

The patulous eustachian tube is a benign, yet annoying condition that may be over-looked or misdiagnosed. It is frequently associated with peritubal atrophy, and usually responds to conservative measures such as weight gain or mucus-producing agents. In patients not responding to these measures, a variety of surgical options are available. We have been using a technique that avoids elevating a tympanomeatal flap, is reversible, and is effective. This technique, as well as other surgical options, are reviewed. The advantages and disadvantages inherent in them are discussed.

Patulous eustachian tube: a radiological perspective.

The diagnosis of a patulous Eustachian tube relies mainly upon the history, examination and a clinical awareness of the condition. More recently, objective tests such as tympanometry and sonotubometry have been shown to be useful in confirming the diagnosis. This paper describes how computed tomographic imaging may also be useful in aiding the diagnosis. Supporting this, clinical cases of patulous tubes are demonstrated radiologically associated with congenital hemifacial microsomia and following radiotherapy for a nasopharyngeal carcinoma. The role of computed tomographic imaging in the diagn…

Otitis media and eustachian tube caliber.

Clinical data supported the hypothesis that most otitis media requiring surgery is part of a life-long bilateral process that correlates with small mastoid air system, and with a bougie-assessable large eustachian tube. Cleft palate, even in the microform of cleft uvula, was an indicator of large eustachian tube (p less than 0.001). The 1.48 mm mean eustachian tube caliber of the otitis patients was larger (p less than 0.001) than the 0.73 mm mean of the reference eustachian tubes. Eustachian tube caliber was not a correlate of age or sex. Comparatively small caliber eustachian tubes were asso…

Analyzing eustachian tube function in patients with symptoms of chronical Eustachian tube dysfunction by pressure chamber measurements.

PURPOSE: The determination of Eustachian tube dysfunction (ETD) subtypes in daily routines is based on symptoms of the patient, otoscopy, nasopharyngoscopy, tympanometry, pure tone audiometry, and Valsalva's test, even though this clinical assessment is often not sufficient to clearly diagnose the subtype. In the study, we have evaluated the possibility of the determination of different subtypes of ETD by pressure chamber measurements. METHODS: In a single-person pressure chamber, we exposed 17 patients to pressure increases and decreases. The pressure profile of the continuous impedance of e…

Myringotomy and tube for relief of patulous eustachian tube symptoms.

Reports on the effectiveness of myringotomy and tube for treatment of symptoms of the patulous eustachian tube are discouraging, even though there are no large series substantiating its ineffectiveness. To further investigate this problem, we have reviewed the charts of all patients at the Otologic Medical Group (OMG) from 1974 to 1981 with the diagnosis of patulous eustachian tube who underwent myringotomy and tube for their symptoms. We identified 60 ears of 46 patients with complaints consistent with patulous eustachian tube symptoms. The treatment either eliminated or substantially reduced…

Consensus on treatment of obstructive Eustachian tube dysfunction with balloon Eustachian tuboplasty.

OBJECTIVE: There is a great variability in diagnosis of obstructive Eustachian tube dysfunction and its treatment by balloon Eustachian tuboplasty (BET). The aim of this paper was to present a consensus on indications, contraindications, methodology, complications and results after BET. MATERIAL AND METHODS: We obtained a consensus on BET, after a systematic review of the literature on BET from 1966 to November 2018, using MESH terms «Eustachian tube and (dilation or dysfunction)», including a total of 1.943 papers in Spanish, English, German and French. We selected 139 papers with a relevant…
