[Changed thyroid function parameters in severe extrathyroidal diseases].

Patients with critical non-thyroidal illnesses (NTI) often show decreased serum levels of thyroid hormones. We find the so-called low T3-syndrome and the low T3-low T4-syndrome in cases of severe liver- or kidney-diseases, inflammation, metabolic disorders and in emergency situations with danger to life. A strong decrease of the serum-T4-level signals a bad prognosis. In spite of the relative low concentrations of thyroid hormones and a possible disturbance in pituitary regulation, the patients are clinically in an euthyroid situation. There is no hypothyroidism in the common sense, but possib

Energy metabolism dysregulation in schizophrenia with non-thyroidal illness syndrome: Roles of citric acid and Tyrosol-4-sulfate.

BACKGROUND: Non-Thyroidal Illness Syndrome (NTIS) is frequently observed in schizophrenia and is traditionally regarded as a peripheral endocrine disturbance. However, whether NTIS reflects a distinct metabolic phenotype linked to central energy dysregulation in schizophrenia remains unclear. We hypothesized that NTIS in schizophrenia reflects a distinct metabolic state characterized by disrupted energy metabolism, which may be linked to specific symptom dimensions. METHODS: A total of 185 patients with schizophrenia were enrolled and classified into NTIS and non-NTIS groups based on standard

Covid-19, the thyroid and the pituitary - The real state of play.

Thyroid and pituitary disorders linked to the coronavirus SARS-CoV-2, responsible for the COVID-19 epidemic, are mainly due to direct infection of the endocrine glands by the virus and to cell damage induced by the immune response. The two most frequent thyroid complications of COVID-19 are low T3 syndrome, or "non-thyroidal illness syndrome" (NTIS), and thyroiditis. Studies among in-patients with COVID-19 have shown that between one out of six and half of them have a low TSH level, related to NTIS and thyroiditis, respectively, sometimes found in the same patient. In NTIS, the decrease in fre

[Modified postoperative prevention of recurrence in euthyroid struma nodosa].

In a prospective clinical study after uni- or bilateral subtotal thyreoidectomy, cause of euthyreote goitre, no substitution with L-thyroxin was ordered postoperatively. The follow-up after three months and one year shows a representative time for judgement of the hormonal situation after three months. Over one year 55% with bilateral resection need hormone substitution but only 4% with unilateral. Bilateral subtotal thyroid resection seems to be a too extensive resection to leave enough functional thyroid tissue for a physiological function.

Thyroid function tests in patients with familial dysalbuminaemic hyperthyroxinaemia (FDH).

Two patients with familial dysalbuminaemic hyperthyroxinaemia (FDH) are described in whom the albumin variant resulted in raised total T4 levels, and artefactually raised free T4 using a 'single-step' technique employing an analogue of T4 as tracer. The first patient was clinically euthyroid and presented with relapse of schizophrenia and abnormal thyroid function tests (total T4 336 nmol/L, total T3 4.2 nmol/L, TSH 1.8 mU/L, free T4 73 pmol/L). These results led to diagnostic confusion and the patient was treated with a short course of anti-thyroid drugs. The second patient had signs and symp

Evaluation of non-thyroidal illness syndrome in shock patients admitted to pediatric intensive care unit in a developing country.

During critical illness, children my experience various changes in their thyroid hormone levels. Such changes are termed non-thyroidal illness syndrome (NTI). The extent of change correlates with the severity of the illness and its outcomes in critically ill patients. This study aimed to investigate the correlation between the severity of shock and thyroid hormone derangement. This prospective observational study included forty patients aged one month to five years who were admitted to the pediatric intensive care unit (PICU) with shock. Thyroid function tests were conducted on admission, afte

Selenium involvement in mitochondrial function in thyroid disorders.

Selenium (Se), an important oligoelement, is a component of the antioxidant system. Over the last decade, it has been ever more frequently discussed in the context of thyroid disorders. Graves' disease and Hashimoto's thyroiditis, differentiated thyroid cancer, and even endemic goiter may have common triggers that are activated by excess reactive oxygen species (ROS), which are involved in various stages of the pathogenesis of thyroid disorders. Most oxidative events occur in mitochondria, organelles that contain enzymes with Se as a cofactor. Mitochondria are responsible for the production of

Sexual function and depressive symptoms in young women with thyroid autoimmunity and subclinical hypothyroidism.

OBJECTIVE: The results of few studies conducted to date suggest an increased prevalence of sexual dysfunction in patients with thyroid disorders. DESIGN: The aim of this study was to compare female sexual function and depressive symptoms between women with autoimmune thyroid disease and with mild thyroid failure. PATIENTS: The study included four groups of young women: euthyroid women with Hashimoto's thyroiditis (Group 1), women with nonautoimmune subclinical hypothyroidism (Group 2), women with autoimmune subclinical hypothyroidism (Group 3) and healthy euthyroid females without thyroid au

Decreased thyroid function in Korean women with bipolar disorder receiving valproic acid.

We report here two adult female patients with bipolar disorder who developed abnormal thyroid function after short-or long-term administration of valproic acid (VPA). The first patient developed sick euthyroid syndrome (relative to her pretreatment thyroid levels) after only 2 months of treatment with VPA and quetiapine. The second patient, who had no pretreatment medical history including thyroid disease, developed hypothyroidism after several years of treatment with VPA and quetiapine.

Diagnosis of thyroid hormone transport protein anomalies: an overview.

Inherited or acquired variations in the serum concentrations of TBG, TTR or albumin are rather common. Clinical studies have shown that most patients with transport protein variations were referred for thyroid testing because of incidentally detected "unusual" thyroid function tests. Although, usually no palpable thyroid abnormalities were found, the interpretation of these unexpected thyroid function tests resulted very often in inappropriate treatment with surgery, 131-iodine, antithyroid drugs as well as T4-replacement therapy. The clinical impact in the interpretation and diagnosis of thyr

Euthyroid hyperthyroxinemia due to familial excess of thyroxine-binding globulin.

The correct diagnosis of benign hyperthyroxinemia in this patient and his family members will spare them the unnecessary testing and treatment for thyrotoxicosis that has befallen some such patients. Results of the usual blood tests for assessment of thyroid function, such as T4, T3, and thyrotropin determinations, were not uniformly diagnostic, and were potentially misleading. An increased T4 level, a nonsuppressed TSH level, normal levels of FT4 and FT4D, and a low level of T3RU were clues that led to a request for specific measurement of serum TBG levels in multiple family members; family t

[Mineralocorticoid function of the rat adrenal gland and thyroid hormones].

Adrenal mineralocorticoid function was studied in 195 male albino Wistar rats with a different level of thyroid hormones in the organism. Aldosteronemia, aldosteronuria, and plasma renin activity were studied in in vivo experiments. In vitro experiments were carried out to study the content of aldosterone in the adrenals and its discharge into the incubation medium in euthyroid, hyper- and hypothyroid animals. Hyperthyroidization leads to manifest inhibition of the adrenal mineralocorticoid function, and suppresses aldosterone excretion in the urine and plasmic metabolic clearance in diminishe

Severe hyperthyroidism without symptoms due to nonthyroidal illness in a child with acute hepatitis: case report and literature review.

While liver enzyme changes are frequently reported in hyperthyroidism, liver dysfunction itself can lead to alterations in thyroid hormone metabolism. However, the exact relationship between hyperthyroidism and liver dysfunction is unclear. We report an 11-year-old boy presenting with acute hepatitis of unknown etiology, who was incidentally found to have asymptomatic biochemical hyperthyroidism. Despite significant total and free T4 elevation, clinical evidence of thyrotoxicosis was absent. Thyroid I-123 uptake was also reduced. Additional testing revealed slight T3 elevation and significant

[Thyroid and cardiovascular disorders].

In this study three problems concerning interactions between thyroid and cardiovascular system are discussed. Cardiac arrhythmias, congestive heart failure, pleural effusion, hyperlipidaemia, arterial hypertension may be consequences of thyroid disorders leading to inappropriate hormone secretion. During such illnesses as heart failure, myocardial infarction and in patients undergoing coronary artery bypass surgery profound changes may occur in thyroid hormone metabolism known as sick euthyroid syndrome. Treatment with amiodarone may lead to changes in thyroid tests results and to development

Thyrotoxicosis induced sick sinus syndrome: medical therapy may avoid permanent pacing.

A 65-year-old white female developed sick sinus syndrome associated with thyrotoxicosis which resolved upon achieving a euthyroid state, thus obviating the need for permanent pacemaker implantation. TSH was less than 0.1 ng/dL, and 24-hour iodine uptake was 28%. After correction of the hyperthyroid state with propylthiouracil and potassium iodine, sinoatrial pauses up to 6 seconds resolved over 10 days. A subsequent sinus node recovery time was normal and no symptoms recurred over 22 months.

A Case of Septic Shock Complicated by Abnormal Thyroid Hormone Levels.

BACKGROUND: Severe illness induces large changes in thyroid hormone economy. Non-thyroidal illness syndrome (NTIS), also known as low T3 syndrome, refers to thyroid hormone alterations induced by systemic or consumptive non-thyroidal diseases. These alterations strongly correlate with disease severity but are often overlooked in clinical practice. METHODS: Here we report a case of septic shock complicated by abnormal thyroid hormone (TH) levels. The patient presented with markedly reduced triiodothyronine (T3) and free triiodothyronine (fT3), profoundly suppressed thyroid-stimulating hormone

Role of non-thyroidal illness syndrome in predicting adverse outcomes in COVID-19 patients predominantly of mild-to-moderate severity.

OBJECTIVE: Existing studies reported the potential prognostic role of non-thyroidal illness syndrome (NTIS), characterized by low triiodothyronine (T3) with normal/low thyroid-stimulating hormone (TSH), mainly in severe COVID-19. None considered the significant impact of SARS-CoV-2 viral load on adverse outcomes. We aimed to clarify the prognostic role of NTIS among predominantly mild-to-moderate COVID-19 patients. TcTU before and after exogeneous TSH suppression in diagnosing thyroid autonomy.

In 104 euthyroid patients with goiter and suspected thyroid autonomy two thyroid scintiscans with quantification of the TcTU were performed before (TcTUo) and after exogeneous TSH suppression (TcTUs). In 15 patients with subnormal TSH before suppression the TcTUs revealed no difference to the TcTUo. In contrast to the TcTUo, in euthyroid patients the TcTUs is not relevantly influenced by individual iodine supply. In euthyroid goitreous patients with normal TSH a TcTUo less than 2.0% resulted in a TcTUs less than 1.1% in all 22 cases and, therefore, in these patients a scintiscan after TSH supp
