Policy design and implementation of jail-based medication for opioid use disorder: A five-state qualitative comparative analysis.

BACKGROUND: Justice-involved individuals face dramatically elevated post-release overdose risk, yet correctional facility-based medications for opioid use disorder (MOUD) implementation remains uneven despite strong effectiveness evidence. This study examined policy configurations associated with high implementation in rural correctional systems. METHODS: We used fuzzy-set Qualitative Comparative Analysis to examine five rural states: Rhode Island, Vermont, Kentucky, Alabama, and Tennessee. Seven policy domains were calibrated: statutory authority, medication coverage, funding mechanisms, wor

Effectiveness and cost-effectiveness of cognitive adaptation training as a nursing intervention in long-term residential patients with severe mental illness: study protocol for a randomized controlled trial.

BACKGROUND: Despite the well-known importance of cognitive deficits for everyday functioning in patients with severe mental illness (SMI), evidence-based interventions directed at these problems are especially scarce for SMI patients in long-term clinical facilities. Cognitive adaptation Training (CAT) is a compensatory approach that aims at creating new routines in patients' living environments through the use of environmental supports. Previous studies on CAT showed that CAT is effective in improving everyday functioning in outpatients with schizophrenia. The aim of this study is to evaluate

Assessing carbon emission impacts of sponge city development: insights from runoff reduction analysis.

This study aimed to quantify the impact of sponge city facilities on both runoff reduction and carbon emission mitigation, providing valuable insights for sustainable urban development. Using the Storm Water Management Model (SWMM) 5.2 in conjunction with carbon emission factor calculations, we comparatively evaluated the annual runoff reduction and carbon emission abatement potential of traditional drainage systems versus those incorporating sponge city facilities. Our results showed that the implementation of sponge city facilities resulted in a substantial decrease in runoff volume (100,840

The Importance of the First 24 Postoperative Hours: Does Current Evidence Support Short-Stay High-Acuity Care After Gynecologic Oncology and Complex Abdominal Surgery?

Background: Postoperative admission to critical care facilities is frequently employed following complex abdominal and gynecologic oncology surgery; however, the clinical value of routine short-stay (≤24 h) high-acuity care remains uncertain as the existing evidence is limited, heterogeneous and derived primarily from non-randomized studies. Consequently, the present critical narrative review aims to discuss the rationale for planned short-duration postoperative high-acuity care, summarize the contemporary evidence, and identify priorities for future research. Methods: Relevant studies evaluat

From targeted exemptions to user fee abolition in health care: experience from rural Zambia.

Poor access to health care is one of the greatest impediments to improved health in Africa. In Zambia, user fees are considered to be partly responsible for substantial disparities in access to health care. When the Government introduced user fees in 1993, considerable concern was expressed about the adverse effects on utilisation and access. A national exemption policy was designed to protect the poorest sections of the population. However, this was largely ineffective in reaching the majority of the eligible population. On January 13th, 2006, the President of Zambia announced a policy to abo

Formative research for the design of a scalable water, sanitation, and hygiene mobile health program: CHoBI7 mobile health program.

BACKGROUND: The Cholera-Hospital-Based-Intervention-for-7-Days (CHoBI7) is a handwashing with soap and water treatment intervention program delivered by a health promoter bedside in a health facility and through home visits to diarrhea patients and their household members during the 7 days after admission to a health facility. In a randomized controlled trial among cholera patient households in Bangladesh, the 7-day CHoBI7 program resulted in a significant reduction in cholera among household members of cholera patients and sustained improvements in drinking water quality and handwashing with

Feasibility of establishing a serosurveillance system using residual specimens from district-level hospitals: A case study in Choma and Ndola Districts of Zambia.

Residual blood specimens collected at health care facilities provide a low-cost and readily available specimen source to monitor population immunity through serological surveillance compared to more resource-intensive probability-based surveys. Despite concerns about the representativeness of these specimens, there has recently been increased interest in the use of residual specimens, driven by the need for rapid estimates of seroprevalence during outbreaks to inform response and prevention. Although residual specimen collection is a key component of surveillance systems in some settings, ther

A Comprehensive Needs Assessment Tool for Planning RHD Control Programs in Limited Resource Settings.

Rheumatic heart disease (RHD) is an important cause of disability and death in low- and middle-income countries. However, evidence-based interventions have not been implemented systematically in many countries. We present a RHD Needs Assessment Tool (NAT) that can be used at country or regional levels to systematically develop and plan comprehensive RHD control programs and to provide baseline data for program monitoring and evaluation. The RHD NAT follows a mixed-methods approach using quantitative and qualitative data collection instruments. Evidence is mapped to a conceptual model that foll

Mitigating disrespect and abuse during childbirth in Tanzania: an exploratory study of the effects of two facility-based interventions in a large public hospital.

BACKGROUND: There is emerging evidence that disrespect and abuse (D&A) during facility-based childbirth is prevalent in countries throughout the world and a barrier to achieving good maternal health outcomes. However, much work remains in the identification of effective interventions to prevent and eliminate D&A during facility-based childbirth. This paper describes an exploratory study conducted in a large referral hospital in Dar es Salaam, Tanzania that sought to measure D&A, introduce a package of interventions to reduce its incidence, and evaluate their effectiveness. METHODS: After exte

Sampling constraints, sparse-cell estimates, and cross-sectional inference: considerations on hr-HPV prevalence and associated factors among HIV-positive women in Mekelle, Ethiopia.

Shfare et al. report a high-risk human papillomavirus (hr-HPV) prevalence of 39.7% among HIV-positive women attending public health facilities in Mekelle, Ethiopia, and identify several clinical and behavioural predictors using multivariable logistic regression. The study addresses an important evidence gap in a high-burden setting and employs rigorous molecular detection methods. Nevertheless, three methodological considerations affect the interpretation of the findings. First, facility-based recruitment from selected antiretroviral therapy (ART) clinics may limit the generalizability of the

Applying a participatory approach to the promotion of a culture of respect during childbirth.

Disrespect and abuse (D&A) during facility-based childbirth is a topic of growing concern and attention globally. Several recent studies have sought to quantify the prevalence of D&A, however little evidence exists about effective interventions to mitigate disrespect and abuse, and promote respectful maternity care. In an accompanying article, we describe the process of selecting, implementing, and evaluating a package of interventions designed to prevent and reduce disrespect and abuse in a large urban hospital in Tanzania. Though that study was not powered to detect a definitive impact on re

"Practice so that the skill does not disappear": mixed methods evaluation of simulator-based learning for midwives in Uganda.

BACKGROUND: Postpartum hemorrhage and neonatal asphyxia are leading causes of maternal and neonatal mortality, respectively, that occur relatively rarely in low-volume health facilities in sub-Saharan Africa. Rare occurrence of cases may limit the readiness and skills that individual birth attendants have to address complications. Evidence suggests that simulator-based training and practice sessions can help birth attendants maintain these life-saving skills; one approach is called "low-dose, high-frequency" (LDHF). The objective of this evaluation is to determine the facilitating factors and

Providers' perceptions of implementing standardized postpartum family planning: a qualitative study of midwives and nurses in Ghana.

BACKGROUND: The World Health Organization publishes and maintains evidence-based guidance for the delivery of essential family planning services in the postpartum period. Despite significant time and effort in the development of these guidelines in the past decades, a significant unmet need for postpartum family planning persists, exposing an implementation gap. Implementation strategies involving education and infrastructure support can improve family planning uptake and mobile health interventions are a feasible adjunct in improvement of health outcomes. OBJECTIVE: We sought to understand p

Sustained delirium reduction in an inpatient neuroscience unit.

BACKGROUND: Delirium is described as acute brain dysfunction that often fluctuates throughout the day and is highly prevalent in hospitalized adults. Delirium negatively affects patient outcomes with consequences including decreased cognitive functioning and increased mortality and healthcare costs. Furthermore, neurological patients are at increased risk for developing delirium due to their underlying diagnoses. PURPOSE: The purpose of this evidence-based practice project was to evaluate the long-term impact of nursing education and use of trained volunteers to prevent the development of del
