Implementation of psycho-existential symptom distress screening among Italian healthcare providers.

OBJECTIVES: Existential distress is a debilitating condition in end-of-life cancer patients. The Psycho-existential Symptom Assessment Scale (PeSAS) was developed to screen psycho-existential symptoms in palliative care, but limited research has examined its use. This study aimed to implement the Italian version of the PeSAS in palliative care services and to evaluate changes in healthcare providers' (HCPs) competence after experiential training. It also aimed to estimate the frequency of psycho-existential symptoms and explore the scale structure using network analysis. METHODS: Two-hour exp

Towards a definition of an existential approach in cancer patients: a co-produced scoping review.

BackgroundAttention to existential needs has become part of daily treatment. Studies have described the concepts of existential experiences and existential interventions. However, a consensus or conceptual clarity regarding an existential approach in cancer patients is currently missing. This scoping review investigates the term existential approach and suggests a definition.MethodsWe searched PubMed, Embase, Web of Science, and PsycINFO databases until March 2024. We used the Joanna Briggs Institute guidance, and the Preferred Reporting Items for Systematic reviews and Meta-Analyses extension

Associations of religious and existential variables with psychosocial factors and biomarkers of cardiovascular risk in bereavement.

Bereavement increases in prevalence as people age and is associated with multiple psychological and health risks, including cardiovascular risk. Religious and existential variables may play an important role in the health impacts of bereavement. Theorized pathways linking religious and existential variables with health have suggested these associations are due to intermediary psychosocial variables, but have not been tested in bereavement. This research empirically tested these pathways in a bereaved population. In N = 73 adults within 1 year of bereavement (mean age = 64.36), this study exami

The Self and its World: A Neuro-Ecological and Temporo-Spatial Account of Existential Fear.

The current international crisis situation caused by the COVID-19 pandemic is having a strong psychological impact on our subjectivities. We are constantly threatened by the danger of i) being infected, ii) infecting other people, and (iii) by the loss of social relation. Departing from these premises, we here aim to investigate the psychological and neurodynamics of this complex phenomenon. First, we discuss about recent psychological and neuronal findings on fear and its disorders, related to an unbalanced intero-exteroceptive processing and emotional regulation. Secondly we move to the psyc

Physician-Assisted Suicide: Considering the Evidence, Existential Distress, and an Emerging Role for Psychiatry.

Physician-assisted suicide (PAS) is one of the most provocative topics facing society today. Given the great responsibility conferred on physicians by recent laws allowing PAS, a careful examination of this subject is warranted by psychiatrists and other specialists who may be consulted during a patient's request for PAS. In this article, recent evidence regarding the implementation of PAS in the United States and The Netherlands is reviewed. Support is found for some concerns about PAS, such as the possibility that mental illness occurs at higher rates in patients requesting PAS, but not for

Effect of cognitive-existential group therapy on survival in early-stage breast cancer.

PURPOSE: Cognitive-existential group therapy (CEGT) was developed to improve mood and mental attitude toward cancer in women with early-stage breast cancer receiving adjuvant chemotherapy. Given the debate about group therapy's association with increased survival in women with metastatic breast cancer, we were curious to check its effect at a much earlier stage in the cancer journey. PATIENTS AND METHODS: We randomly assigned 303 women with early-stage breast cancer who were receiving adjuvant chemotherapy to either 20 sessions of weekly group therapy plus three relaxation classes (n = 154) o

Rumination, existential anxiety and professional quality of life among palliative care professionals in India.

OBJECTIVE: Palliative care enhances the quality of life for individuals with life-limiting illnesses, but frequent exposure to death and suffering poses emotional challenges for professionals. This study examines rumination's role in the relationship between existential anxiety (EA) and professional quality of life (ProQOL) among palliative care professionals in India. METHODS: A mixed-method research design was employed. Quantitative data were collected from 500 palliative care professionals using the Event-Related Rumination Inventory, EA Questionnaire and ProQOL scale. In-depth interviews

A Systematic Review of Qualitative Studies on Meaning-Making and Existential Experiences Among Cancer Survivors.

Cancer survivorship frequently involves confrontation with mortality, yet qualitative findings on meaning-making and existential experiences remain fragmented. This PRISMA 2020-guided systematic review synthesized qualitative evidence on meaning-making among adult cancer survivors across Scopus, Web of Science, MEDLINE, and the Psychology & Behavioral Science Collection (final search: 25 November 2025). Twenty-eight qualitative studies were included and synthesized using thematic synthesis, with confidence assessed via GRADE-CERQual. Five themes were identified: confrontation with mortality an

Existential concerns, community integration and psychological depression among female stroke survivors in Nigeria.

INTRODUCTION: Female stroke survivors experience considerable vulnerabilities and existential concerns, shaped by sociocultural factors and gender roles, which heighten stroke morbidity and limit community reintegration. Yet, the existential concerns of female stroke survivors in Nigeria, and their relationships with psychological depression and community reintegration have not been explored. METHODS: A mixed-methods study was conducted among female stroke survivors recruited from selected hospitals in South-west, Nigeria. Perceived Gaps Between Wishes and Practice in End-of-Life Sedation for Psycho-Existential Suffering.

CONTEXT: Continuous deep sedation (CDS) for psycho-existential suffering is controversial and often regarded as a last-resort option at the end of life. The frequency with which patients desire CDS for this reason remains unclear. OBJECTIVES: This study aimed to explore the prevalence of patients with cancer under palliative care in Japan who expressed a desire for CDS to alleviate psycho-existential suffering. It also sought to understand their condition at the time of expressing this desire, underlying suffering, and alternative interventions. METHODS: This nationwide questionnaire survey

An existential-phenomenological exploration of transition to motherhood after genetically determined infertility experience in Lithuania.

BACKGROUND: Recurrent pregnancy loss linked to the methylenetetrahydrofolate reductase (MTHFR) genetic mutation is common in infertility journeys. Identifying these mutations and offering targeted treatments can save pregnancies and assist in the transition to motherhood. However, the psychological effects of past miscarriages often persist, highlighting the need for a clear care pathway. This study explores the existential-phenomenological aspects of transitioning to motherhood after genetically determined infertility, emphasising the importance of understanding human existence to gain insigh

Using a Patient-Reported Outcome Measure to Assess Physical, Psychosocial, and Existential Issues in COPD.

Background: Chronic obstructive pulmonary disease (COPD) is marked by severe physical symptoms, impaired quality of life, and high psychological distress. Despite its impact, the identification of not only physical but also psychosocial and existential issues in the clinic lags behind that of other patient groups. Methods: This study aimed to assess physical, psychosocial, and existential issues among patients with COPD using a patient-reported outcome measure for general palliative care (the 'PRO-Pall') in a Danish outpatient clinic. We included 115 adults with COPD who completed the PRO-Pall

Expression of existential suffering in two patients with advanced cancer in an acute palliative care unit.

BACKGROUND: Recognizing and managing existential suffering remains challenging. We present two cases demonstrating how existential suffering manifests in patients and how to manage it to alleviate suffering. CASE DESCRIPTION: Case 1: A 69-year-old man with renal cell carcinoma receiving end-of-life care expressed fear of lying down "as he may not wake up." He also expressed concerns of not being a good Christian. Supportive psychotherapy and chaplain support were provided, with anxiolytic medications as needed. He was able to express his fear of dying and concern about his family, and Edmonto
