BACKGROUND: External fixators have been extensively applied in the treatment of open tibiofibular fractures and have yielded positive outcomes. The stability of an external fixator primarily hinges on its structure. Employing additional external fixation components can undoubtedly enhance stability. However, there is scant research on the topic of achieving superior stability with fewer external fixation components.
METHODS: Utilizing 3D modeling software, constructed three different external fixation models in middle tibial fractures in Group A, constructed four external fixation models in p

BACKGROUND: Fibular hemimelia (FH) represents the most common deficiency of the long bones and is associated with multiple deformities. Reconstructive treatment with external fixators in FH restores normal lower extremity alignment and length with plantigrade feet for a balanced and effective gait. The aim of this study is to evaluate the outcomes of lower limb lengthening and simultaneous tri-plane deformity correction with a computer-assisted hexagonal external fixator in children with FH.
METHODS: A retrospective review was performed for FH cases treated with a computer-assisted hexagonal

CASE: Three male patients, 8-, 13-, and 18-year-old, all developed posttraumatic cubitus varus elbow deformity after distal humerus supracondylar fractures. Each underwent a distal humerus osteotomy with application of a hexapod external fixator to gradually correct the deformity.
CONCLUSION: In patients with cubitus varus malunion, the stability of the hexapod external fixation, percutaneous nature of the osteotomy, and availability of running a residual computer-assisted program provides a consistent technique for deformity correction.

External fixation is useful for the treatment of selected injuries to the hand. Some authors have suggested that external fixation of a phalanx may tether the extensor hood, thereby hindering active movements and predisposing to permanent adhesions. There is no consensus as to the best site for placement of the pin to minimise these problems. This study was performed on cadaver specimens to investigate the influence of the pin site on the range of simulated active movement of the interphalangeal joint. The dorsal midline position produces least interference with the extensor mechanism; radial

The removal torque and the loosening and infection rates of AO/ASIF 4.5 mm Schanz screws (MS) have been compared with those of hydroxyapatite-coated (HA) and hydroxyapatite/silver phosphate-coated (HA/P) AO/ASIF Schanz screws in vivo. In sheep, the left tibia was fixed after a midshaft osteotomy by means of unilateral fixator with 6 Schanz screws in an antero-lateral position. The osteotomy gap was filled with bone wax to avoid bony overgrowth. After operation, the animals were allowed to walk around immediately. The loosening rate was 2.8% at the site of the two HA-coated screws and 36% at th

This study aimed to analyse the load-deformation behaviour of pelves treated with external fixation following AO61C1.3a pelvic injury. Designing a biomechanical setup , the load-bearing capacity of pelves was assessed across varying pin configurations (two, three, or four external fixator pins). Mechanical parameters such as stiffness, peak-to-peak, valley-to-valley, fracture line, total displacement, deformation, and maximum load were derived to quantify pelvis stability. The 3-pin configuration demonstrated superior stability with significantly smaller pubis and sacral fracture displacements

BACKGROUND: Stabilization of extra-articular distal radius fractures by wrist joint bridging (WB) dynamic fixation allows for early motion of the wrist, but relies on exact positioning of the device. In fact, physiological movement appeared to be compromised with even distinctly aberrant positioning of such device. To investigate this issue in more detail, we developed an in-vitro testing apparatus suitable for assessing the forces required for flexion and extension of the wrist.
METHODS: The experimental set-up enables the transmission of the translational movement of the traverse of a unive

BACKGROUND: Temporary external fixators are often used to stabilize fractures when definitive fracture surgery must be delayed. Sometimes, external fixators are left in place during repeat operations, including definitive internal fixation of tibial pilon and tibial plateau fractures. It is unknown how well current surgical preparation sterilizes these devices, which become part of the surgical field. Our hypothesis was that our institution's standard surgical preparation creates a low rate of culture-positive environments on external fixators at the time of surgical skin incision.

The management of comminuted tibial fractures is a challenging problem in small animal orthopedics. External skeletal fixators with Maynard clamps and Schanz pins are commonly used for the osteosynthesis of such fractures. The aim of this study was to perform a clinical assessment of Schanz pins with type II frame external fixators in the treatment of comminuted tibial fractures, and the possibility of pin changing when osteolysis and pins loosening appear using Maynard clamps. Schanz pins and external fixators were applied in five dogs. The healing of the treated tibia bones was noted in all

Distraction radio-metacarpal external fixation is an excellent technique for the osteosynthesis of distal radius fractures, in particular of comminuted articular fractures. The alternative is the locked palmar plate, a more demanding technique. The published literature does not allow concluding if one method is better than the other. The other main possible indications of external fixation at the wrist are comminuted fractures of the base of the thumb metacarpal, distal radius osteotomies, and wrist arthrodeses. At the hand, external minifixation is an excellent technique of osteosynthesis. St

Pelvic fractures account for 4-5% of all fracturated patients, and they occur in 4-5% of polytraumatized patients. In the most of the cases, they are consequent to high-energy trauma with a high percentage of lesions of other organs (cerebral, thoracic, and abdominal lesions. The most of the patients (80%) who die are dying within the first hours after trauma for a massive hemorrhagic shock. When the pelvic fracture and the patient's hemodynamic conditions are both unstable, osteosynthesis of the fracture is mandatory. Fracture stabilization should be performed within the first hour after trau

External fixation of mandible fractures is a useful technique when an open treatment is contraindicated because of extensive comminution, bone or soft tissue loss, and infection. This technique can also be used temporarily until definitive treatment is delivered. A uniphasic or biphasic system can be placed to reduce and stabilize mandibular fractures. These systems use surgically placed threaded pins and different types of connectors that can be manipulated to optimize the reduction of fractures. External fixation remains a quick, safe, and simple method to treat mandible fractures in selecte

Hindfoot salvage procedures by definition present circumstances requiring extraordinary measures to prevent or limit damage or destruction. Although this technique is used at some centers as the only means of correcting and fixating foot and ankle deformities, it is an expensive procedure. Other available methods may be more expeditious depending on the circumstances. However, the benefits of external fixation in certain circumstances are invaluable. Circular frame external fixators are important tool for the foot and ankle surgeon.

External fixation is a proved method for high tibial osteotomies. It is easy to access, allows postoperative corrections of the alignment, and can be removed without additional narcosis. Disadvantages are the frequent infection of the pin tracks and discomfort in bearing compared to internal fixation. In our own study no difference between unilateral and bilateral fixation could be found regarding loss of correction and complications. The advantage of unilateral fixation is the possibility of doing an open-wedge procedure by unilateral continuous callus distraction (hemicallotasis). This is in

Deformity, instability, and ulceration are present in a high percentage of patients who have Charcot arthropathy. Traditional treatment of these conditions has consisted of debridement, antibiotics, and immobilization with limited weight bearing. These measures are followed by long-term use of various foot and ankle bracing devices, such as the CROW walker, double metal upright, and the lined clam shell AFO with accommodative footwear either incorporated or attached. Sometimes these conservative measures fail and surgery is indicated for foot and ankle deformities with: (1) unbraceable deformi

Historical evolution in concepts, indications, application, reduction techniques, and the rehabilitation program and complications of external fixation of the wrist are discussed. Indirect reduction techniques using overdistraction and flexion are recommended. Additional stabilization of the epiphysis allows immediate reduction of overdistraction, thus eliminating the potential complications related to overdistraction. Used with the due attention to detail, external fixation of the wrist is a rewarding and efficient treatment method.

We report a series of 20 children with 24 fractures that were treated using the AO tubular external fixator in a paediatric orthopaedic unit. There were no major complications related to fixator use. There were two cases of superficial pin track sepsis and one case of non-union. There were no cases of refracture after fixator removal or of leg length inequality.
