Static and dynamic repairs of facial nerve injuries.

The patient with facial paralysis presents a daunting challenge to the reconstructive surgeon. A thorough evaluation is key in directing the surgeon to the appropriate treatment methods. Aggressive and immediate exploration with primary repair of the facial nerve continues to be the standard of care for traumatic transection of the facial nerve. Secondary repair using dynamic techniques is preferred over static procedures, because the outcomes have proved to be superior. However, patients should be counseled that facial movement and symmetry are difficult to mimic and none of the procedures de

Management of facial soft tissue injuries in children.

Pediatric facial trauma can present a challenge to even the more experienced plastic surgeon. Injuries to the head and neck may involve bone and soft tissues with an assortment of specialized organs and tissue elements involved. Because of the active nature of children, facial soft tissue injuries can be diverse and extensive as well as some of the more common injuries a plastic surgeon is asked to treat. In 2007, approximately 800,000 patients younger than 15 years presented to emergency departments around the country with significant open wounds of the head that required treatment. In this re

Nerve injuries and treatment in facial cosmetic surgery.

Surgical intervention remains a popular choice in patients seeking facial rejuvenation. Although uncommon, temporary or permanent peripheral nerve injury may complicate almost any type of invasive aesthetic procedure of the face, resulting in functional and psychological consequences for patients. Prompt recognition and appropriate intervention are necessary to avoid the long-term sequelae and improve the chances of complete neurologic recovery. Depending on the type of injury, various interventions may range from observation and close follow-up to interposition nerve grafting. This article re

Facial fractures and concomitant injuries in trauma patients.

OBJECTIVES/HYPOTHESIS: Maxillofacial fractures often occur with serious concomitant injury in trauma patients, and knowledge of the type and severity of associated injuries can assist in rapid assessment and treatment. The objective was to identify the most commonly occurring injuries associated with facial fractures in severely injured trauma patients. STUDY DESIGN: Review of medical records. METHODS: A retrospective review was made of 151 patients with facial fractures presenting to a level-one trauma center over a 4-year span. All patients had an injury severity score of 12 or greater. Facial injuries caused by animals in northern Nigeria.

We did a retrospective survey of 34 patients who sustained facial injuries after animal associated incidents and presented to three Nigerian teaching hospitals. There were 27 male (79%) and 7 female patients, whose ages ranged from 6 to 40 years. Sixteen (47%) were less than 15 years. Cows (n=14), camels (n=9), and donkeys (n=6) were the most commonly involved. Three dog bites were seen. Knocks from cows' horns (n=9) were the predominant mechanism of injury, followed by falls (n=8) and kicks (n=7). There were many facial lacerations and fractures, and the cheek (n=14), forehead (n=13), and chi

[Emergency treatment of patients with maxillo-facial injuries].

A small contribution is made to the work of widening the knowledge of the main principles followed in the emergency management of injuries to the face and jaws. The topic is rendered easily comprehensible by the nonspecialist through the use of many pictures. A short compendium of emergency therapies and personal experience is presented, and no new therapeutic indications are put forward.

Facial injury: a review of biomechanical studies and test procedures for facial injury assessment.

A review of biomechanical studies that have attempted to measure fracture tolerances of facial bones has been carried out. The particular bones of interest were the mandible, the zygoma, the maxilla and the nasal bones. Numerical values have been given for the peak force and pressure fracture tolerances for these bones. A study of these values illustrates just how variable the bone strength of various individuals is. A review of various methods that have attempted to measure and quantify the physical effects of a blow to the face has also been carried out. Three major types of test procedure e

Blast-Related Otologic and Facial Nerve Injuries in Gaza.

Explosive weapons used in densely populated settings often disproportionately injure craniofacial nerves and lead to otologic trauma. During the Israeli military invasion of Gaza, the collapse of referral pathways and surgical capacity constrained otologic and craniofacial care. The authors aimed to characterize otologic trauma and facial nerve palsy, and to identify predictors of hearing loss severity in this context. The authors conducted a retrospective cohort study of civilians with conflict-related otologic trauma presented to the Nasser Medical Complex in Palestine's Khan Yunis, Gaza Str

[Treatment of traumatic injuries of the peripheral facial nerve].

This review summarises the current knowledge of treatment strategies in traumatic extracranial facial nerve injuries. Facial nerve injuries cause significant psychologic and functional morbidity. We present a guideline for the clinical management including physical examination and surgical treatment. To regain mimetic function after facial nerve injuries it is crucial to ensure that microsurgical reconstruction is completed preferably within 24 hours, and no later than 72 hours from the time of accident.

Management of nasolacrimal duct injuries in mid-facial advancement.

Nasolacrimal duct (NLD) may be injured following trauma. During certain surgical procedure, inadvertent damage to the nasolacrimal apparatus may occur as well. Such instances should be immediately managed to prevent any subsequent complications. The management of such inadvertent injuries may be accomplished only with a thorough anatomic comprehension and surgical expertise in the field of operation. It also requires the presence of armamentarium required for its management in situations when complications arise. The case report discusses one such rare complication of NLD injury during Le Fort

Primary repair of facial dog bite injuries in children.

OBJECTIVES: The management of dog bite wounds is controversial, and current data on risk of infection are variable and inconsistent. Furthermore, the use of prophylactic or empiric antibiotics for the treatment of these wounds is debatable. We investigate the rate of wound infections and other complications after primary repair of pediatric facial dog bite injuries. METHODS: We reviewed 87 consecutive patients aged 18 years or younger who had facial dog bite injuries from January 2003 to December 2008. Variables examined were age, sex, setting of repair, number of sutures used for repair, whe

A review of facial injuries due to dog bites.

Bite injuries to the head and neck region can result in facial disfigurement with distressing physical and psychological consequences. This article reviewed the causes and management of facial bite wounds due to dog bites. A PUBMED search of the National Library of Medicine from 1995 to December 2005 was conducted. Keywords used in the search were facial wound, bite wound, dog bite. The results showed that the risk factors for dog attacks include: school-aged children, male, households with dogs, male dogs and certain breeds (german shepherds, bull terriers, blue/red heelers, dobermans and rot

An epidemiologic survey of facial fractures and concomitant injuries.

A 5-year review of facial fractures and concomitant injuries at a level 1 trauma center is presented. The anatomic location of the facial fractures, age and sex of the patients, cause of injury, and associated systems injuries are presented. The majority of facial fractures were found in males; the most prevalent age range was 16 to 30 years. Mandible fractures outranked zygomatic and maxillary fractures (6:2:1). Assaults and motor vehicle accidents were the most frequent causes of facial fractures, and lacerations followed by neurologic and orthopedic injury were the most frequently encounter

The incidence of hospital-treated facial injuries from vehicles.

The annual incidence rate of facial injuries from vehicle crashes, 278 per 100,000 residents, was determined from a population-based study involving all Dane County, Wisconsin, hospitals with emergency departments. Applying this figure to the U.S. population yields an estimated 625,000 hospital-treated facial injuries from vehicles occurring in the United States each year. Vehicle crashes were the source of a substantial proportion of facial injuries from all causes, and were found to be the single leading cause of the most severe facial lacerations and facial fractures. The majority of injuri

Review of 1,000 major facial fractures and associated injuries.

The records of 1,020 major facial fractures were reviewed, and it was found that the occurrence of life-threatening associated injuries was highly predictable on the basis of the pattern of facial fractures and the circumstances of the injury. Certain groups of patients have a high probability of associated serious injuries of the central nervous system, the trunk, or the extremities. Probably these patients should be primarily under the care of a surgeon who is capable of the diagnosis and emergency therapy of these associated injuries, as well as the facial injuries, so that appropriate prio
