Utilization of maternity waiting homes: before, during, and after the Ebola virus disease outbreak in Bong County, Liberia.

BACKGROUND: Maternity waiting homes (MWHs) are used to increase the number of women delivering at health care facilities. The first MWHs in Liberia were opened in 2012, prior to the Ebola virus disease (EVD) outbreak. METHODS: Longitudinal data were collected from registries on MWH use, antenatal care, postnatal care and facility deliveries from 2012 to 2016 to assess MWH utilization. RESULTS: All indicators examined declined during the EVD outbreak, but within 6 months of the cessation of the outbreak they returned to pre-EVD levels. CONCLUSIONS: Findings suggest MWH utilization remained s

Predictors of Retention Among HIV Exposed Infants in the Early Infant Diagnosis Program in Lilongwe Rural, Malawi.

BACKGROUND: The Early Infant Diagnosis (EID) Program exists to improve the identification of HIV-exposed infants(HEIs) to accelerate linkage to care. The trends of retention in EID have been 88%, 91% and 93% retention at 24th month age cohort in the last quarter of 2022, 2023 and 2024 respectively. However, these rates varied across health facilities, with some reporting much lower retention. This study explored factors influencing the retention of HEIs within the program. METHODS: We conducted a barrier analysis study at four facilities. We recruited 90 HIV-exposed infants 0-24 months of age

Strengthening the sustainability of neglected tropical disease programs in Rwanda: An assessment of access and utilization of domestically-financed services for soil-transmitted helminthiases and schistosomiasis.

INTRODUCTION: Soil-transmitted helminth (STH) and schistosomiasis (SCH) infections remain some of the most prevalent neglected tropical diseases (NTDs), causing significant morbidity in most of sub-Saharan Africa (SSA), including Rwanda. With dwindling international funding for NTD services and recent commitments focused on other diseases considered easier to eliminate as a public health problem, it is essential to assess domestic financing sources' scale, efficiency, and effectiveness. The study aims to strengthen domestic efforts towards sustainable financing for neglected tropical disease p

Clinical and system-level predictors of postacute care use among rural emergency general surgery patients.

INTRODUCTION: Postacute care services (PACS) are one of the fastest-growing programs funded by the Centers for Medicare and Medicaid (CMS), with potential promise to rural communities. Emergency general surgery (EGS) is essential to many hospitals. Yet little is known about post-EGS PACS utilization, especially among rural communities, which have longstanding barriers to healthcare access and greater travel distances. This study seeks to uncover drivers of PACS utilization after EGS among rural patients. METHODS: Using the 2016 to 2022 National Inpatient Sample, we conducted a retrospective c

Effects of the 2018 Japan Floods on long-term care insurance costs in Japan: retrospective cohort study.

BACKGROUND: Climate change has increased the frequency and severity of torrential rains and floods around the world. Estimating the costs of these disasters is one of the five global research priorities identified by WHO. The 2018 Japan Floods hit western Japan causing extensive destruction and many deaths, especially among vulnerable elderly. Such affected elderly would need long-term care due to the various health problems caused by the disaster. A Long-Term Care Insurance (LTCI) system provides care services in Japan. The aim of this study was to evaluate the effect of the 2018 Japan Floods

Tailoring Treatment for Substance Use Disorders in Older Adults: A Mixed-methods Study.

BACKGROUND AND OBJECTIVES: The number of persons in mid-to-late life with substance use disorders (SUDs) in the United States has tripled over the past 2 decades, with opioid use disorder largely accounting for this increase. Older individuals with SUDs have unique medical, psychological, and social needs. Despite these needs and growing demand for SUD treatment within this age group, little is known regarding availability of specialized SUD treatment programs for older persons and barriers or facilitators to implementing these services. RESEARCH DESIGN AND METHODS: We conducted a mixed-metho

The costs and financing needs of delivering Kenya's primary health care service package.

INTRODUCTION: For many Kenyans, high-quality primary health care (PHC) services remain unavailable, inaccessible, or unaffordable. To address these challenges, the Government of Kenya has committed to strengthening the country's PHC system by introducing a comprehensive package of PHC services and promoting the efficient use of existing resources through its primary care network approach. Our study estimated the costs of delivering PHC services in public sector facilities in seven sub-counties, comparing actual costs to normative costs of delivering Kenya's PHC package and determining the corr

Utilization of Maternity Services and Its Relationship with Postpartum Use of Modern Contraceptives Among Women of Reproductive Age Group in Nigeria.

BACKGROUND: Utilization of maternity services (UMS) exposes mothers to family planning (FP) counseling and other FP promotional activities. Uptake of postpartum modern contraceptives (PPMC) reduces both infant and maternal mortalities by reducing unwanted pregnancies and promoting good child spacing. Understanding the relationship between UMS and uptake of PPMC was therefore very important. PURPOSE: To determine the association between UMS and uptake of PPMC among women of the reproductive age group in Nigeria taking into consideration the influence of the place of access to the maternity ser

Women's social networks and use of facility delivery services for uncomplicated births in North West Ethiopia: a community-based case-control study.

BACKGROUND: High maternal mortality has remained an unmet public health challenge in the developing world. Maternal mortality in Ethiopia is among the highest in the world. Since most maternal deaths occur during labor, delivery, and the immediate postpartum period, facility delivery with skilled birth attendants is recommended to reduce maternal mortality. Nonetheless, the majority of women in Ethiopia give birth at home. Individual attributes and availability and accessibility of services deter service utilization. The role of social networks that may facilitate or constrain service use is n

Identifying Homelessness among Veterans Using VA Administrative Data: Opportunities to Expand Detection Criteria.

Researchers at the U.S. Department of Veterans Affairs (VA) have used administrative criteria to identify homelessness among U.S. Veterans. Our objective was to explore the use of these codes in VA health care facilities. We examined VA health records (2002-2012) of Veterans recently separated from the military and identified as homeless using VA conventional identification criteria (ICD-9-CM code V60.0, VA specific codes for homeless services), plus closely allied V60 codes indicating housing instability. Logistic regression analyses examined differences between Veterans who received these co
