Evaluation of the Prevalence of Factitious Disorder and Its Demographic and Clinical Characteristics in Psychiatric Inpatients in Iran; A Cross-Sectional Study.

BACKGROUND: Diagnosing factitious disorder (FD) poses significant medical challenges; delays impact patient care and costs. Cultural factors of each country also affect illness and behavior disorders. This study examines the prevalence, demographics, and clinical features of factitious disorder patients in Iranian psychiatric hospitals. METHODS: This cross-sectional study reviewed patient data from three psychiatric hospitals in Tehran from 2017 to 2022, confirming FD diagnoses by psychiatry faculty. Inclusion criteria were the diagnosis of FD according to ICD-10 in the last five years. We re

Case of Frontal Glioma With a Factitious Disorder of Self-Inflicting Dental Injuries Managed by Coronally Advanced Flap With Orthodontic Buttons.

Factitious disorders represent deliberately fabricated dissimulation of physical and psychological signs and symptoms seeking medical attention by the patient. Usually, they are ignorant of conventional treatment and consistently change their version of signs and symptoms. Due to various changes in the version, they do not respond to the treatment. They describe their signs and symptoms as dissimulated, imaginative, and exasperated, involving any part of the body. Gingivitis artefacta is an unusual and dramatic presentation with self-inflicted physical injury to the gingival tissues. We presen

Concepts of neurotic and personality disorders in ICD-10: results of the Research Criteria Study.

A discussion on personality disorders (F6) is conducted within the framework of the Research Criteria Study on the basis of one case each of borderline syndrome (F60.31), transsexualism (F64.0), and factitious disorder (F68.1). In the Research Criteria Study the main agreement achieved about personality disorders was 77%, for borderline disorders 94%, transsexualism 91%, and factitious disorders 53%. Additional diagnoses were given in the case of factitious disorder by 43%, for borderline disorders in 15%, and for transsexualism in only 3%. Alternative diagnoses improved the overall agreement

Clinical and demographic associations of recorded feigning in functional neurological disorder.

Although functional neurological disorder (FND) is common, increasingly recognized, potentially disabling, and treatable, it remains stigmatized, and concerns about feigning persist among clinicians. We examined the prevalence of malingering and factitious disorder diagnoses in individuals with FND, their associated demographic and clinical characteristics, and evidence of clinician bias in the diagnosis of feigning. In this retrospective cohort and case-control study using the international TriNetX electronic health record network, we analysed diagnostic codes (International Classification of

A case of factitious homicidal ideation.

Homicidal ideation is often fabricated or embellished by psychiatric patients in both the emergency room and inpatient settings. Typically, this symptom is malingered to achieve short-term hospital admission and temporary relief from complications of substance abuse, homelessness, and illicit activities. Very rarely, a patient may feign homicidal intent for the primary purpose of remaining in the role of patient (factitious disorder). Although factitious disorder with psychological symptoms has been described in a variety of circumstances, the psychiatric literature lacks any reports of factit

Prevalence and characteristics of factitious hypoglycaemia in non-diabetic patients in a department of endocrinology.

INTRODUCTION: Factitious hypoglycaemia is defined as the surreptitious use of insulin or oral hypoglycaemic agents to deliberately induce self-harm. It represents a challenging diagnosis and misdiagnosis is associated with significant morbidity and mortality. The aim of this study was to assess the prevalence and the associated factors of factitious hypoglycaemia in non-diabetic patients. METHODS: This was a single-centre, retrospective study including 70 non-diabetic patients who were admitted for the investigation of hypoglycaemia. All patients fulfilled the Whipple triad. [Factitious diseases and Münchausen's syndrome].

Factitious diseases in which the symptoms are simulated or created intentionally are very varied: functional manifestations, haemorrhages, anaemia, pyrexia, metabolic disturbances (hypoglycaemia, hypokalaemia, hyperthyroidism, etc.), skin lesions and others. Different symptomatic associations are possible. The Munchausen syndrome which is mainly seen in anglo-saxon countries, is characterised by a long history of factitious symptoms in subjects with psychological profiles which are difficult to define because of the very behaviour of the patients themselves. Besides this syndrome, many psychol

Clinical impact and misdiagnosis of functional ophthalmological symptoms: a case report.

BACKGROUND: There is a high prevalence of somatoform disorders and medically unexplained symptoms. When it comes to deciding whether a patient is able to work, it is essential to differentiate a somatoform disorder from a factitious disorder. The case presented demonstrates the impact on disability benefits and the subsequent psychosocial repercussions of misdiagnosing between a factitious disorder and a somatoform disorder. CASE PRESENTATION: A 42-year-old Caucasian woman worked as a 100% fiduciary accountant until the age of 32 when she was placed on medical leave due to persistent trigemin

A systematic review of factitious decompression sickness.

We present a case of factitious decompression sickness (DCS) involving a patient emergently treated at a hyperbaric medicine facility in New Zealand. Patients with factitious disorder feign illnesses such as DCS in order to receive care and attention despite the lack of an underlying illness. Other studies have suggested that 0.6% to as many as 9.3% of hospital admissions are factitious in nature. Therefore we believe that factitious DCS is occurring more often than hyperbaric clinicians suspect. DCS can be life-threatening, and hyperbaric medicine clinicians will almost always "err on the sid

[Self injury in adolescence].

Self-injury often occurs for the first time in adolescence. The current classification distinguishes several groups: (1) factitious disorder imposed on self and factitious disorder imposed on another, (2) nonsuicidal self-injury (NSSI) and (3) obsessive-compulsive (OCD) and related disorders. The differential diagnosis includes a wide spectrum of psychiatric diseases, including borderline personality disorder (BPD), inherited disorders like Lesch-Nyhan syndrome (LNS) and also body modifications. In all these cases, self-harm is a subphenomenon. This review focuses on typical dermatological pic

A case of comorbid anorexia nervosa, bulimia nervosa, and Munchausen's syndrome.

Reports of comorbid eating disorders and Munchausen's syndrome are rare. As part of a follow-up study of the outcome of anorexia nervosa, medical records and direct structured psychiatric interview data were collected on all women treated for anorexia nervosa in Christchurch, New Zealand between 1981-1984. On interview, one participant received diagnoses of anorexia nervosa, bulimia nervosa, and factitious disorder (Munchausen's syndrome). The subject interwove symptoms of her eating disorders with factitious presentations on several occasions. This diagnosis had not been apparent prior to com

Nasal manifestations of self-destructive behaviour.

Self-destructive behaviour is prevalent in a variety of different psychiatric disorders. Most common manifestations are self-inflicted cuts to the skin, especially the skin of the forearms. Manifestations of self-destructive behaviour involving the nasal area are rather rare. A series of cases is presented in which nasal diseases were initially misinterpreted, but could finally be identified as factitious disorders. Presented symptoms were foreign body insertions, hemodynamically relevant epistaxis and impetiginous inflammations of the paravestibular skin of the nose. Factitious disorders of t

Factitial pemphigus-like lesions.

The maxillofacial region is rarely subjected to self-inflicted conditions such as factitious disease. Nasal ulceration, facial emphysema, periorbital ecchymosis, mandibular subluxation, gingival and mucosal ulceration, dental and salivary gland pain and glossopharyngeal neuralgia have been reported as possible manifestations of factitious disease. We report a case of a young woman who presented with unilateral bullous and ulcerative oral and erythematous facial lesions that were initially diagnosed as pemphigus vulgaris but was later determined to be secondary to self-inflicted injuries. Patients with medically unexplained symptoms: DSM-III diagnoses and demographic characteristics.

This study reports DSM-III diagnoses and demographic characteristics of 100 patients consecutively referred to a university hospital consultation-liaison service for evaluation of medically unexplained symptoms suggesting physical disorders. Thirty-seven percent of patients received diagnoses of somatoform, dissociative, or factitious disorders, and 14% were felt to have psychologic factors affecting physical conditions. Although black and male patients were less often referred for medically unexplained symptoms, once referred they were more likely than white and female patients to receive dia

Gaslighting in alleged assault ascertained as Munchausen syndrome transcending to malingering in highly counterintuitive self inflicted acid burns.

A factitious disorder leading to the self-infliction of highly counter-intuitive burns was diagnosed in a middle-aged female. The injuries were otherwise alleged to have been sustained by assault inflicted upon her by an unknown person. The case was diagnosed by medico-legal interpretation of injuries, in spite of a highly deceptive and concocted history by the patient and her husband. The entity was unique in being associated with magnificent primary, secondary and tertiary gains. The exploitation of the morbid sequel to malinger by the patient, and the involvement of the husband for the prol

Factitial illness mimicking rheumatic disease with panniculitis.

Factitial disorders can present with a wide spectrum of clinical features and might be overlooked. We identified two patients believed to have factitial disorders mimicking panniculitis at one medical center within 15 months. Both were females in their early forties who had long histories of cutaneous ulcerations, recurrent cellulitis, abscesses, atrophy, and scarring with sparing of inaccessible body areas. They had a) undergone surgical interventions during multiple hospitalizations, b) demonstrated no response to apparently appropriate medical management, c) had thick charts, clinical depre

[Behçet's disease and factitious manic-depressive psychosis: a case of Münchausen syndrome].

BACKGROUND: Munchausen syndrome is frequently observed in men unlike other factitious conditions. The patient presents a characteristic triad: apparently acute but factitious disorders, migration from hospital to hospital resulting in unnecessary explorations and treatments, and fabulated medical history. CASE REPORT: A 57-year-old man was hospitalized in the psychiatric unit for alleged insomnia, psychomotor excitation and multiple hallucinatory phenomena. The factitious nature of the patientís condition was rapidly suspected in light of the large number of previous unconfirmed medical condi

Factitious hypoglycemia in children and adolescents with diabetes.

BACKGROUND: Factitious hypoglycemia is a condition of self-induced hypoglycemia due to surreptitious administration of insulin or oral hypoglycemic agents. In adults, it is an uncommon, but well known clinical entity observed in individuals with and without diabetes. OBJECTIVES: To report a case of factitious hypoglycemia highlighting diagnostic pitfalls, to identify common characteristics of children and adolescents with factitious hypoglycemia, and to examine whether the information on long-term outcome exists. Psychiatric and behavioral correlates of factitious blindness.

In factitious disorder, an individual feigns, exaggerates, or actually self-induces physical or psychiatric illness to achieve ends such as mobilizing care and concern, ventilating aggression, diminishing guilty feelings, or gratifying dependency wishes. We present 2 new cases of factitious binocular blindness, which has rarely been reported but readily illustrates the dramatic range of factitious illness behaviors. Psychiatric and behavioral correlates culled from these cases include extravagant claims about the impairment and ludicrous claims for nonvisual sensory abilities. These patients t
