Familial Adenomatous Polyposis: Development, Presentation, and Treatment Strategies.

Familial adenomatous polyposis (FAP) is an inherited disorder that typically presents with multiple polyps in the colon. These polyps become cancerous if not monitored in the early stages or if left untreated. For those with a family history of FAP, colonoscopy screenings begin in adolescence and continue throughout adulthood because polyps can arise at an early age. Colon cancer in untreated individuals develops before age 40 years. This topic warrants discussion to aid in the prompt recognition of symptom presentation. This article will address the development of FAP, its presentation, and p

STD/HIV prevention practices among primary care clinicians: risk assessment, prevention counseling, and testing.

OBJECTIVE: To describe current practices of primary care (PC) clinicians for STD/HIV control services: risk assessment, prevention counseling, and offering tests. STUDY DESIGN: We identified clinical strategies through qualitative interviews. We then surveyed by mail a random sample of Washington State family physicians, general internists, obstetrician-gynecologists, nurse practitioners, and certified nurse midwives. We identified characteristics of clinicians and their practices associated with each strategy and universal provision of each service. RESULTS: We report on 519 clinicians (80%

Life events and postpartum depressive symptoms among Black and Latina mothers: The role of protective factors.

Approximately, 15% of women in the United States experience postpartum depression, but Black and Latina women continue to suffer at disproportionately higher rates. While literature has shown that the experience of life stressors is a strong predictor of postpartum depressive symptoms, less research has examined personal and contextual resources that can protect minoritized groups from poor mental health outcomes during the perinatal period. The current article investigated whether four types of protective factors (family support, doctor support, nurse support, and perceived control) moderated

Health and homelessness: weaving a net of care as a specialist practitioner.

Reflective practice was used as a process of self-inquiry and transformation in a PhD study of my specialist role in single and family homelessness. New to homelessness when I embarked on the study, there was little to guide my nursing practice and, notably, there was insufficient research evidence to demonstrate the effectiveness of specialist practice with homeless people. Through my reflexive narrative study, that research gap was addressed. The aim of the narrative was to inform the reader of the reflexive journey and my transformation as a specialist practitioner in homeless health care,

Refractory cachexia and truth-telling about terminal prognosis: a qualitative study.

The purpose of this paper is to examine the consequences that medical practitioners' decisions about whether or not to be candid about terminal prognosis have for those suffering from refractory cachexia and their families. It presents the findings of a qualitative study that used focus groups and semi-structured interviews of a volunteer sample of doctors, nurses and dieticians in a cancer centre of a large teaching hospital in Northern Ireland. Respondents reported that some physicians tended to avoid discussing terminal prognosis in a direct manner with their patients. Nurses and dieticians

Cerebral palsy: introduction and diagnosis (part I).

Cerebral palsy (CP), a static, nonprogressive disorder caused by brain insult or injury in the prenatal, perinatal, and postnatal time period, is the major developmental disability affecting function in children. It is characterized by the inability to normally control motor functions, and it has the potential to have an effect on the overall development of a child by affecting the child's ability to explore, speak, learn, and become independent. Effective management can improve the quality of life for the child and family. The first step for the nurse practitioner is to understand the definit

Nurses as substitutes for doctors in primary care.

BACKGROUND: Current and expected problems such as ageing, increased prevalence of chronic conditions and multi-morbidity, increased emphasis on healthy lifestyle and prevention, and substitution for care from hospitals by care provided in the community encourage countries worldwide to develop new models of primary care delivery. Owing to the fact that many tasks do not necessarily require the knowledge and skills of a doctor, interest in using nurses to expand the capacity of the primary care workforce is increasing. Substitution of nurses for doctors is one strategy used to improve access, ef

[Spontaneous fracture: a potential clinical concern].

Spontaneous fracture is an issue of increasing concern in clinical care. However, this topic has received only limited attention in nursing research. Gaps in understanding related to spontaneous fractures may increase medical-legal risks faced by frontline care nurses, exacerbate nursing pressures, and serve as a disincentive to remain in the already understaffed nursing profession. This article reviews previous research on spontaneous fractures to determine the epidemiological causes of clinical spontaneous fracture and identify effective prevention strategies. We hope this paper may increase

Providing nursing care for a Chagga client of Tanzania.

A case study is used to present features of a cultural assessment of the Chagga tribe. The purpose is to identify guidelines for provision of culturally congruent nursing care. "Mrs. Chuwa" lives on the foothills of Kilimanjaro. Daily survival is a major concern; the extended family is an important source of support. Traditional beliefs have a major impact on her life; traditional practitioners are alternative resources to dispensaries and clinics. Nurses providing care for Chaggas need to be aware of how important it is to communicate with respect, to obtain an accurate treatment history, and

Hair loss in children and adolescents.

No other body system is more exposed to the public eye than the skin and the hair. Pediatric nurse practitioners (PNPs) frequently encounter patients in their practice settings with the complaint of "hair loss." The most often diagnosed causes of hair loss include tinea capitis, alopecia areata, traction alopecia, and trichotillomania. This article defines these conditions, explores the causes for them, and describes the incidences. Emphasis is placed on the PNP's role on making a differential diagnosis based on the presenting clinical manifestations, patient history, and laboratory testing. T

Interpreting Context in Rural and Remote Aged Care Facilities in Readiness for a New Care Model: A Mixed Method Study.

BACKGROUND: Geographical isolation compounds limited access to healthcare services and skilled workforce for the provision of rural aged care. Residents have complex chronic disease management and end-of-life care needs. An undersupply of general medical practitioners due to retirement, attrition or unfilled training places in Australia has impacted recruitment to rural areas. Nurse practitioners have been identified as a potential solution. AIM: To describe and explore the inner (local and organisational) and outer (wider health system) contexts of healthcare, from the perspective of care st

Mapping Geospatial Gaps in Early Identification of Children With Autism Spectrum Disorder.

Despite the known developmental benefits of early intervention for autism spectrum disorder (ASD), diagnosis before age 5 years is often not achieved. Research suggests that lack of health insurance and living in rural areas and areas of severe provider shortages contribute significantly to these delays. The purpose of this project was to conduct a geospatial evaluation of potential gaps in early ASD diagnosis of uninsured children in Virginia. A secondary purpose was to show the use of geospatial analysis by pediatric nurse practitioners for policy advocacy. We mapped data from a statewide pr

The battered woman: is she a silent victim?

Although hardly a recent phenomenon, the battered woman has still not been a clearly recognizable presence to primary caregivers. Unawareness of actual victims stems from an incomplete understanding of the reality of the "battered wife syndrome," lack of knowledge about the various forms of abuse and the silence of the victims themselves. The victim rarely reports abuse to the primary care provider without being asked. She does, however, present with common psychosomatic complaints, such as choking, gastrointestinal disorders or nervousness. Her injuries may range from bruises to fractures, an

Advanced practice clinicians' interest in providing medical abortion: results of a California survey.

CONTEXT: California's Reproductive Health Privacy Act, which became law in January 2003, clarified that advanced practice clinicians could legally provide medical abortion. Little is known about the characteristics associated with nonphysician clinicians' interest in receiving medical abortion training or their perceptions of barriers to medical abortion provision. METHODS: In early 2003, a total of 1,176 licensed advanced practice clinicians in California-nurse practitioners, physician assistants and certified nurse-midwives-completed a mail-in survey assessing their personal characteristics

Hidden work and blurred boundaries: a qualitative study of how community nurses navigate and adapt injectable medication processes to provide timely end-of-life symptom control.

BACKGROUND: With rising numbers of people dying at home, often with complex multimorbidity, community healthcare professionals are increasingly responsible for providing end-of-life care. This care is usually led by community nurses, many of whom do not have specialist palliative care training. A core part of their role is providing end-of-life symptom control using injectable medications prescribed ahead of need and stored in patients' homes. However, cross-organisational systems around injectable medication processes are complex and time consuming. We aimed to explore community nurses' perce

Supporting Nurse Participation in Deprescribing: A Qualitative Study of Long-Term Care Nurses in Alberta, Canada.

BACKGROUND: Long-term care (LTC) nurses are well positioned to support deprescribing given their integral role in resident care. However, there are numerous challenges to nurse involvement in deprescribing and limited research on supports to improve participation. OBJECTIVES: The objective of this study is to explore nurses' perspectives on deprescribing in LTC and identify supports that may improve their involvement. METHODS: In this qualitative descriptive study, progressive group discussions were conducted with 32 nurses at two LTC care facilities in Edmonton, Canada. Participants include

A nurse practitioner as the first point of contact for urgent medical problems in a general practice setting.

OBJECTIVES: The aim of this study was to examine the suitability of nurse practitioners for assessing and managing urgent clinical problems presenting in primary care. METHODS: Patients registered at a suburban group practice presenting with acute medical problems were offered the choice of seeing a GP or a nurse practitioner. The outcomes of 1000 consultations were analysed by recording the repeat consultation rate, the prescription-issue rate and the rate of referral to secondary care, as well as investigating patient satisfaction and the number of dysfunctional consultations and misdiagnos

Pancreatic disorders in the newborn.

Except for the hyperinsulinism associated with the infant of a diabetic mother (accounting for about 5 percent of NICU admissions annually), pancreatic disorders of the newborn are rare. Congenital anomalies (such as annular pancreas) and endocrine disorders (such as hyperinsulinism of nesidioblastosis or hyperglycemia of neonatal diabetes mellitus) present many challenges to the personnel caring for these infants and their families. The potential mortality and morbidity of these disorders make it imperative for nurses and nurse practitioners working with infants to recognize and understand pa
