Familial hypercholesterolemia treatments: Guidelines and new therapies.

Familial hypercholesterolemia (FH) is a genetic disorder resulting from mutations in genes encoding proteins involved in the metabolism of low density lipoproteins (LDL) and characterized by premature cardiovascular disease due to the exposure to high levels of LDL-cholesterol (LDL-C) from birth. Thus, the early identification of FH subjects, followed by appropriate treatment is essential to prevent or at least delay the onset of cardiovascular events. However, FH is largely underdiagnosed; in addition, FH patients are frequently not adequately treated, despite the availability of several phar

Therapy for families manifesting manic-depressive behavior.

This is a companion piece to the article "Some Features of Families with Major Affective Disorders," published in Family Process (25: 325-336, 1986). In addition to the family features mentioned in the first article, the authors report on other features that have come to the fore since then. Subsequently, they deal with the therapeutic problems that derive from all of these features. In particular, they elaborate on how the therapists must (and can) maintain their neutrality in the face of the massive polarizations and extremes of the either/or thinking found in these families. There follows a

[The snowflake family - Integrative play therapy for traumatized child with complex developmental disorder induced by family environment. Case study].

In the case of a little preschool boy, I would like to show how traumatic environmental influences affect a child's development, coping strategies, as well as his symptomatic behavior, emotion and mood regulation, and social functioning. This little boy has also suffered many damaging environmental effects during his own life, losses, stressful situations, conflicts. In addition, the losses, traumas and injuries suffered by his parents also shaped his development, the development of his difficulties and symptoms. The range of early fractures was compounded by the trauma at Ákos, the loss of hi

Family Focused Therapy for Family Members of Patients with Bipolar Disorder: Case Reports of Its Impact on Expressed Emotions.

Caregivers of patients with bipolar disorder (BD) undergo a considerable amount of burden. In India, family caregivers are the primary source of support and care for their ill relatives. The burden faced by family members of patients with BD often results in physical and mental health consequences. This may lead to negative interaction patterns such as hostility, criticality, and overinvolvement, termed as expressed emotions (EE). Here, we report how we addressed the EE in family members, using a single-subject design that involved the family caregivers (n = 2) of two adults who presented with

FPI Writers Fellowship Special Section: Expanding the Lens in Family Therapy.

The Family Process Institute (FPI) Writers Fellowship supports the development of scholars who will advance systemic theory, research, and clinical practice grounded in a commitment to social justice. In this introduction to the Writers Fellowship special section, I describe the rationale for and history of this FPI initiative and how these eight authors help expand whose perspectives, interests, and voices are included in the family therapy field's knowledge base. I highlight the broader systemic/relational lenses the authors bring to issues and communities that are often individualized or ov

Multifunctional PDA-graphene family nanocomposites for antibacterial and anticancer photothermal therapy.

The increasing demand for multifunctional, biocompatible nanomaterials has spurred the exploration of hybrid systems with synergistic antimicrobial, anticancer, antioxidant, and regenerative properties. In this study, polydopamine (PDA)-based nanocomposites incorporating graphene oxide (PDA-GO) and graphene quantum dots (PDA-GQD) were synthesized and systematically characterized for their physicochemical and biological functionalities. The nanocomposites raised temperature > 50 °C within 5 min under 808 nm laser irradiation (1.5 W/cm2, 250 μg/mL). Both composites showed antibacterial activity

Family experience of palliative sedation therapy: proportional vs. continuous deep sedation.

PURPOSE: Some patients experience intense symptoms refractory to intensive palliative care, and palliative sedation is sometimes used. Palliative sedation may be classified into proportional and continuous deep sedation (CDS). The primary aim of this study was to compare family experience between families of patients who received proportional or CDS. METHODS: A multicenter questionnaire survey was conducted involving bereaved families of cancer patients who received proportional or CDS based on a sedation protocol. Overall evaluation of sedation (satisfaction, family-perceived distress, appro

Does adjunctive family therapy enhance recovery from bipolar I mood episodes?

BACKGROUND: Family therapy is sometimes used as adjunctive treatment to pharmacotherapy to help patients recover from mood episodes of bipolar I disorder. However, the efficacy of this practice is not known. METHODS: Ninety-two patients meeting criteria for a current bipolar I mood episode were randomly assigned to family therapy plus pharmacotherapy, multifamily psychoeducational group therapy plus pharmacotherapy, or pharmacotherapy alone. Time to recovery was analyzed with survival analysis. RESULTS: The proportion of subjects within each treatment group who recovered did not significantl

Transforming the legacies of childhood trauma in couple and family therapy.

A multi-theoretical couple/family therapy clinical social work practice model synthesizes various social, family, trauma, and psychodynamic theories to inform a biopsychosocial assessment that guides clinical interventions. The client population involves adult partners who have negotiated the impact of childhood trauma, i.e., physical, sexual, and emotional abuses, including culturally sanctioned trauma. Couples may also be dealing with the aftermath of acute trauma related to interpersonal violence, political conflict, and/or the dislocations related to refugee or new immigrant status. Clinic

Bolstering family therapy: Assimilating EMDR into ABFT in youth residential treatment.

The psychotherapy field has a long history of integration to improve treatment effectiveness. One type, assimilative integration, offers innovative opportunities to family therapy to incorporate the clinical and research contributions of different approaches. This paper contributes to the literature on integration by exploring how Eye Movement Desensitization and Reprocessing (EMDR) can be assimilated into Attachment-Based Family Therapy (ABFT) for youth in residential psychiatric treatment. ABFT seeks to improve attachment security to parental figures by asking adolescents and young adults to

Formulation in eating disorder focused family therapy: why, when and how?

In the broadest sense, formulation, or case conceptualisation, is the process of collaborative sense-making. It is something all clinicians are doing constantly, whether formally and consciously or not. Most clinicians are familiar with the general principals and need to formulate as it is a core part of clinical training across most disciplines. Treatment manuals often discuss formulation, but the practical steps involved and the process of developing, building upon and revising formulations are not always provided. In clinical practice, the active use and continued development of formulation

Family Therapy for Child and Adolescent Eating Disorders: A Critical Review.

Eating disorder-focused family therapy has emerged as the strongest evidence-based treatment for adolescent anorexia nervosa, supported by evidence from nine RCTs, and there is increasing evidence of its efficacy in treating adolescent bulimia nervosa (three RCTs). There is also emerging evidence for the efficacy of multifamily therapy formats of this treatment, with a recent RCT demonstrating the benefits of this approach in the treatment of adolescent anorexia nervosa. In this article, we critically review the evidence for eating disorder-focused family therapy through the lens of a moderate

Eating disorders, parent-child conflicts, and family therapy in Shenzhen, China.

In this article, I report the results of a qualitative study conducted in Shenzhen, China, in which I aimed to (a) identify the characteristics of parent-child conflicts between Shenzhen youths suffering from eating disorders and their parents and (b) contextualize these intergenerational conflicts to understand their meanings within the immediate sociocultural context. I have employed multiple-case studies as the research design. Data have been collected through reviews of the family treatment sessions for ten families, each with an adolescent daughter suffering from eating disorders, and wit

Engaging suffering: towards a mindful re-visioning of family therapy practice.

The myth of problem-free living is easily identifiable in Western culture through its childhood fairy tales and modern love stories. Even if inadvertently, the field of mental health perpetuates this myth with the very concept of "mental health," which implies a state without suffering. Using mindfulness, Buddhists assume an alternative stance towards suffering: rather than eliminate it, they compassionately engage it. This article explores the potentials of mindfulness as an alternative approach to suffering within the context of marriage and family therapy. Family therapists can integrate mi

Family attachment narrative therapy: healing the experience of early childhood maltreatment.

Based on attachment theory and research, Family Attachment Narrative Therapy is introduced as a new family therapy modality developed to heal the experience of early childhood maltreatment. Unresolved childhood trauma has been correlated with impaired and delayed cognitive, behavioral and emotional functioning. Gentle, soothing, nonprovocative and nonintrusive narratives told by parents provide an alternative restorative experience designed to shift and change the child's destructive internal working model. The result is improved functioning and the ability to accept nurturing and care in rela

Behavioral, cognitive, and family therapy for obsessive-compulsive and related disorders.

Behavioral therapy and cognitive therapy, individually and combined, are a solid base in any therapy, the goal of which is to decrease the maladaptive behaviors associated with obsessive-compulsive spectrum disorders. Future research into this area involves two branches: (1) better resolution in what components of current treatments are effective and (2) a better understanding of the cause of OCD. The therapies of choice are behavioral therapy and cognitive therapy, but often what is described as behavioral therapy and cognitive therapy varies. Further refinement of the specific components of
