Short report: assessing field vaccine efficacy for measles in famine-affected rural Ethiopia.

Measles is a major cause of mortality in complex emergencies. Both high vaccination coverage and vaccine efficacy are required to prevent major epidemics of measles in such situations. Evaluation of field vaccine efficacy is a critical but underutilized component of program monitoring in emergencies, and is particularly important in rural areas where the integrity of the cold chain is difficult to guarantee. In July 2000, we evaluated the field vaccine efficacy for measles vaccination by comparing the incidence of cases in vaccinated and unvaccinated groups during a two-stage cluster survey of

Differential Epigenetic Regulation of Glutamatergic Synapse Pathway in Adults With Prenatal Exposure to Famine.

It has been hypothesized that poor nutrition during prenatal growth may alter the development of vital organs like the brain, thus "program" predisposition to certain diseases later in life, including mental disorders. Although with support from studies using animal models and epidemiologic observations, the biological aspect of the hypothesis has been rarely studied in humans. Using famine as a natural experiment, we explored the altered DNA methylation patterns in genes of the glutamate synapse pathway in whole blood of adults born during the Chinese famine of 1959-1961. We detected signific

Impact of Early Life Famine Exposure on Body Composition and Metabolic Profiles in Adulthood.

The relationship between the famine and metabolic syndrome has been reported, but there is a lack of more detailed changes in metabolic profiles. It is unclear how famine affects body composition. This study included 21,142 participants from the China National Health Survey. The body mass index (BMI), fat mass index (FMI), and fat-free mass index (FFMI) were calculated. Systolic blood pressure (SBP), diastolic blood pressure (DBP), blood lipids, and fasting blood glucose (FBG) were measured. Multivariate adjusted linear regression models were used to assess the association between famine and o

Association of Famine Exposure on the Changing Clinical Phenotypes of Primary Hyperparathyroidism in 20 years.

BACKGROUND & AIMS: Primary hyperparathyroidism(PHPT) has been evolving into a milder asymptomatic disease. No study has assessed the association between famine exposure and such a shift. We aim to explore the effects of China's Great Famine exposure on the changing pattern of PHPT phenotypes. METHODS: 750 PHPT patients diagnosed from 2000 to 2019 were studied. The clinical presentations were compared between them in recent 10 years (2010-2019) and previous 10 years (2000-2009). Participants were then categorized into fetal, childhood, adolescent, adult exposure, and unexposed groups. Logistic

[Link between prenatal exposure to the "Winter of Famine" and long-term medical consequences].

The 'foetal origins' hypothesis (Barker hypothesis) proposes that undernutrition of the foetus during critical periods of development increases the risk of coronary heart disease in later life. The Dutch famine caused by the second World War in the winter of 1944--although a historical disaster--provides a unique opportunity to study effects of undernutrition during gestation in humans. The health of people born around the time of the Dutch famine, in the Wilhelmina Gasthuis in Amsterdam, of whom detailed birth records exist, was assessed. There were indications that chronic diseases in later

Longitudinal changes in late-life brain health after prenatal exposure to the Dutch famine.

BACKGROUND: The consequences of harmful prenatal exposures for brain health may last a lifetime. Previous studies showed smaller adult brain volumes and altered perfusion at age 68 after prenatal famine exposure, particularly in men. To investigate whether the previously observed effects reflect persistent developmental effects or accelerated brain aging, we here studied longitudinal changes in brain volumes, white matter integrity, white matter hyperintensities and perfusion between ages 68 and 74 in individuals exposed and unexposed to famine in early gestation. METHOD: Brain MRI scans were

Early life exposure to Chinese famine modifies the association between hypertension and cardiovascular disease.

OBJECTIVE: Studies have shown that famine exposure during early life may increase the risk of cardiovascular disease (CVD), hypertension, and diabetes during adulthood. We aimed to assess whether exposure to the Chinese famine (1959-1961) modifies the association between hypertension and CVD. METHODS: We investigated data of 5772 adults born between 1954 and 1964 from the China Health and Retirement Longitudinal Study. CVD was based on self-reported doctor's diagnosis of heart problems (heart attack, coronary heart disease, angina, congestive heart failure, or other heart problems) and stroke

Self-reported depression and anxiety after prenatal famine exposure: mediation by cardio-metabolic pathology?

Evidence from previous studies suggests an association between prenatal exposure to famine and increased risk for depression. The aim of this study was to investigate whether prenatal exposure to the Dutch famine is associated with self-reported depression/anxiety and whether a potential association is mediated by the presence of cardio-metabolic disease. A total of 819 persons, born as term singletons around the 1944-1945 Dutch famine, filled out the Hospital Anxiety and Depression Scale (HADS) and were asked about their medical history. As indicators of cardio-metabolic disease we included t

Risk factors for measles mortality among hospitalized Somali refugees displaced by famine, Kenya, 2011.

BACKGROUND: Measles among displaced, malnourished populations can result in a high case fatality ratio. In 2011, a large measles outbreak occurred in Dadaab, Kenya, among refugees fleeing famine and conflict in Somalia. The aim of this study was to identify predictors of measles deaths among hospitalized patients during the outbreak. METHODS: A retrospective cohort study design was used to investigate measles mortality among hospitalized measles patients with a date of rash onset during 6 June-10 September 2011. Data were abstracted from medical records and a measles case was defined as an il

Exposure to Chinese famine in early life modifies the association between hyperglycaemia and cardiovascular disease.

BACKGROUND AND AIMS: The Great Leap Forward Famine during 1959-1961 was the world's largest famine, and its adverse long-term effects might be more apparent in the coming decade with ageing of the exposed populations. The aim of this study was to examine whether the Chinese Famine modified the effect of hyperglycaemia on cardiovascular disease (CVD). METHODS AND RESULTS: We used data of 4337 adults born between 1952 and 1964 collected from the China Health and Retirement Longitudinal Study (CHARLS). Logistic regression was used to estimate the odds ratios (ORs) and confidence intervals (CIs)

[Effects of the periodical spread of rinderpest on famine, epidemic, and tiger disasters in the late 17th Century].

This study clarifies the causes of the repetitive occurrences of such phenomena as rinderpest, epidemic, famine, and tiger disasters recorded in the Joseon Dynasty Chronicle and the Seungjeongwon Journals in the period of great catastrophe, the late 17th century in which the great Gyeongsin famine (1670~1671) and the great Eulbyeong famine (1695~1696) occurred, from the perspective that they were biological exchanges caused by the new arrival of rinderpest in the early 17th century. It is an objection to the achievements by existing studies which suggest that the great catastrophes occurring i

The developmental origins of ageing: study protocol for the Dutch famine birth cohort study on ageing.

INTRODUCTION: Evidence from animal studies suggest that the rate of ageing may be influenced not only by genetic and lifestyle factors, but also by the prenatal environment. We have previously shown that people who were exposed to famine during early gestation performed worse on a selective attention task, which may be a first sign of cognitive decline, and were on average 3 years younger at the time of coronary artery disease diagnosis. Women in this group seem to die at a younger age. We hypothesise that an accelerated ageing process, set in motion by the poor prenatal environment, underlies

The Dutch famine, 1944-1945, and the reproductive process. I. Effects on six indices at birth.

Prenatal exposure to the Dutch famine of 1944-1945 reduced postpartum maternal weight, birth weight, placental weight, length at birth, and head circumference at birth. These conclusions are based on maternities occurring in the famine area and exposed to famine during some part of gestation as compared both with maternities in nonfamine areas, and with maternities before and after the famine. Third trimester exposure accounted for the whole of the famine effects. Length of gestation was not affected by third trimester exposure, and did not mediate the effects on infant exposure, and did not m

Fetal exposure to the Ukraine famine of 1932-1933 and adult type 2 diabetes mellitus.

The short-term impact of famines on death and disease is well documented, but estimating their potential long-term impact is difficult. We used the setting of the man-made Ukrainian Holodomor famine of 1932-1933 to examine the relation between prenatal famine and adult type 2 diabetes mellitus (T2DM). This ecological study included 128,225 T2DM cases diagnosed from 2000 to 2008 among 10,186,016 male and female Ukrainians born from 1930 to 1938. Individuals who were born in the first half-year of 1934, and hence exposed in early gestation to the mid-1933 peak famine period, had a greater than t

Perinatal famine is associated with excess risk of proliferative retinopathy in patients with type 2 diabetes.

PURPOSE: Intrauterine undernutrition is associated with increased risk of type 2 diabetes. Children born premature or small for gestational age were reported to have abnormal retinal vascularization. However, whether intrauterine famine act as a trigger for diabetes complications, including retinopathy, is unknown. The aim of the current study was to evaluate long-term effects of perinatal famine on the risk of proliferative diabetic retinopathy (PDR). METHODS: We studied the risk for PDR among type 2 diabetes patients exposed to perinatal famine in two independent cohorts: the Ukrainian Nati
