Anal Sphincter Defect and Fecal Incontinence.

Anal sphincter defects can lead to fecal incontinence. The relationship between the extent of defect and continence is controversial. Magnetic resonance imaging (MRI) of the pelvis can assess anal sphincter defects. Transrectal ultrasonography (TRUS) is used to assess sphincter integrity in adults. We present a 17-year-old male with history of sexual abuse, rectal prolapse, and fecal incontinence. MRI showed a small defect that did not explain his clinical presentation. TRUS identified more extensive defects which were not picked up by MRI. The patient had rectopexy, and his rectal prolapse an

Fecal Incontinence: Etiology, Diagnosis, and Management.

INTRODUCTION: Fecal incontinence is a debilitating condition affecting primarily the elderly. Many patients suffer in silence resulting in both underdiagnosis and undertreatment often culminating in an overall poor quality of life. METHODS: We sought to review the etiology, diagnosis, and treatment of fecal incontinence based on current literature. Additionally, newer treatment methods such as Solesta will be evaluated. RESULTS: There are many diagnostic modalities available to assess the degree and severity of the patient's incontinence; however, a thorough history and physical exam is crit

Overt rectal prolapse and fecal incontinence.

PURPOSE: Rectal prolapse is frequently associated with fecal incontinence; however, the relationship is questionable. The study was designed to evaluate fecal incontinence in a large consecutive series of patients who suffered from rectal prolapse, focusing on both past history, anal physiology, and imaging. METHODS: Eighty-eight consecutive patients who suffered from an overt rectal prolapse (72 women, 16 men; mean age, 51.1 +/- 19.5 years) as a main symptom were analyzed; 48 patients also experienced fecal incontinence compared with 40 without incontinence. Logistic regression analyses were

[Sacral nerve stimulation in fecal incontinence].

INTRODUCTION: Sacral nerve stimulation for the treatment of faecal incontinence has gained increasing use in Europe over the last two years. Experience with the first patients treated in Denmark is described here. MATERIAL AND METHODS: Fourteen patients with severe faecal incontinence were given sacral nerve stimulation. The first treatment was temporary, and if this was successful they had a device for permanent stimulation implanted. RESULTS: The result of the test stimulation was good in ten of the 14 patients and a permanent system was implanted. After a median of 4.5 months' stimulation

Ischemic fecal incontinence and rectal angina.

In 36 patients who consulted for fecal incontinence or rectal pain, or both, there was grossly visible scarring of the rectum and biopsy revealed mucosal atrophy and fibrosis. A steal from the hemorrhoidal arteries to the iliac vessels was demonstrated in 3 subjects. Maximum tolerable volumes within a rectal balloon were smaller than in control subjects, both in men (192 vs. 273 ml) and in women (142 vs. 217 ml) (p less than 0.01). The rectoanal inhibitory reflex was abnormal in all but 1 patient. Specific abnormalities were a decreased amplitude or a prolonged duration of the reflex. It was t

A Content Incontinent: Report of Liposomal Bupivacaine Induced Fecal Incontinence.

Proper surgical management of anal fistula demands sound clinical judgment and extraordinary care to prevent incontinence and adequate postoperative pain control and provide satisfactory resolution to optimize quality of life. Fecal incontinence can be a devastating complication of procedures performed for fistula in ano. We report a unique case in which temporary incontinence (for less than 4 days) followed injection of liposomal bupivacaine for postoperative pain control after draining seton placement for fistula in ano. Patients and physicians should be aware as it may be mistaken for a mor

Fecal Incontinence Drives the Psychosocial Burden in Patients with Dual Incontinence.

INTRODUCTION AND HYPOTHESIS: Urinary incontinence (UI) and fecal incontinence (FI) are debilitating conditions affecting patients' quality of life (QOL). Dual incontinence (DI) of both urine and stool represents an additional burden. We compare the physical and mental health of patients with UI, FI, and DI. We hypothesize that DI might be associated with increased psychosocial burden. METHODS: Patients completed online questionnaires on sociodemographics, urinary and bowel symptoms, and psychosocial comorbidity. The Patient-Reported Outcomes Measurement Information System (PROMIS) measures ph

Surgical approaches to fecal incontinence in the adult.

Surgical treatment of anal incontinence is indicated only for patients who have failed medical treatment. Sphincterorraphy is suitable in case of external sphincter rupture. In the last decade, sacral nerve stimulation has proven to be a scientifically validated solution when no sphincter lesion has been identified and more recently has also been proposed as an alternative in cases of limited sphincter defect. Anal reconstruction using artificial sphincters is still under evaluation in the literature, while indications for dynamic graciloplasty are decreasing due to its complexity and high mor

Neoappendicostomy in the management of pediatric fecal incontinence.

PURPOSE: The Malone appendicostomy, for antegrade enemas, has improved the quality of life for many children with fecal incontinence. In patients whose appendix has been removed, a neo-appendix can be created. We describe our approach and experience with this procedure as an option for surgeons managing children with fecal incontinence. METHODS: The procedure involves creating a transverse flap of cecum that receives its blood supply by a transverse mesenteric branch. This flap is then tubularized around a feeding tube. The surrounding colon is plicated around the neo-appendix to prevent leak

[Quality of life of patients with fecal incontinence].

Using the Gastrointestinal Quality of Life Index (GIQLI) we investigated the quality of life in a consecutive series of 35 patients with faceal incontinence compared with patients suffering from haemorrhoidal disease (n = 96), fissure in ano (n = 38) and healthy controls (n = 75). The mean score for the different groups were as follows: incontinence 93 (65% of best possible score of 144), haemorrhoids 120 (83%), fissure 104 (72%) and controls 134 (93%). Incontinent patients had the significantly lowest overall GIQLI.

[Electrophysiologic studies of fecal incontinence in the woman].

The cause of urinary or fecal incontinence may be a complex one. A central or peripheral neurogenic lesion is frequently found to be accounting for the problem. The diagnosis should be specified by neurologic examinations and adequate neurophysiologic workup prior to any conservative or surgical treatment. Of particular expressiveness are, neurological history, both neurography of the pudendal nerve and electromyography of the external anal sphincter. Additional measures such as recording of the motor or sensory potentials, determination of reflex latencies and an electromyogram of other stria

Laparoscopic appendicostomy for management of fecal incontinence.

Fecal incontinence from any cause is a socially unacceptable condition and a major detriment to educational and psychosocial development in young people with certain disabilities. Recently several procedures have been described that allow antegrade colonic enemas in the treatment of this problem. We report here an additional method of establishing this proximal colon access, namely laparoscopic appendicostomy. Although not applicable to all patients, this procedure has been successful in a select group with excellent results, and we recommend its consideration when open surgery is not otherwis

[The small and large intestine. Fecal incontinence].

Except for impaired resorption of vitamin D and calcium, the small bowel does not exhibit significant functional impairment during aging. Vascular problems in the small and large bowel are rare, but are of dramatic importance. Chronic constipation and diverticulosis are classical disorders of aging which should be treated by adding fiber to the normal diet. Surgery is only indicated in the case of complicated diverticulitis. Adenoma and colorectal carcinoma show an age-related incidence. Angiodysplasia, preferentially localized in the caecum, is the major cause of lower gastrointestinal tract
