Pattern of Audio Telehealth Among Medicare Beneficiaries With Alzheimer's Disease and Related Dementias.

OBJECTIVES: The Medicare telehealth expansion increased telehealth use, but the utilization patterns of audio-only telehealth remain unclear among those with Alzheimer's disease and related dementias (ADRDs). We aimed to describe 2020 to 2022 trends in audio-only telehealth utilization and provider specialty and to examine variations in utilization patterns by patient characteristics within the ADRD population. DESIGN: Cohort study. SETTING AND PARTICIPANTS: We identified 5,021,632 Medicare beneficiaries with a diagnosis of ADRD enrolled in Medicare fee-for-service plans between 2020 and 202

Unmet Need for Medical Care Among Fee-for-Service Medicare Beneficiaries with High and Low Need.

BACKGROUND: Unmet need for medical care is common among Medicare beneficiaries, but less is known whether unmet need differs between those with high and low levels of need. OBJECTIVE: To examine unmet need for medical care among fee-for-service (FFS) Medicare beneficiaries by level of care need. DESIGN, SETTING, AND PARTICIPANTS: We included 29,123 FFS Medicare beneficiaries from the 2010-2016 Medicare Current Beneficiary Survey. MAIN MEASURES: Our outcomes included three measures of unmet need for medical care. We also examined the reasons for not obtaining needed medical care. Our primary

Hospital Readmissions and Mortality Among Fee-for-Service Medicare Patients With Minor Stroke or Transient Ischemic Attack: Findings From the COMPASS Cluster-Randomized Pragmatic Trial.

Background Mortality and hospital readmission rates may reflect the quality of acute and postacute stroke care. Our aim was to investigate if, compared with usual care (UC), the COMPASS-TC (Comprehensive Post-Acute Stroke Services Transitional Care) intervention (INV) resulted in lower all-cause and stroke-specific readmissions and mortality among patients with minor stroke and transient ischemic attack discharged from 40 diverse North Carolina hospitals from 2016 to 2018. Methods and Results Using Medicare fee-for-service claims linked with COMPASS cluster-randomized trial data, we performed

What is Medicare Advantage and Why is it the Most Important Contemporary Policy Affecting Kidney Disease?

Medicare Advantage (MA) enrollment among patients receiving dialysis has surged following the passage of the 21st Century Cures Act, which lifted prior restrictions on enrollment. As MA becomes the plurality payer for dialysis, understanding its implications for patients, providers, and policymakers is critical. MA offers out-of-pocket spending caps and additional services not covered under fee-for-service (FFS) Medicare. Some plans also prioritize care coordination, which may improve patient outcomes. However, concerns remain regarding limited provider networks, prior authorization barriers,

Telemedicine Adoption and Low-Value Care Use and Spending Among Fee-for-Service Medicare Beneficiaries.

IMPORTANCE: Low-value care is a persistent problem with direct and cascading harms. Telemedicine is now commonly used and may reduce low-value testing by introducing barriers to completing tests at a given visit or expand opportunities for low-value testing by contributing to higher visit volumes. OBJECTIVE: To quantify the association between telemedicine adoption and low-value testing among fee-for-service Medicare beneficiaries. DESIGN, SETTING, AND PARTICIPANTS: In this cohort study using 100% fee-for-service Medicare claims data, US health systems were divided into quartiles based on 20

Potentially avoidable emergency department transfers among Medicare beneficiaries.

INTRODUCTION: Transfer of patients between hospitals is common, costly, and over 20 % are estimated to be avoidable, meaning patients do not receive specialized interventions once transferred. Older adults are more likely to be transferred and may be at increased risk for developing delirium or other complications due to transfer. We aimed to determine the frequency of potentially avoidable transfer (PAT) among older adults; identify conditions most likely to involve a PAT; and describe factors associated with PAT. METHODS: We performed a retrospective analysis of ED visits resulting in trans

A qualitative study of stakeholder views on the effects of provider payment on cooperation, quality of care and cost-containment in integrated stroke care.

BACKGROUND: Stroke services are a form of integrated care which have been introduced in many countries, including the Netherlands, to improve health outcomes and processes of care by connecting the acute, rehabilitative, and chronic phases of stroke care. Limited research exists on the effects of payment systems on the functioning of integrated care services from the perspectives of those involved in providing, planning and contracting the care. This qualitative study identified stakeholder views on i) challenges in integrated stroke care associated with fee-for-service systems; ii) other poss

U.S. health plan coverage of Neuromuscular Disease Therapies: An assessment of policy availability and restrictions.

BACKGROUND: Health plan policies managing neuromuscular disease (NMD) therapies may impose burdensome requirements on patients and providers. While prior research has explored payer restrictions for rare NMD treatments, limited evidence exists on policies specifically governing therapy initiation and reauthorization. OBJECTIVES: To examine initial and reauthorization coverage policies for therapies targeting five NMDs-Duchenne muscular dystrophy, amyotrophic lateral sclerosis, spinal muscular atrophy, generalized myasthenia gravis, and Lambert-Eaton myasthenic syndrome-across all U.S. state M

Variability in technical fee billing for cardiac CT across congenital cardiac centers.

BACKGROUND: Cardiac Computed Tomography (CCT) is increasingly used to provide 2D, 3D and 4D information in patients with congenital heart disease of all ages. Historically, negotiated rates for professional and technical fees associated with cardiac imaging were confidential, with variability in professional, technical and global charges, reimbursement and cost to patients for the same current procedural terminology (CPT) code at different institutions. Billing transparency is a key component of both the CARE act passed in 2020 and the Health Care PRICE Transparency Act 2.0 passed in 2021. Ins

Primary care payment models and avoidable hospitalizations in Ontario, Canada: A multivalued treatment effects analysis.

Improving access to primary care physicians' services may help reduce hospitalizations due to Ambulatory Care Sensitive Conditions (ACSCs). Ontario, Canada's most populous province, introduced blended payment models for primary care physicians in the early- to mid-2000s to increase access to primary care, preventive care, and better chronic disease management. We study the impact of payment models on avoidable hospitalizations due to two incentivized ACSCs (diabetes and congestive heart failure) and two non-incentivized ACSCs (angina and asthma). The data for our study came from health adminis

Growth In Participation Of Mental Health Counselors, Marriage And Family Therapists After Medicare Coverage Expansion.

Beginning in January 2024, Medicare allowed mental health counselors and marriage and family therapists to bill directly under fee-for-service Medicare for diagnosing and treating mental illnesses, marking a major expansion of the Medicare mental health workforce. Using 100 percent fee-for-service claims from the period 2023-24, we conducted the first national study of Medicare billing for these clinicians after the policy change to assess early billing trends, including the volume and mix of services, clinicians' and beneficiaries' characteristics, and geographic patterns. Family planning agencies: services, policies and funding.

Approximately 2,614 agencies are providing family planning services in at least 5,460 clinics throughout the nation, a slight increase over the 2,462 agencies and 5,174 clinics that were estimated to be providing such services in 1983. Health departments operate an estimated 52% of clinic sites; Planned Parenthood affiliates, 15%; hospital, 6%; and other agencies, 27%. Although many agencies also offer noncontraceptive services, the large majority provide family planning services in separate clinic sessions devoted to that purpose. According to a 1992-1993 survey of a random sample of family p

Mismatch Between the Distribution of Neuromuscular Specialists and Myasthenia Gravis Patients in the US Medicare Fee-for-Service Population.

INTRODUCTION/AIMS: There is a mismatch between clinical need and access to neurologists across the US. Myasthenia gravis (MG) incidence and prevalence are increasing, particularly in US patients older than 65 years, raising concern for access to neuromuscular specialists that represent only 4% of neurologists. We sought to compare the prevalence of MG patients over 65 years of age to the number of board-certified neuromuscular physicians (BCNMP) across the US. METHODS: Utilizing Medicare fee-for-service Parts A/B coverage non-HMO claims data, MG cases were ascertained using a validated algori
