Editorial Commentary: Hip Arthroscopy for Femoroacetabular Impingement in Systemic Inflammatory Disease: Pirates Gold in Uncharted Waters.

Hip pain in patients with systemic inflammatory diseases may arise from synovitis, cartilage degeneration or bony abnormalities like femoroacetabular impingement or acetabular dysplasia. With advances in early diagnosis and medical management of the autoimmune response, these often-young patients may benefit from correction of underlying bony abnormalities like femoroacetabular impingement. Short-term improvement in patients' pain and function can be achieved with hip arthroscopic treatment. However, the long-term efficacy of hip arthroscopy in systemic inflammatory disease needs further resea

Editorial Commentary: Chicken or the Egg? The Ligamentum Teres and Degenerative Hip Disease.

The ligamentum teres has traditionally been described as a vestigial remnant or redundant structure with little contribution to hip biomechanics or function. However, interest in the ligamentum teres has been renewed as evidence has emerged that it not only plays a role as a pain generator in the hip but also acts as a secondary stabilizer to supplement the work of the capsular ligaments. Furthermore, an association has been proposed between tearing of the ligamentum teres and the development of degenerative hip disease. However, the question remains, is it a cause-and-effect relationship?

Hip Osteoarthritis: Etiopathogenesis and Implications for Management.

Highly prevalent among the elderly, hip osteoarthritis (OA) carries a heavy burden of disease. Guidelines for the management of hip OA are often extrapolated from knee OA research, despite clear differences in the etiopathogenesis and response to treatments of OA at these sites. We propose that hip OA requires specific attention separate from other OA phenotypes. Our understanding of the etiopathogenesis of hip OA has seen significant advance over the last 15 years, since Ganz and colleagues proposed femoroacetabular impingement (FAI) as an important etiological factor. This narrative review s

Editorial Commentary: Excessive Femoral Anteversion Adversely Affects Clinical Outcomes Following Hip Arthroscopy for Femoroacetabular Impingement Syndrome.

Excessive femoral anteversion is related to inferior patient reports outcomes after hiparthroscopy. While hip femoral acetabular impingement is a more common diagnosis, awareness of the importance of femoral version for prognosis following hip arthroscopy is growing. An algorithm for the practical assessment of clinically significant femoral version is proposed.

Editorial Commentary: Small Hip Labrum, Big Long-Term Clinical Implications?

The acetabular labrum has long been a centerpiece in the treatment of femoroacetabular impingement syndrome and in our understanding of hip biomechanics. The labrum has a number of biomechanically distinct functions, including increasing the overall contact area of the hip joint, preventing synovial fluid egress from the central compartment during compressive loading, and increasing hip stability via negative pressure generation during destabilizing movements. Despite the delineation of these functions, our understanding of the labrum's influence on outcomes in hip preservation procedures is y

The Alpha Angle.

➢ The alpha angle was originally defined on magnetic resonance imaging (MRI) scans, using a plane, parallel to the axis of the femoral neck. However, much of the literature on the alpha angle has used radiographs or other imaging modalities to quantify the alpha angle.➢ The measurement of the alpha angle can be unreliable, particularly on radiographs and ultrasound.➢ If radiographs are used to measure the alpha angle, the circle of best-fit method should be used on multiple different views to capture various locations of the cam lesion, and "eyeballing" or estimating the alpha angle should be

Dynamic joint stiffness in individuals with femoroacetabular impingement syndrome pre- and post-hip arthroscopy.

BACKGROUND: Patients with hip-related pain often fail to return to their desired level of activity following hip arthroscopy. Lasting biomechanics alterations may be one potential explanation. Dynamic joint stiffness assesses the mechanistic controls of the lower limb during high impact movements, and thus, may provide valuable clinical targets to improving movement and optimizing return to activity after surgery. METHODS: Twenty-five participants (13 females) with hip-related pain underwent 3D motion capture during a drop jump task before surgery and six months post-operatively. Nineteen hea

Factors associated with cam deformity in Japanese local residents.

Femoroacetabular impingement has increasingly been recognized as a cause of primary hip osteoarthritis; however, its epidemiological indications remain unclear. We aimed to clarify the epidemiological indications and factors associated with cam deformity in a large-scale population-based cohort in Japan. Overall, 1480 participants (2960 hips) (491 men, 989 women; mean age, 65.3 years) analyzed in the third survey of the Research on Osteoarthritis/Osteoporosis Against Disability study were included. The α angle and spinopelvic parameters (lumbar lordosis, sacral slope, pelvic tilt, and pelvic i

Finite Element Modeling of Chondrolabral Lesions in Pincer-Type Femoroacetabular Impingement.

BACKGROUND: The prevalence of chondrolabral lesions due to femoroacetabular impingement (FAI) is 93% in cadaveric material. As a biomechanically determined factor, it can lead to hip destruction and early osteoarthritis in pincer-type impingement. The aim of this work was to study the biomechanism and stress-strain behavior of chondrolabral lesions in pincer-type impingement during daily activity-associated movements. MATERIAL AND METHODS: The SolidWorks package was used to build a 3D pelvis model with and without pincer-type impingement in the femoroacetabular (FA) region. Finite element ana

Endoscopic Ischial Tuberosity Osteophyte Resection for Treatment of Ischiofemoral Impingement: A Case Report.

Ischiofemoral impingement is a distinct pathologic finding with abnormal osseous contact between the ischium and the lesser trochanter of the femur. Lesser trochanter excision has been recommended for recalcitrant ischiofemoral impingement through an open or endoscopic approach; however, no study has included ischial tuberosity osteophyte resection and refixation of the hamstring tendon. We report an endoscopic procedure involving ischial tuberosity osteophyte resection with refixation of the partially detached hamstring insertion through a posterior approach in the prone position. Using this

Hip Pain: Imaging of Intra-articular and Extra-articular Causes.

Hip pain is a common and complex clinical entity. The causes of hip injuries in athletes are many and diverse, requiring efficient, accurate diagnosis for proper management. Imaging is an important step in the clinical evaluation of hip pain, and familiarity with multiple imaging modalities as well as characteristic imaging findings is a helpful tool for sports medicine clinicians. This article discusses imaging recommendations and gives imaging examples of common causes of intra-articular and extra-articular hip pain including femoroacetabular impingement, labral tears, cartilage defects, lig

Tonnis Angle and Acetabular Retroversion Measurements in Asymptomatic Hips Are Predictive of Future Hip Pain: A Retrospective, Prognostic Clinical Study.

BACKGROUND: This study evaluated the prevalence of radiographic abnormalities potentially indicative of femoroacetabular impingement on AP pelvic radiographs in asymptomatic adolescents and young adults and aimed to determine whether the abnormalities were predictive of future hip pain. METHODS: AP pelvis images from scoliosis radiographs were obtained from patients 12 to 25 years of age free of any clinical hip/lower extremity symptoms between January 2006 and September 2009. The following radiographic abnormalities were collected: lateral center-edge angle of Wiberg >40° or <25°, Tönnis ang

Editorial Commentary: Please Don't Call It a Mesenchymal Stem Cell.

One of the holy grails in orthopedics, and for medicine in general, is to have easy access to an immediately available and viable source of progenitor cells for use in tissue regeneration. The use of the term "mesenchymal stem cell" has been called into question, as it has historically represented a wide variety of tissue-specific cell types, only some of which can be categorized as true stem cells. More recent literature has better defined the characteristics of a stem cell, yet the landscape is still littered with unsubstantiated claims of cures for many human diseases, both within orthopaed

Editorial Commentary: Caveat Flexor-To Release or Not to Release the Iliopsoas, That Is the Question.

In nonarthritic patients with femoroacetabular impingement syndrome, borderline dysplasia, and symptomatic iliopsoas snapping, arthroscopic iliopsoas fractional lengthening carries a significant risk of postarthroscopic instability. The iliopsoas is a dynamic stabilizer of the anterior hip. Thus, although statistically significant and clinically important improvements in hip function have the potential to be achieved with iliopsoas fractional lengthening, surgeons must be supremely confident in their ability to perform a secure capsular plication, labral preservation (not debridement), compreh

Surgical treatment of femoroacetabular impingement after slipped capital femoral epiphysis.

PURPOSE OF REVIEW: Femoroacetabular impingement (FAI) post slipped capital femoral epiphysis (SCFE) may lead to degenerative changes on the hip. We have reviewed the current procedures in the literature, aiming to correct the SCFE to prevent FAI and the ones that treat FAI post SCFE. RECENT FINDINGS: The trends of managing moderated or severe SCFE is to correct the displacement by reduction and fixation with articular hematoma decompression in unstable hips and Dunn modified procedure even for unstable and stable situations. However, after FAI is settled, the options are osteochondroplasty by

Surgical indications for treatment for femoroacetabular impingement with surgical hip dislocation.

PURPOSE: There is a lack of detailed information about the indications of surgical treatment for femoroacetabular impingement (FAI), particularly using open surgical dislocation. The purpose of this review was to systematically review the reported indications for surgical dislocation of the hip for FAI. METHODS: Two databases (MEDLINE and EMBASE) were screened for clinical studies involving the treatment for FAI with surgical hip dislocation. We conducted a full-text review and the references for each included paper were hand-searched for other eligible studies. Papers published until Septemb

Ischiofemoral impingement: Clinical perspectives for enhancing diagnosis, and rehabilitation.

INTRODUCTION: Buttock pain often presents a diagnostic dilemma for healthcare professionals, with misdiagnosis resulting in poor treatment outcomes. Ischiofemoral impingement (IFI) - impingement of the quadratus femoris between the ischium and lesser trochanter - is one of the most overlooked causes of buttock pain. Ischiofemoral impingement is most common in females and results in pathology within the quadratus femoris muscle, with additional changes sometimes present within the iliopsoas and proximal hamstring tendons, and the adjacent sciatic nerve. Structural risk factors that narrow the i
