[Significance of subsequent hip unloading after surgical intertrochanteric osteotomy of idiopathic femur head necrosis].

PURPOSE: To judge the influence of consequent non-weight-bearing on the postoperative results, we reviewed 28 patients in whom we performed intertrochanteric osteotomies for idiopathic osteonecrosis of the femoral head during the years 1996 and 1997. Postoperatively 12 patients were supplied with a walking device for non-weight-bearing which was used for 1 year. 16 patients were mobilized by using 2 crutches for 15 kp partial weight-bearing until the osteotomy healed up and were then allowed to bear full weight. METHOD: For clinical evaluation of the early results after about 18 month the Mer

[Preliminary results of treatment with vascular pedicled iliac bone chip in femur head necrosis].

For revascularisation of femoral head with osteonecrosis and pseudarthrosis of the femoral neck 68 femoral head were treated with a pedicled bonegraft. Therefore were taken the medial bonegraft (A. circumflexa ilium profunda) and the lateral bonegraft, based on the deep branches of the superior gluteal vessels. On the basis of postexamination it is reported about the first 35 patients, who were treated with a medial autological pedicled bonegraft. Prae- and postoperative each of them was clinical, radiological and angiological examinated. Because of this operation in 78.8% the clinical picture

Dual-Bone Grafting Technique of Neck Reconstruction in Non-union Neck Femur Fracture With Avascular Necrosis of the Femur Head.

A 36-year-old male farmer with known sickle cell disease presented four months after a road traffic accident with right hip pain and difficulty walking. He had initially received conservative treatment with traction but discontinued it due to financial constraints. Clinical examination revealed signs of hip instability, a 3 cm true limb shortening, and a Harris Hip Score of 38. Imaging showed a Sandhu Stage 3 non-union of the femoral neck with >50% neck resorption and Ficat-Arlet Grade 2a avascular necrosis (AVN). Given the relatively favorable prognostic indicators, including a Kerboul angle

[Intertrochanteric flexion osteotomy and allo-arthroplasty in femur head necrosis. A comparative retrospective study].

Intertrochanteric flexion osteotomy and total hip arthroplasty are the most frequent operative treatments of advanced osteonecrosis of the femoral head in adults. In a retrospective study the postoperative results of intertrochanteric osteotomy and total hip arthroplasty were determined in 68 cases. The evaluation was based on the modified Harris-Hip-Score, the clinical examination and the radiological classification of the ARCO. The patients after osteotomy showed significantly worse subjective results. After osteotomy an increase of the stage of osteonecrosis was found in 73% of the patients

[Management of early avascular femur head necrosis by flexion osteotomy. 2-year follow up].

Authors report on the results of treatment in 19 patients, operated for avascular necrosis of the femoral head. The first operation was performed in October, 1989. The average follow-up was 28 (24-42) months, the minimal follow-up 2 years. The state of one patient grew worse, one was unchanged, the rest improved. Operation in early cases is suggested if less than 50 per cent of the femoral head is attainted.

[Tendency of acetabular development of conservatively treated hip dislocation with femur head necrosis (1)].

The late results of 101 cases of avascular necrosis in congenital dislocation of the hip are reported, which occurred after closed reduction between 1954 and 1960. A good development of hips was observed in 24 cases (23,8%). A severe deformity took place in 33 hips (32.7%), which mostly had total femoral head necrosis. In the groups with partial necrosis a good development was occurred only in the half of all cases. Early treatment led to better results. Besides the necrosis the grade of luxation plays an important role for the development of the hips.

[Aseptic necrosis of the head of the femur in patients with major rheumatic diseases of inflammatory and dystrophic genesis].

A total of 2392 patients treated at the Unit of Rheumatology, Tula Regional Hospital, in 2002 and 2003 were analyzed. Among them, there were 71 patients who were found to have aseptic necrosis of the head of the femur. A statistical analysis of clinical, laboratory, and X-ray data revealed a number of correlations. It was established that the risk factors of aseptic necrosis of the femoral heads were anemia concurrent with the high laboratory values in rheumatoid arthritis and elevated lipid levels and blood coagulative system activation in primary coxarthrosis.

Clinical acupuncture therapy for femur head necrosis: A protocol for systematic review and meta analysis.

BACKGROUND: Femur Head Necrosis (FHN) is a common clinical joint orthopedic-related disease, and its incidence is increasing year by year. Symptoms include dull pain and dull pain in the affected hip joint or its surrounding joints. More severely, it can lead to limited joint movement and inability to walk autonomously. Surgical treatment has many sequelae. The high cost makes it unaffordable for patients, and the side effects of drug treatment are unknown. A large number of clinical studies have shown that acupuncture is effective in treating femoral head necrosis. Therefore, this systematic

Operative treatment of avascular necrosis of the femoral head after proximal femur fractures in adolescents.

PURPOSE: Our aim was to assess operative treatment for post-traumatic avascular necrosis of the femoral head (ANFH) in adolescents. METHODS: Eleven patients with an average age of 17 (range 14-26) years were operated up on for ANFH after proximal femoral fractures. The average interval between injury and reconstructive surgery was four (range two to eight) years. The average follow-up of the entire cohort was 89 (range 48-132) months. Five patients with total ANFH were treated by total hip replacement (THR). Six patients with partial ANFH were treated with valgus intertrochanteric osteotomy (

Intertrochanteric osteotomy for avascular necrosis of the head of the femur. Survival probability of two different methods.

We have compared different types of intertrochanteric osteotomy for avascular necrosis of the hip and evaluated their performance in the light of improving outcome after total hip arthroplasty (THA). During a period of 14 years we performed 63 flexion osteotomies (partly combined with varus or valgus displacement), 29 rotational osteotomies, 13 varus osteotomies, eight medialising osteotomies and two extension osteotomies. The mean period of follow-up for all 115 operations was 7.3 years (maximum 24.6). At follow-up, 27 of 29 patients with a rotational osteotomy had already undergone a THA, co

[Aseptic femur head necrosis and therapeutic possibilities by using a bone graft with a vascular pedicle].

The review of the occurrence, etiopathology, classification and diagnosis of the aseptic necrosis of the femoral head is followed by the description of the method of transplantation of the vascular pedicled bone graft. During 5 years implantation of 12 vascular pedicled bone grafts in 10 young patients with severe and of major extension necrosis of the femoral head was carried out. The early result was in every case the quick and significant decrease of pains and that the necrosis did not progress. In the early stage the severe consequence of the necrosis the collapse can be prevented with cor

[Avascular femur head necrosis as severe complication after femoral intramedullary nailing in children and adolescents].

Nailing of femoral fractures before closure of the growth plates may lead to avascular necrosis of the femoral head in 3-4% of cases. In addition to the 14 cases described in the literature we present 3 more. Analysis of these cases reveals a common pathogenesis. The nails were all inserted anterograde and were designed for the adult femur. The problem appears to be related to the large diameter of the nail and its entry point in the relatively small femoral neck basis, close to the vessels supplying the femoral head. The role of the open physis remains unclear. Even though the complication of

[Developmental tendency of the femur head following femoral head necrosis due to conservative treatment of a dislocated hip (2nd report)].

The development of the proximal femur was studied in a follow up of 64 patients--on average 24.4 years of age--with congenital hip dislocation and 101 subsequent aseptic necroses of the femoral head. For measurement of the angle of antetorsion and the femoral neck-shaft angle, biplanar radiographs were taken. A coxa valga with a true femoral neck-shaft of 137 to 164 degrees was present in one third of the cases with partial necrosis and in the group without necrosis. An increasing degree of necrosis led to coxa vara. Damage of the capital epiphysis caused development of coxa vara (average of 1

A 28 year old male patient, known case of pemphigus vulgaris was on dexamethasone pulse therapy. Total 7 pulses were given after that he developed avascular necrosis of head of femur on both sides, which was confirmed by digital X- ray and MRI. Avascular necrosis is a disabling and progressive condition in young patients gradually leads to femoral head collapse and eventual total hip arthroplasty. As per WHO-UMC causality assessment criteria, the association between reaction and drug was possible, Naranjo's score was 7. According to Modified Schumock and Thornton's criteria, this reaction was

[Surgical treatment of the sequelae of ischemic postdysplasia femur head necrosis in children. II. Surgical technique].

The author presents its concept concerning the possibilities of the surgical treatment of the sequelae of the postdysplastic necrosis of the femoral head originating from the treatment of congenital dislocation or subluxation of the hip joint. The classification is based on the division according to Bucholz and Ogden into 4 types. Indicated for the surgical treatment are hips with the injury of the type II and III. In Grade II type the joint congruence is not affected and the method of operation is the redirection pelvic osteotomy. In children up to the age of 10 the author applies Salter oste
