We report a rare case of proximal fibular fatigue fracture developing 14 years after total knee arthroplasty in a known case of rheumatoid arthritis. A valgus deformity of the knee can put abnormal stress on the upper fibula leading to its failure. We believe that, as the fibula acts as an important lateral strut, its disruption due to a fracture led to rapid progress of the valgus deformity of the knee in this patient.

The increase in proportional loading of the fibula with progression of hindfoot deformity would lead to high fibular loads during rapid walking, resulting in insufficiency fractures. We report an unusual mechanism of such fracture in a textile worker resulting from valgus alignment from a stage III flatfoot deformity. The lateral approach to the distal peroneal artery has been used by vascular surgeons for 25 years. No complications specifically related to this approach have previously been reported. We reviewed 18 cases of peroneal bypass for limb salvage using the lateral approach with fibula resection and found that two of these cases had ipsilateral tibia fractures within 1 year of the bypass. Fracture-dislocation of the ankle with entrapment of the fibula behind the tibia is a rare condition. Prompt recognition and treatment are necessary to prevent permanent disability. In order to prevent permanent disability, a proper diagnosis of the fixed posterior fracture dislocation of the fibula with fracture dislocation of the ankle joint must be made. Full-length (knee to ankle) radiographs are necessary to make this diagnosis. Although a closed reduction has been described as successful, we feel that an open reduction is usually necessary. Freeing of the trapped fibula from behind the tibia results in a satisfactory reduction of this fracture dislocation. The reduction and fixation of the malleolar fragment is then simple, and an excellent result can be expected.

A 26-year-old woman was injured in a motor vehicle accident and sustained a bimalleolar fracture of the right ankle. Radiographs revealed a shearing fracture of the medial malleolus and a gap in the distal end of the right fibula that resembled epiphysiolysis in children. Fracture of the distal end of the fibula through a persistent physis was suspected. Histological examination of material obtained from the fracture site during surgery revealed remnants of hyaline cartilage. We believe that the fracture occurred at a persistent physis of the distal end of the fibula.

The author reports a case of fracture-dislocation of the ankle associated with a posterior dislocation of the proximal part of the fibula behind the tibia: Bosworth's fracture. Hugier first described this "new dislocation of the ankle" in 1848, and Bosworth classified it in 1947. Because the correct diagnosis was not done at first in our patient, our attempt at closed reduction failed. Open reduction was performed, followed by internal fixation of the fibula. A good functional result was obtained at 12 weeks.

The authors present a case of fracture dislocation of the ankle joint with the proximal part of the fibula entrapped behind the tibia ; Bosworth's fracture. This fracture was described for the first time in 1848 by Hugier, and classified by Bosworth in 1947. In our case, diagnosis was not done in a first time, and closed reduction failed. This fracture was operated on and open reduction was performed. At the present time, the functional result is good.
