Hallux duplication, postaxial polydactyly, absence of the corpus callosum, severe mental retardation, and additional anomalies in two unrelated patients: a new syndrome.

Two unrelated patients, a 4-year-old boy and a 2 1/2-year-old girl, presented with a similar pattern of abnormalities. Both had severe mental retardation, macrocephaly, absence of the corpus callosum, unusual facial appearance, duplication of hallucal phalanges, postaxial hexadactyly of finger phalanges, and 2/3-syndactyly of toes. The boy also had postaxial hexadactyly of toe phalanges, inguinal hernias and umbilical hernia, and growth retardation. We suspect a common cause of this apparently "new" syndrome, most likely a gene mutation.

Masquelet Technique for Phalangeal Reconstruction and Osteomyelitis.

Osteomyelitis of the finger presents a challenging problem. Although there are multiple treatments described in the literature, the infection often results in eventual amputation. The authors present a case of an open fracture of the index finger complicated by nonunion, infection, and osteomyelitis. This was successfully treated by the placement of an antibiotic cement spacer and subsequent reconstruction with iliac crest bone graft, after the 2-stage Masquelet technique, using intravenous and oral antibiotics. They show that this technique, although infrequently described in the hand and fin

[Polyphalangism--a rare anomaly].

Polyphalangism/hyperphalangism is a very rare autosomal dominant anomaly. Polyphalangism is characterized by an extra phalanx between two other phalanges. The abnormality is distinguished from polydactyly in that there are more phalanges in a finger but no more than five fingers to a hand. Polyphalangism can be part of various syndromes, but isolated polyphalangism is very rarely seen. We report the first case of polyphalangism/hyperphalangism in the Danish literature.

Autosomal dominant onychodystrophy and anonychia with type B brachydactyly and ectrodactyly.

A family is reported with nail dysplasia and/or absent nails, long and broad finger-like thumbs, camptodactyly and absent fingers. Radiological studies revealed hypoplasia of metacarpals, metatarsals and distal phalanges. Two affected individuals have absent metacarpals and phalanges. The clinical and radiological features may constitute a distinct syndrome of autosomal dominant onychodystrophy and anonychia with Julia Bell brachydactyly type B.

Bone density and the carpal tunnel syndrome.

Bone density was studied in ten patients before and after release of an idiopathic carpal tunnel syndrome. A corticomedullary ratio was measured at the mid-point of the metacarpals and proximal phalanges of both hands. There was a significant increase in the mean bone density over the period of observation in the thumb and middle finger metacarpals and in the index, middle, and ring finger proximal phalanges of the operated hand. There was no significant increase in the density of either metacarpal or proximal phalanx on the non-operated side. Release of the carpal tunnel syndrome provides not

Rare phenotype of juxtaepiphyseal osteochondroma of the proximal phalanx and its management: A case report.

BACKGROUND: Osteochondroma of the hand and phalanges is rare. Although rare, solitary phalangeal osteochondromas can result in pain, cosmetic deformity, compression of nearby neurovascular structures and tendons, or functional limitation of adjacent joints that warrant surgical correction when symptomatic. At times, involvement of the growth plate by juxtaepiphyseal exostosis can cause angulation deformity, necessitating corrective osteotomy. CASE SUMMARY: We reported a case of a 3-year-old female with a right-sided painless deformity of the index finger along with swelling over the base of t

Radiographic changes in the hands following childhood frostbite injury.

Three young adult patients who had sustained severe frostbite of the hands as children were recently evaluated for progressive deformity and joint pain in the fingers. Characteristic radiographic abnormalities including dwarfing of the middle and distal phalanges, irregular and malapposed articular surfaces, malalignments at the proximal and distal interphalangeal joints, and evidence of degenerative arthritis in the interphalangeal joints were observed. The metacarpal phalangeal joints and wrists were spared in all patients, and fingers which had been protected from the initial cold injury we

Minimal Invasive Corrective Osteotomy with the Ilizarov Mini-Fixator for Malunited Fractures of Proximal Phalanges in Adolescence: Report of Three Cases and a Review of the Literature.

Posttraumatic malunion with combination of angular, rotational, and shortening deformity of the proximal phalanx may cause scissoring of a finger and impairment of hand function. Cosmetic disfigurement and severe dysfunction of fingers require surgical correction, most often via open corrective osteotomies and rigid fixation with a plate or screws. However, such an approach requires a longer incision, inevitably results in a scar, and has a higher potential for extensor tendon adhesion. Also, abruption of the periosteum and plating of the phalange requires longer bone healing time. Thus, we de

Sclerosteosis (craniotubular hyperostosis-syndactyly) with complex hyperphalangy of the index finger.

We report a 4-year-old boy with sclerosteosis associated with severe digital dysostosis. The initial medical consultation was prompted by bilateral, asymmetrical syndactyly of the index and middle fingers. The left index finger had complicated phalangeal anomalies: hyperphalangy (supernumerary phalanx distal to the middle phalanx) and hypoplasia with bracket epiphyses of the proximal and middle phalanges. Development of facial nerve palsy, hearing impairment and generalized osteosclerosis had occurred between 3 years and 4 years of age, with the subsequent identification of a homozygous SOST m

Extensor Mechanism Damage With Proximal Phalanx Antegrade Intramedullary Screws: A Cadaveric Study.

BACKGROUND: Antegrade intramedullary screw instrumentation for treatment of proximal phalangeal fractures has become increasingly common. To date, there has been little investigation describing and quantifying extensor mechanism damage incurred with this treatment option. METHODS: Thirty-two fresh cadaveric proximal phalanges (thumbs excluded) underwent percutaneous antegrade screw insertion. Radial-based and ulnar-based intramedullary screws were inserted per phalange, and the sagittal band and/or extensor tendon defect were evaluated. The mean sagittal band violation was measured and calcul

Development of a joint space width measurement method based on radiographic hand images.

This study presents a novel algorithm to measure joint space widths (JSWs) in patients with rheumatoid arthritis (RA) using radiographic hand images. Radiographic images were first preprocessed, and then phalangeal regions corresponding to the bone structures of each finger were extracted using step-wedge functions. Phalangeal branch paths were also extracted. Each of the five extracted phalangeal branch paths matched the bone structures of each finger exactly and ran through the center of each finger. The algorithm automatically detected 14 joints, which were identified as sharp changes in gr

Brachydactyly type A3 is caused by a novel 13 bp HOXD13 frameshift deletion in a Chinese family.

Brachydactyly type A (BDA) is defined as short middle phalanges of the affected digits and is subdivided into four types (BDA1-4). To date, the molecular cause is unknown. However, there is some evidence that pathogenic variants of HOXD13 could be associated with BDA3 and BDA4. Here, we report a Chinese autosomal dominant BDA3 pedigree with a novel HOXD13 mutation. The affected individuals presented with an obviously shorter fifth middle phalanx. The radial side of the middle phalanx was shorter than the ulnar side, and the terminal phalanx of the fifth finger inclined radially and formed clas

Clinical outcomes of unstable metacarpal and phalangeal fractures treated with a locking plate system: a prospective study.

We prospectively assessed clinical and radiological outcomes of locking plate fixation in treating unstable fractures in 11 metacarpals, 15 proximal phalanges, and eight middle phalanges in 34 consecutive patients from October 2011 to December 2016. Median length of follow-up was 14 months (range 12-24). The motion of finger joints, bony union, and complication rates were recorded. The median postoperative range of motion of the two interphalangeal joints and the metacarpophalangeal joint was 82% of the contralateral hands. Fractures in the three locations had significantly different recovery

[Lesions of peripheral structures of cadaver fingers caused by Kirschner wires].

Fractures of the hand skeleton are common injuries. These are or are associated with phalangeal fractures. Kirschner wires can be used for osteosynthesis, but can cause lesions of peripheral finger structures. The aim of our study was to record the incidence of lesions in peripheral structures of the fingers caused by retrograde crossed inserted Kirschner wires of 1.0, 1.25, 1.4 and 1.6 mm diameter into the middle and proximal phalanges of the long fingers and the proximal phalanx of the thumb of 120 fresh cadaver hands. We found an increase of insertion attempt frequency and an increasing num
