BACKGROUND: Foot orthoses (FOs) have been widely prescribed to alter various lower limb disorders. FOs' geometrical design and material properties have been shown to influence their impact on foot biomechanics. New technologies such as 3D printing provide the potential to produce custom shapes and add functionalities to FOs by adding extra-components.
RESEARCH QUESTION: The purpose of this study was to determine the effect of 3D printed FOs stiffness and newly design postings on foot kinematics and plantar pressures in healthy people.
METHODS: Two pairs of ¾ length prefabricated 3D printed F

Stiffness-customized passive-dynamic ankle-foot orthoses (PD-AFOs) have been shown to reduce the mechanical cost of transport (COT) of individuals post-stroke. However, the mechanisms underlying this reduced COT are unknown. Therefore, this study aimed to identify the factors driving COT reduction with PD-AFO use for individuals post-stroke. Results showed that changes in limb work were strongly correlated to changes in COT with the PD-AFO compared to No AFO in the paretic (tau = 0.637, p < 0.001) and non-paretic (tau = 0.621, p < 0.001) limbs. There was also a strong correlation between chang

Ponseti treatment of congenital clubfoot utilizes foot abduction bracing to prevent recurrence. In unilateral clubfoot, families have expressed concern about inducing deformity in the uninvolved leg. This study evaluated rotational profiles of uninvolved legs in children with unilateral clubfoot. We retrospectively reviewed 78 children (mean age: 9.5 ± 3.4 years) with unilateral idiopathic clubfoot treated by Ponseti method at a single institution from 2008 to 2025. Motion capture kinematics evaluated hip, tibial, ankle rotation, and foot progression angle (FPA). Physical exam evaluated bimall

BACKGROUND: The effects of foot orthoses on lower limb biomechanics during walking have been studied extensively. However, the lack of knowledge regarding the effects of various foot orthoses models for the same population complicates model selection in clinical practice and research. Additionally, there is a critical need to enhance our ability to predict the outcomes of foot orthoses using clinical tests, such as the supination resistance test.
RESEARCH QUESTION: What are the effects of two commonly prescribed types of FO (thin-flexible and medially wedged) on lower limb biomechanics during

Stiffness of ankle-foot orthoses (AFOs) has been assessed using many methods; however, results are unstandardized due to varied testing conditions such as range of motion and speed. Therefore, the aim of this scoping review was to provide a state-of-the-space review of the studies that investigate AFO ankle stiffness in the sagittal plane and then provide insight into the details of the developed devices and protocols. Six electronic databases (MEDLINE, SCOPUS, Web of Science, CINAHL, EMBASE, and IEEE Xplore) were searched for studies from when indexing began to April 2024. Articles of any des

Ankle-foot orthoses (AFOs) are of important in the management of gait deformities in most neurological conditions through stabilising and supporting the ankle and foot. Despite its importance, there is a lack of knowledge about how some design parameters, particularly trimline geometry, affect AFO stiffness. This study employs a parametric finite element (FE) model to quantify the impact of trimline design on rigidity to improve standardisation of AFO prescription manufacture, and quality control. A parametric model was developed to systematically modify trimline placement and analyse its effe

Although foot orthoses are often prescribed to patients with haemophilia (PWH) and ankle arthropathy, the efficacy and biomechanical effects of such devices are not fully understood. We experimentally investigated the effects of orthopedic insoles (OI) and shoes (OS) in PWH presenting ankle arthropathy, with specific attention being paid to pain, spatiotemporal parameters, kinematics and kinetics of lower limb joints, as well mechanical and energetic variables. Using three-dimensional gait analysis (3DGA), synchronous kinematics, kinetics, spatiotemporal, mechanics, and metabolic gait paramete

This study examined the effect of conventional plastic bilateral ankle-foot orthoses (AFOs) on the energy expenditure of ambulation in spastic diplegic cerebral palsy patients. Eighteen subjects (aged 8.3 +/- 2.83 years) ambulated five minutes at freely chosen velocities with and without their AFOs. Heart rate (beats per minute) and distance (meters) ambulated were recorded for each minute. A physiologic cost index was calculated by determining the ratio of walking heart rate to velocity [(HRamb-HRrest) divided by meters per minute]. The mean physiologic cost indexes for the last three minutes

PURPOSE: Despite ample evidence supporting ankle foot orthoses (AFOs) for enhancing ambulation in those with neuromuscular impairment, a prevalent belief among rehabilitation professionals is that AFO use may lead to disuse and reduced muscle activity of the lower leg. To determine the effects of AFO intervention on electromyography (EMG) activity during walking in individuals with neuromuscular impairment.
MATERIALS AND METHODS: Five databases were searched for studies that met the predefined inclusion criteria and were published any time through April 2024. AFO design characteristics, muscl

The purpose of this study was to create and validate a standardized framework for the evaluation of the ankle stiffness of two designs of 3D printed ankle foot orthoses (AFOs). The creation of four finite element (FE) models allowed patient-specific quantification of the stiffness and stress distribution over their specific range of motion during the second rocker of the gait. Validation was performed by comparing the model outputs with the results obtained from a dedicated experimental setup, which showed an overall good agreement with a maximum relative error of 10.38% in plantarflexion and

BACKGROUND: Ankle-foot orthoses (AFO) are commonly prescribed to provide functional assistance for patients with lower limb injuries or weakness. Their passive mechanical elements can provide some energy return to improve walking ability, but cannot restore plantar flexor push-off. Powered AFOs provide an assistive torque about the ankle to address the limitations of passive devices, but current designs have yet to be implemented on a large scale clinically.
PURPOSE: To compare passive AFOs to a new untethered, powered AFO design in a clinical population with lower limb reconstruction.

Use of orthotics and calf stretching may alleviate symptoms in runners with medial tibial stress syndrome (MTSS). The objective of this study was to determine which patients with MTSS have a positive response to off-the-shelf foot orthoses and calf stretching based on selected clinical tests to establish a clinical prediction rule. This prospective cohort/predictive validity study enrolled 23 women and men aged 22 to 44 years with symptoms of MTSS. Interventions included off-the-shelf basic foot orthotics and calf stretching. Fifteen of the 23 runners had a 50% reduction of pain in 3 weeks of
