A 39-year-old male was admitted because of slowly progressive disturbance of consciousness, diplopia, and ataxia after laparotomy. Ventriculoperitoneal shunting and removal of an arteriovenous malformation had been performed previously. Neurological examination on admission revealed drowsiness, rotatory nystagmus, Parinaud's sign, and truncal ataxia. Computed tomography scan revealed extraordinary dilatation of the fourth ventricle compared with other dilated ventricles, and old low-density areas in the cerebellar hemispheres. After an external ventricular drainage (EVD) was inserted, all the

Congential hemifacial spasm is a rare condition that is characterized by the occurrence of paroxysmal hemifacial contractions in neonates. We review the clinical, neurophysiological, neuroimaging, and histopathological findings, as well as the differential diagnosis, therapeutic approach, and outcome of all the described cases. Moreover, we report two new cases including the ictal video-electroencephalography recordings. Hemifacial spasm starts early in life, and is characterized by unilateral, involuntary, irregular tonic or clonic contractions of muscles innervated by the seventh cranial ner

A dilated fourth ventricle due to outlet obstruction is a clinical-radiologic entity with symptoms similar to those of a posterior fossa space-occupying lesion. Indeed, blockage of the foramina of Luschka and Magendie and of the aqueduct results in a "trapped" fourth ventricle. Continued cerebrospinal fluid (CSF) production within the fourth ventricle leads to cystic dilatation of the fourth ventricle. We could not, until now, understand the phenomenon, mainly on exploring endoscopically the permeability of the whole aqueduct. In adults, we call this condition the functional trapped fourth ven

Two cases of isolated fourth ventricle are reported, the first due to cerebellar haemorrhage, and the second due to congenital hydrocephalus with multiple shunt revisions and Dandy-Walker cyst. In our opinion, there are two basic treatment for isolated forth ventricle. The direct approach to the fourth ventricle is indicated when there is presence of an intraventricular cyst. The fourth ventricular shunting, independent of the supratentorial shunt, is the best treatments for patients with an isolated fourth ventricle without the presence of a cyst.

The isolation and enlargement of the fourth ventricle after a ventriculoperitoneal (V-P) shunt was classified as "isolated fourth ventricle (IFV)". The term, "disproportionately large communicating fourth ventricle (DFV)" was first introduced by Scotti et al as being an enlarged fourth ventricle communicating with the third ventricle. The authors present a case of DFV after the resection of an astrocytoma. Upon recurrence of the tumor a second resection was carried out 5 years later. It was found that IFV had evolved because a cyst in the right temporal lobe was obstructing the aqueduct. After

BACKGROUND: Management of the disproportionately large communicating fourth ventricle is still problematic.
CASE PRESENTATION: Two cases of disproportionately large communicating fourth ventricle were treated successfully. One was a case of a 51-year-old Han Chinese woman with a complaint of headache and dizziness of 1 year's duration. Magnetic resonance imaging (MRI) demonstrated hydrocephalus with a disproportionately large fourth ventricle. She underwent a ventriculo-peritoneal shunt of the right lateral ventricle. Her symptoms were relieved totally. Five years later, magnetic resonance im

BACKGROUND: Ectopic craniopharyngioma is an uncommon entity. We report the first case of ectopic craniopharyngioma confined purely within the fourth ventricle, exophytic from pons.
CASE DESCRIPTION: A 12-year-old girl presented with 2 months history of headache and recent evidence of raised intracranial pressure and left abducent palsy. Magnetic resonance imaging suggested intra-fourth ventricular mass with hydrocephalus. The patient underwent midline posterior fossa craniotomy and tumor excision, with prompt relief of gaze palsy. Histologic examination proved the lesion to be craniopharyngio

OBJECT: Advances in endoscopic technology have afforded the neurosurgeon new avenues in the treatment of hydrocephalus, rendering many patients independent of shunts, thus averting shunt complications and failure. Cerebral aqueductoplasty has gained popularity as an effective treatment for membranous and short-segment stenoses of the sylvian aqueduct. Traditionally, this procedure has been performed via a coronal approach, passing through the lateral ventricle, foramen of Monro, and third ventricle into the aqueduct. The authors report on the success of a novel technique for this operation, in

We report the case of a 4-year-old girl with a fourth ventricle tumor diagnosed as a cerebellar liponeurocytoma which recurred, showing ependymal differentiation, 14 months after surgery. Magnetic resonance imaging at initial presentation revealed a large mass in the fourth ventricle, and histology showed a neoplasm characterized by a combination of well-differentiated neurocytes and cells resembling adipocytes. The tumor recurrence was histologically identical to the original tumor in some regions, but with fewer adipose-like cells, while other areas presented an endocrine architecture with o

Thirty four patients with tumors of the cerebellum and the fourth ventricle were examined. All the patients underwent craniography and computed tomography (CT), 28 had magnetic resonance imaging (MRI). According to the histological pattern, the tumors were divided as follows: astrocytomas in 14 patients, hemangioblastomas in 8, ependymomas of the fourth ventricle in 5, medulloblastomas in 3, choriopapillomas in 2, two patients were found to have metastases. MRI was shown to be the most informative technique in the diagnosis of bulky abnormal formations of the cerebellum and the fourth ventricl

A case of meningioma with inflammatory reaction within the fourth ventricle in a 37 year-old man is described. The differential diagnosis between inflammatory meningioma, plasma cell granuloma and a possible collision of a meningioma with a plasmacytoma is discussed. The immunohistochemical examination of plasma cell population is emphasized. This is the first case reported of inflammatory meningioma located in the fourth ventricle.

A case of choroidal epithelial cyst in the fourth ventricle is described. The cyst occupied the fourth ventricle almost completely, and produced an intermittent hemiparesis and hemisensory disturbance. After removal of the cyst wall, the patient recovered completely. On light microscopy, a single epithelial layer with a basement membrane lining the cyst wall was observed. An electron microscopic study was also made.

Mucus-secreting adenomas of the choroid plexus are very rare. We report the case of a 72-year-old woman who was first seen because of difficulty in walking. Her CT scan showed a lesion compatible with the appearance of a cystic mass in the fourth ventricle. A posterior fossa exploration was performed and an acinar choroid plexus adenoma of the fourth ventricle was removed.

Symptomatic subependymomas of the fourth ventricle are rare and usually not included in the preoperative differential diagnosis of tumors in this region. The case of a 63-year-old man with fourth ventricle subependymoma is described. For several years he had suffered with nausea and vomiting, and now presented signs of direct involvement of the posterior fossa. He was investigated preoperatively with computerized tomography, but the ultimate diagnosis was not suspected at that time. The tumor was diagnosed as an subependymoma at operation and was totally excised. This tumor type can easily be

A case is reported where it was possible, by surgical intervention, to show a cysticercus in the fourth ventricle to be the cause of a serious increase in intracranial pressure. The difficulties of preoperative contrast diagnosis in the case of space occupying lesions in the rhomboid fossa are described with reference to this particular case. Also discussed is the increasing importance of cysticercosis in our latitudes.
