Totally absorbable internal fracture fixation devices were introduced clinically in the treatment of fractures and osteotomies of the extremities at our department in 1984. A total of 2,500 patients were managed using bone or ligament fixation devices made of self-reinforced (matrix and fibers of same polymer) absorbable alpha-hydroxy polyesters between November 5, 1984, and January 12, 1994. The devices used included cylindrical rods or pins, screws, tacks, plugs, and wires. The most common indication for the use of absorbable implants was displaced malleolar fracture of the ankle. We report a series of 20 children with 24 fractures that were treated using the AO tubular external fixator in a paediatric orthopaedic unit. There were no major complications related to fixator use. There were two cases of superficial pin track sepsis and one case of non-union. There were no cases of refracture after fixator removal or of leg length inequality.

Although external fixation is not indicated in every type of facial fracture, its use alone or with intermaxillary or internal fixation may be of benefit with problems resulting from avulsed segments, infection, inadequate previous reduction, delayed reduction, severe displacement and comminution, non-union, cerebrospinal fluid leak or where a combination of these factors exists. The judicious application of extraskeletal fixation can save valuable anesthetic time as well as assure accesibility of the pharynx and maintenance of an airway and routes for nutrition when these might otherwise be c

The objective of surgery for acetabular fractures is to achieve precise reduction to restore joint congruence, fix internal bone fragments, avoid displacement of the fracture and allow rapid rehabilitation.Open reduction and internal fixation is the benchmark method for displaced acetabular fractures, but open reductions can increase morbidity, causing neurovascular injury, blood loss, heterotopic bone formation, infection and poor wound healing. An anatomical reduction with a gap of 2 mm or less is a predictor of good joint function and reduced risk of post-traumatic osteoarthritis. The percuta

Recent advances in techniques and implants have led to renewed interest in percutaneous screw fixation of acute scaphoid fractures. The closed (cast) treatment of acute scaphoid fractures generally has good outcome, with bony union resulting; however, closed treatment can result in delayed union, nonunion, malunion, cast- induced joint stiffness, and lost time from employment and avocations. Acute percutaneous fixation of scaphoid fracture has been proposed as a means to minimize some of the complications of closed (cast) treatment. Percutaneous treatment of both nondisplaced and displaced sca

The main indications for internal fixation of intracapsular fractures are undisplaced and minimally displaced fractures and displaced intracapsular fractures in those aged less than about 70 years. For displaced fractures, closed reduction is to be preferred to open reduction. Numerous different implants may have to be used; with current practice favouring two or three parallel cannulated cancellous screws. These may be inserted either percutaneously or with minimal surgical exposure. Attention to surgical details of fracture reduction and implant positioning will minimise the risk of fracture

The scaphoid proximal pole and waist fractures presented here were treated by a novel dorsal percutaneous technique with arthroscopic assistance. All fractures healed, with good final functional results and no complications. The advantages of the dorsal percutaneous approach to scaphoid fixation are: (1) the proximal-to-distal placement of the guide pin and screw allow for more precise placement along the central axis of the scaphoid, which decreases healing time and reduces risk of screw thread exposure. (2) The dorsal approach avoids injuring the vulnerable volar ligament anatomy. And (3) th

A case of open carpal dislocation fracture not previously described is reported on. Reduction and stabilization of the dislocation were successfully performed using external fixation without additional Kirschner wires. Despite extensive trauma, hand function is now excellent, and there are no signs of residual carpal instability.

PURPOSE: Although mandibular condylar fractures are very common, the treatment remains controversial. Many techniques of reduction and many devices have been suggested. The purpose of this study was to evaluate the advantages and drawbacks of semirigid fixation compared with rigid fixation using an external fixation system.
PATIENTS AND METHODS: Between 1990 and 2005, 137 patients (83 males, 54 females; median age, 24.2 years; 116 with monocondylar fracture and 21 with bicondylar fracture) were treated with an external fixation system (Mand-X-Fix, Leibinger, Germany). In these cases, the dist

BACKGROUND: We investigated associations of cannabis and tobacco use with complications after open reduction and internal fixation (ORIF) of wrist fractures.
METHODS: We retrospectively analyzed PearlDiver data from 337 538 patients who underwent ORIF for wrist fractures from 2010 to 2022. We identified 5587 patients with a history of cannabis use disorder ("cannabis group"); 12 806 patients with a history of tobacco use and no history of cannabis use disorder ("tobacco group"), and 310 108 patients with no history of cannabis use disorder or tobacco use ("control group"). We excluded patient

In a prospective investigation in 1984 and 1985, all Colles' fractures (not comminuted or intraarticular) that redisplaced after two closed reductions were allocated at random to one of two groups: Group I (n = 23), in which the dorsal bone deficiency was filled with bone cement (methylmethacrylate), and Group II (n = 25), in which the fracture was treated with external fixation. All were followed for one year and examined with respect to anatomy of the fracture, wrist movement, strength, appearance, pain, function, and complications. Roentgenograms, 12 months after treatment, showed that all

We present a fixation system for trochanteric or subtrochjanteric fractures using a dynamic external fixator. The Citieffe/CH-N fixator was proposed for elderly patients with a significant operative risk (ASA III or IV). A long dynamic screw measuring 9 mm in diameter is inserted into the neck and the head of the femur on a guide wire through a tube protecting skin and muscles. This long screw is connected to the body of the fixator to enable impaction at the fracture level if compression is needed. The dynamic screw can be released to allow free gliding to achieve a dynamic fixation of the fr

Distal tibial fractures are rare and difficult to treat because the bones are subcutaneous. External fixation is commonly used, but the method often results in delayed union. The aim of the present study was to find out the factors that affect fracture union in tibial pilon fractures. For this purpose, prospective data collection of tibial pilon fractures was carried out in 1998-2004, resulting in 159 fractures, of which 83 were treated with external fixation. Additionally, 23 open tibial fractures with significant > 3 cm bone defect that were treated with a staged method in 2000-2004 were ret

The incidence of intra-articular injuries associated with ankle fractures is high. Several studies demonstrated the role of ankle arthroscopy to aid in anatomic reduction, limit surgical exposure, and soft tissue trauma. It also acts as a reliable mean of diagnosis and intervention of intra-articular pathology associated with ankle fractures. There is insufficient evidence to suggest its use in all cases of ankle fracture fixation but it can be a very useful adjunct in the treatment of certain fractures. We present our experience in the use of this technique in the management of ankle fracture

Pediatric lunate fractures are exceedingly rare, with limited evidence to guide management. We present the case of a 12-year-old boy who sustained bilateral wrist injuries with a displaced right lunate fracture after an electric bicycle fall. The fracture was reduced and fixed percutaneously with a headless compression screw. At 8 months, radiographs confirmed union and the patient had full, pain free wrist motion without evidence of osteonecrosis or instability. We propose that percutaneous compression screw fixation is a reasonable option for achieving reliable union in older pediatric patie

PURPOSE: Patients with surgically treated ankle fractures are traditionally kept non-weightbearing for at least six weeks post-operatively; however, recent literature suggests numerous benefits of early weightbearing (EWB) before six weeks without significantly impacting long-term outcomes. This study aims to review the safety of early vs late weightbearing following ankle fracture fixation by assessing the complication rate.
METHODS: This was a single-centre retrospective study. Between 2020 and 2023, all ankle fixations that commenced weightbearing at two weeks were added to the EWB group.
