Nonunion of tibial stress fractures in patients with deformed arthritic knees. Treatment using modular total knee arthroplasty.

In two years we treated four women with ununited stress fractures of their proximal tibial diaphyses. They all had arthritis and valgus deformity. The stress fractures had been treated elsewhere by non-operative means in three patients and by open reduction and internal fixation in one, but had failed to unite. After treatment with a modular total knee prosthesis with a long tibial stem extension, all the fractures united. A modular total knee prosthesis is suitable for the rare and difficult problem of ununited tibial stress fractures in patients with deformed arthritic knees since it correct

Treatment of scaphoid fractures with carpal instability.

Since 1981, I treated 81 scaphoid fractures: 14 wrists (group A) with acute unstable fracture, 37 (B) with ununited fracture accompanied by dorsiflexed intercalated segment instability (DISI), and 30 (C) with ununited fracture without DISI. The methods and results were evaluated retrospectively. Bone union occurred in 100% of group A wrists, 92% of group B, and 90% of group C. By a modification of Cooney's clinical scoring chart, mean scores were 91 in group A, 84 in group B, and 87 in group C. In 43% of group A, a scapholunate gap remained after surgery. Of 21 wrists in group B treated by bon

Total knee arthroplasty for arthritic knees with tibiofibular stress fractures: classification and treatment guidelines.

Forty-two tibial and 5 fibular stress fractures in 34 patients with knee arthritis were radiologically classified into intraarticular malunited and ununited fractures, and extraarticular impending, acute, united, malunited, and ununited fractures. Depending on fracture type, total knee arthroplasty was performed using long-stem tibial component, metal augments, corrective osteotomy, or in routine fashion. At a mean follow-up of 36 months, the mean Knee Society knee score improved from 36.7 points preoperatively to 90.3 points; function score improved from 24 to 86.2 points. All fractures had u

Helical plating for a re-nonunion and a peri-implant fracture after humeral intramedullary nailing: A report of two cases.

INTRODUCTION AND IMPORTANCE: A re-nonunion after failed intramedullary nailing for a nonunion of the humeral shaft and a peri-implant distal humeral fracture with an ununited humeral shaft are rare cases. Therefore, no consensus has been established regarding the treatment strategies. CASE PRESENTATION: Case 1: An 84-year-old woman presented with humeral shaft re-nonunion after intramedullary nailing. The images revealed callus formation and loosening of the implant, suggesting a lack of mechanical stability. Nail removal and helical plating were performed. One year postoperatively, bony unio

Does Fixation Method Affect the Correlation of mRUST and Healing Strength?

BACKGROUND: Previous biomechanical studies correlating strength of healing with mRUST are limited to a single mode of intramedullary fixation. This study evaluated the correlation between mRUST and biomechanical strength in a fracture healing model using fixation methods that generated different modes of healing. METHODS: Biomechanical data were sourced from previous ovine osteotomy studies and included 24 sheep, 12 fixed with rigid constructs and 12 fixed with a single relatively stable construct. The sheep were sacrificed at 9 weeks and the tibiae were loaded to failure in torsion. Load to

Percutaneous screw fixation without bone grafting for established scaphoid nonunion with substantial bone loss.

Ununited fractures of the scaphoid with extensive bone resorption are usually treated by bone grafting and internal fixation, using either an open or a minimally invasive technique. We studied the feasibility of percutaneous fixation without bone grafting in a consecutive series of 27 patients with established nonunion of an undisplaced fracture of the scaphoid and extensive local resorption of bone. They were treated by a single surgeon with rigid fixation alone, using a headless cannulated screw inserted through a volar percutaneous technique. Clinical examination, standard radiographs and C

Olecranon Fracture through Persistent Olecranon Apophysis in a 21-Year-Old Male: A Case Report and Systematic Review of the Literature.

Traumatic fractures involving an ununited olecranon apophysis in adults have been rarely documented in the literature. We present the case of a 21-year-old male wrestler with an elbow injury after a fall. Imaging revealed an acute fracture of the olecranon with sclerotic rounded edges indicating an injury through a persistent olecranon apophysis. Open reduction and internal fixation was performed with plate fixation and bone grafting with radiographic and clinical healing at 6 weeks. Review of the literature revealed 5 case reports showing high rates of non-union with tension band constructs w

Fixation of scaphoid delayed union and non-union with absorbable polyglycolide pin or Herbert screw. Consolidation and functional results.

Internal fixation with an absorbable polyglycolide pin or a Herbert screw was used to treat 34 patients with ununited and delayed union of fractures of the carpal scaphoid. The late clinical result could be reviewed in 14 patients in the polyglycolide group and 10 in the Herbert group with an average follow-up time of 68 and 58 months, respectively. The rate of union was 64% in the polyglycolide group and 60% in the Herbert group. The functional outcome was better in the Herbert group than in the polyglycolide group. A transient local abacterial tissue reaction occurred in 5 of 20 (25%) patien

Amputation as a salvage for patients with incapacitating infected fractures of the lower limbs.

Most serious lower-limb injuries with severe bone and soft tissue damage heal when treated wisely by techniques available today. Unfortunately, a small percentage of patients face prolonged physical and economic disability as a result of extensive damage to bone and soft tissue, nonunion of fractures with infection, nerve and blood-vessel injury, and complications of surgical treatment. Every effort should be made to effect union by established methods but a small percentage of fractures do not heal satisfactorily. Failure of infected lower-limb fractures to unite after prolonged treatment or

Interobserver reliability of computed tomography to diagnose scaphoid waist fracture union.

PURPOSE: To determine the interobserver agreement and diagnostic performance characteristics of computed tomography (CT) for determining union of scaphoid waist fractures. METHODS: A total of 59 orthopedic and trauma surgeons rated for union a set of 30 sagittal CT scans of 30 scaphoid waist fractures. Of these fractures, 20 were treated nonoperatively, were imaged between 6 and 10 weeks after injury, and were known to have eventually achieved union. Ten were operatively confirmed to be ununited. We rated each scan as united or ununited using a Web-based rating application. We assessed intero

Osteochondritis dissecans of the knee.

A case of osteochondritis dissecans of the knee is described in which the site of the lesion, on the supero-medial aspect of the lateral femoral condyle, is unique. This case supports the theory that direct recurrent minor trauma causes osteochondral fractures which remain ununited producing the characteristic lesion.

Localized naviculocuneiform arthrodesis combined with osteosynthesis of fracture nonunion of the tarsal navicular bone using a locked plating system.

AIMS: Navicular fracture is still challenging disorder to treat because there is a risk of nonunion, avascular necrosis and symptomatic osteoarthritis. PATIENTS METHODS AND RESULTS: A 40-year-old woman with an ununited fracture of the tarsal navicular bone was treated with localized naviculocuneiform arthrodesis (arthrodesis of the navicular, the middle cuneiform, and the lateral cuneiform). Fusion with the navicular, the middle cuneiform, and the lateral cuneiform as well as union of fracture of the navicular was confirmed on the radiographs. CONCLUSION: Localized naviculocuneiform arthrode

Excision of a Rare Triquetral Body Fracture Nonunion.

Aside from the more common dorsal avulsion fractures, isolated triquetral body fractures are a rare injury and often missed. When they are identified, conservative treatment via immobilization is often the standard of care for initial treatment. Rarely, triquetral body fractures can develop into symptomatic nonunions, causing considerable pain and disability. Multiple classification schemes have been described to categorize triquetrum fractures; however, distal triquetrum fractures fit into none of the established models. There is scarce literature describing treatment of triquetral body fract

Malunited ankle fractures. The late results of reconstruction.

We have analysed 31 malunited ankle fractures treated by reconstructive osteotomies. In all cases the malunited fibula has been corrected; this has been combined with other osteotomies and with fixation of ununited fragments as indicated. Our results show that, unless function was severely impaired, reconstruction was always worth while, no matter how long ago the injury had occurred and even if there were already arthritic changes.

Unusual appearances following intracapsular neck of femur fractures.

PURPOSE: The aim of the study was to investigate the possible outcomes of intracapsular femoral neck fractures managed non-operatively or with a simple form of internal fixation. METHODS: We reviewed 30 fractures and their post mortem reports from the Galler collection, a modern pathology reference series. RESULTS: Our investigation revealed unusual appearances of the hip following femoral neck fractures that are not usually seen in modern orthopaedic practice, including previously undescribed acetabularisation of the femur and ankyloses of ununited femoral heads. CONCLUSION: Appreciation o

Avascular necrosis of the scaphoid in children treated by splint immobilisation: a report of two cases.

Avascular necrosis of the scaphoid following a fracture in children is rare and there is no established treatment protocol in the literature. We present two boys with nonunion and avascular necrosis of the scaphoid treated by simple immobilisation. Both cases healed with painless wrists and full movements. Our cases confirm that an ununited scaphoid fracture in children may heal with conservative treatment, even when an MRI scan suggests avascular necrosis. Unlike in adults, operative treatment need only be considered in children when conservative treatment fails.
