Reconstruction of soft-tissue defects using serratus anterior adipofascial free flap.

The serratus anterior muscle has been suggested as a versatile and reliable flap for reconstruction of head and neck and extremity injuries. The adipofascial layer overlying the serratus anterior muscle is the anatomic layer, which is supplied by the same branch of thoracodorsal artery. Even though great progress has occurred in the prevention of postoperative adhesion of extremity injuries, the problem has not been completely solved and is still of special importance in complex injuries. Between March of 1995 and February of 1996, seven patients underwent reconstructive operation as a result

[Free flap-plasty in a larger soft tissue defect of the hip and gluteal region].

The purpose of this paper is to show a case in which we felt at the end, that the chosen procedure did not fulfill our expectations and in which an alternative procedure would have been preferable. In a 47 year old woman a deep, healed soft tissue defect between the left trochanter and the iliac crest was built up with a free latissimus dorsi musculocutaneous flap. The original plan was to suture the vessels to the gluteal vessels and the nerve to the gluteal nerve. The muscle would have been placed in such a way that the muscle fibres would run from the iliac crest to the trochanter to replac

Clinical outcomes of perforator-based propeller flaps versus free flaps in soft tissue reconstruction for lower leg and foot trauma: a retrospective single-centre comparative study.

BACKGROUND: The efficacy and safety of perforator-based propeller flaps (PPF) versus free flaps (FF) in traumatic lower leg and foot reconstructions are debated. PPFs are perceived as simpler due to advantages like avoiding microsurgery, but concerns about complications, such as flap congestion and necrosis, persist. This study aimed to compare outcomes of PPF and FF in trauma-related distal lower extremity soft tissue reconstruction. METHODS: We retrospectively studied 38 flaps in 33 patients who underwent lower leg and foot soft tissue reconstruction due to trauma at our hospital from 2015

Clinical application of free inguinal flaps with retrograde blood supply anastomotic to repair soft tissue defects of extremities.

To observe the clinical efficacy of free inguinal flaps with retrograde blood supply anastomosis to repair skin and soft tissue defects in the limbs. A total of 25 patients with soft tissue defects of the limbs treated from January 2019 to December 2021 were selected and repaired with free inguinal flaps anastomotic with retrograde blood supply. All 25 skin flaps survived; 1 patient had skin flap infection and the wound healed gradually after symptomatic treatment, and 1 patient had venous embolism and the skin flap survived after re-anastomosis. The patients were followed up for 6 to 18 month

Ultrathin Free Flaps to the Foot and Ankle: New Options for Optimal Soft Tissue Coverage and Functional Contour.

Soft tissue defects and chronic wounds around the foot and ankle can prove difficult to reconstruct using conventional techniques. Traditional flaps used for coverage in this region can result in the need for future debulking, shoe gear modifications, donor site morbidity, decreased function, and scarring. Successful reports of ultrathin free flaps harvested along suprafascial planes have yet to be described in the foot and ankle literature. We present 2 cases of ultrathin flaps used for foot and ankle defects that provide optimal contour while not limiting anatomic function. The resultant fun

Emergency repair of upper extremity large soft tissue and vascular injuries with flow-through anterolateral thigh free flaps.

BACKGROUND/OBJECTIVES: Complex extremity trauma commonly involves both soft tissue and vascular injuries. Traditional two-stage surgical repair may delay rehabilitation and functional recovery, as well as increase the risk of infections. We report a single-stage reconstructive surgical method that repairs soft tissue defects and vascular injuries with flow-through free flaps to improve functional outcomes. METHODS: Between March 2010 and December 2016 in our hospital, 5 patients with severe upper extremity trauma received single-stage reconstructive surgery, in which a flow-through anterolate

Use of de-epithelialized free perforator flaps for simultaneous bone and soft-tissue reconstruction in chronic calcaneus osteomyelitis.

Treating chronic calcaneal osteomyelitis (OM) is complex and challenging. After aggressive surgical debridement of the infected tissue, filling the bone's dead space and covering the defect may be necessary. This study describes the successful treatment of chronic calcaneal OM using a de-epithelialized perforator flap to reconstruct soft-tissue and bone defects simultaneously. The study retrospectively reviewed the medical records of 10 patients diagnosed with Cierny-Mader type III-IV calcaneus OM treated with de-epithelialized free perforator flaps between 2015 and 2023. The data included pat

The use of microvascular free flaps for soft tissue augmentation of the face in children with hemifacial microsomia.

The extent of facial deformity from hemifacial microsomia varies considerably. Minor degrees of asymmetry may be barely perceptible. Severe defects involve deficiencies and asymmetry of bone and soft tissue. Hence, reconstruction of both soft and hard tissues may be necessary. The authors have utilized a free flap based on the superior gluteal vessels to replace soft tissue deficits in three children with hemifacial microsomia. The flap, composed of fascia lata, adjacent muscle and fat (two patients) and overlying skin (one patient), is harvested through an incision high on the lateral thigh i

"INVOS" WE TRUST. Tissue Oximetry for Free Flap Monitoring in Lower Limb Reconstruction.

INTRODUCTION: Free flaps on the lower limb could make the difference between salvage and amputation. Regional tissue oximetry (rSO₂) measured by near-infrared light is a tool that is not yet widely described or recognized, as most published studies focus on its use in breast flap monitoring. However, in the context of lower limb reconstruction, it offers an objective and real-time evaluation of flap tissue perfusion, enabling faster responses for salvage compared to traditional clinical monitoring. MATERIAL AND METHODS: We conducted a retrospective study comparing lower limb free flap monitor
