The author reviews the case of Emmy von N. and provides a historical overview of the Gilles-de-la-Tourette syndrome (tic convulsif) referring also to recent studies of this illness. She revises Freud's diagnosis of hysteria in favour of the Gilles-de-la-Tourette syndrome.

This paper discusses and compares Freudian and Kleinian conceptions of the development of early mental life from the vertices of the genetic hypothesis (and reconstruction), the infantile neurosis, the conception of the evolution of the infant, the issue of reality, and the oedipus complex. In so doing, some of the newer findings of infant development are considered. We can now correlate Kleinian conceptions of early infantile reconstruction in light of these new developments; this paper seeks further to demonstrate that the classical notion of primary narcissism, which is laterally known as t

Suicide represents a complex clinical phenomenon that cannot be adequately understood through categorical diagnoses or risk-factor models alone. This narrative, historical, and critical review examines suicide through a psychoanalytic lens, integrating intrapsychic, intersubjective, and sociocultural dimensions. Drawing on major Freudian and post-Freudian traditions, this paper explores how unconscious dynamics, such as mental pain (psychache), guilt, shame, narcissistic injury, aggression, and fantasies of annihilation or rebirth, organize suicidal ideation and behavior in clinically meaningf

The author questions the classical psychoanalytic assumptions that anxiety in posttraumatic nightmares arises from the same conflict represented in the manifest content of the nightmare, that such nightmares have no latent content, and that the day residue is insignificant. Such assumptions obscure the function of these nightmares, in which the dreamwork transforms shame into fear. Case material from two patients highlights the instigating role of posttraumatic dissociative states and shame in the day residue of posttraumatic nightmare sufferers. The author also emphasizes that conflict in the

Many factors go into a choice of a therapeutic focus: the patient's psychopathology; the therapist's training, countertransference reactions, and ideological beliefs; and, importantly, a decision about what seems most amenable to treatment and change. As a theory, self psychology describes one aspect of the paranoid process; as such, it is an incomplete theory that complements rather than invalidates more classical theories. As a technique, however, it suggests a style and focus conducive to working with paranoid patients, one that is markedly supportive, nonconfrontational, yet also interpret

To start with the clinical description of anorexia nervosa is indicative of the difficulty involved not only with regard to its etiology (whether it is an endocrine, neurological or psychological disorder) but also with regard to its diagnosis (whether or not it is a hysterical disorder, whether or not it is a disease as such) and with regard to its treatment (the stubbornness of the symptom). The early-onset differential criterion is rejection. The subject's eating-related problem must be placed within the framework of the radical dependency the human organism has with regard to the mother's

The xenophobia directed at Gypsies has received little attention from the psychoanalytic vantage. The author approaches the subject by drawing upon Freud's work on the uncanny and Fenichel's on anti-semitism in order to analyse the transformation of fear of the foreign into hatred of the foreign. With reference to the difference in significance between anti-semitism and hostility towards Gypsies as operative factors in the process of western civilization, the author argues that, like anti-semitism, antagonism towards Gypsies is based in the projection of unacceptable aspects of the self onto o

An examination of Breuer's treatment of Anna O. illustrates some of the controversies surrounding the recent rise of case reports of multiple personality disorder. Anna O., the first patient of the cathartic method, psychoanalysis, and dynamic psychiatry, fits current criteria for multiple personality disorder. Breuer's treatment, however, may have contributed to her states of absence; the timing, type, and intensity of Breuer's interventions make it possible that he unwittingly encouraged and amplified Anna's dissociations, reified her ego fragments, and then explained Anna's symptoms with th

The capacity to tell a joke is a highly complex interpersonal event that depends upon the maturation of certain developmental achievements which are absent or stunted in children with Asperger's Syndrome. These include the ability to know another's mind, a sense of interpersonal timing and, most notably, a capacity for abstract thinking. The author discusses Freud's () notion of joke-work, which is akin to dream-work, both of which are pathways to forming mental representations. Freud considered joke-work as a mental activity that operated on the verbal level and the author examines the prever

We describe a new type of agnosia consisting of an impairment in the ability to mentally represent or know what one is feeling. Freud the neurologist coined the term "agnosia" in 1891 before creating psychoanalysis in 1895 but the term has not been previously applied to the domain of affective processing. We propose that the concept of "affective agnosia" advances the theory, measurement and treatment of what is now called "alexithymia," meaning "lack of words for emotion." We trace the origin of the alexithymia construct and discuss the strengths and limitations of extant research. We review

The topic involves two issues. They can be related either to different clinical pictures or to divergent opinions about the same clinical facts. But the above mentioned opposition can be found in Freud's work. Listening in analysis differentiates between situations where the distinction of affect and representation is blurred within the general frame of communication and others where this distinction imposes itself because of the critical or chronic character of the predominance of affect. Problems about the discrimination between affect and representation in the unconscious are elucidated in

To avoid the passions of love and hate between them, therapists and patients construct and preserve negative barriers of angry criticism, dissatisfaction, and sadomasochism. This review aims to help therapists tolerate and work with loving feelings between patient and therapist for the benefit of the patient.

Argument Animals played an important role in the formation of psychoanalysis as a theoretical and therapeutic enterprise. They are at the core of texts such as Freud's famous case histories of Little Hans, the Rat Man, or the Wolf Man. The infantile anxiety triggered by animals provided the essential link between the psychology of individual neuroses and the ambivalent status of the "totem" animal in so-called primitive societies in Freud's attempt to construct an anthropological basis for the Oedipus complex in Totem and Taboo. In the following, we attempt to track the status of animals as ob

The attachment theory of Bowlby and his followers has not previously been applied to the interpretation of charismatic phenomena in the bonding of large numbers of people with their leaders in crisis. This essay, which builds on the author's 'Surviving trauma: loss, literature and psychoanalysis', attempts to use attachment theory and sociological concepts such as homogamy, anomie, negative imprinting and de-individuation, as well as classical Freudian concepts such as transference and splitting of the ego, in interpreting the charismatic bond. It is argued that charismatic homogamy becomes vi

The urgency of the problem of how to learn from the relatively recent past in order not to repeat its devastating effects, a problem that revolves around the ethics of memory and history, has combined with an awareness that later generations of victims and perpetrators-the "post" generations-may find themselves inhibited in relation to moving forwards because they are not truly "post" at all. It is as if they are haunted by the experiences of their predecessors, which is passed on in some way through stories and selective silences, as well as through the older generation's ways of handling the

The author uses Freud's ideas on the natural period of an analysis as a theoretical background for the presentation of a clinical case. The patient came to analysis as a girl of 15. For the two years before the consultation Inés had suffered from 'absences' when faced with situations she has found difficult to cope with. There appeared to be no neurological basis for these absences of consciousness. The analysis has now lasted for two and a half years. The author presents the facts of the case with a view to the response of his readers as to the indications for termination. He asks for the ide

Social phobia is a pervasive pattern of social inhibition, feelings of inadequacy, and hypersensitivity, occurring in about 18% of the clinical population. Despite good results with cognitive-behavioural treatment, social phobia seems to be a chronic disorder with several complications. The author describes an analysis of a divorced woman who was exposed to an early premature sexual seduction by her father, abruptly terminated because of an accident. The loss of the father was repaired by a delusional system as defence against the re-emergence of a catastrophic situation. Her compulsion to rep
