[The development of gender identity beyond rigid dichotomy].

The conflicts individuals with ambiguous sexual characteristics suffer from are not the result of genetic features but of the rigid and dichotomous gender order, which is currently undergoing a renaissance. This also applies to individuals with an uncertain gender identity. In the best interests of the child a concept of gender seems necessary, that goes beyond a binary separation and allows gender-specific intermediary stages in the personal development of identity. Such a gender concept can be developed following psychoanalytic theories. The present discourse contains a scale of connecting f

Gender identity disorder and its medico-legal considerations.

The general belief among behavioural scientists and physicians is that gender identity disorder or transsexualism is an identifiable and incapacitating disease which can be diagnosed and successfully treated by reassignment surgery in carefully selected patients. Although many advances have been made in the reassignment surgery techniques, phalloplasty still remains a major challenge; to date, no ideal technique has been developed. The new gender created by the reassignment surgery has, in turn, led to many legal complications for post-operative transsexuals because, in many developed and the

[Transexual gender identity disorders: diagnostic and legal issues].

The current classification systems (ICD-10, DSM-IV), epidemiology, symptoms and diagnostic features of gender identity disorders are discussed. In the best interest of the patients, and in view of the legal situation, treatment has to follow set "standards of care". The necessity of a psychotherapeutically guided real life test, lasting at least 1 year before somatic treatment can begin, is again pointed out.

Gender identity disorder in adolescence: outcomes of psychotherapy.

Gender identity disorder (GID) is characterized by strong and persistent cross-gender identification and by persistent discomfort with one's sex. This paper reviews reports of GID in which adolescent psychotherapy patients initially desired sex reassignment. In addition, four case studies of adolescents with GID, demonstrating different outcomes of psychotherapy, are presented. It is concluded that great caution must be exercised when treating adolescents with GID, and that sex reassignment must not be started before patients have reached eighteen years of age.

A clinical approach to childhood gender identity disorder.

A review of gender identity disorder is presented along with a case of a four-year-old boy. An overview of some of the theories of gender identity disorder directs our attention via the clinical material to conflict/defense as the most useful. The youngster's cross-gender behavior, possible contributions to it, treatment issues, parental input, boundaries, and transitional objects are considered. The necessity of close communication, cooperation, and guidance to parents, as well as involving them in individual and family therapy, is stressed.

Cross-gender identity in transvestites and male transsexuals.

A self-theory of transvestism and secondary transsexuality in which gender identity is a major self subsystem has been advanced in previous research. Within this framework transsexuals and transvestites were compared on a number of developmental characteristics. While early-onset transsexuals (n = 103) were dominantly female, both late-onset transsexuals (n = 52) and transvestites (n = 36) showed much more feminine behavior than expected. This was interpreted as a sign that they were already developing a feminine gender identity in their early years. Implications for this theory were discussed

[Sex/gender ratio of adolescents with gender dysphoria of the Gender Identity Treatment Unit of Asturias].

BACKGROUND AND OBJECTIVE: Demand from minors with complaints of gender dysphoria has increased in recent years. This increase has been more pronounced in adolescent trans men in some international research studies. The first objective of this research study was to determine the sex/gender ratio of minors requesting a consultation in the Gender Identity Treatment Unit of the Principality of Asturias (UTIGPA) and presenting complaints of gender dysphoria. The second objective was to analyse the relationship of the sex/gender ratio with the age variable at which they requested the first consultat

Gender Identity, Hormone Therapy, and Cardiovascular Disease Risk.

Transgender individuals represent a medically underserved and under researched population. There is a growing number of studies illustrating the importance of hormone therapy treatments in transgender men and women to assist ameliorating gender dysphoria and promoting well-being. However, the cardiovascular effects of these hormones are controversial. Large longitudinal epidemiological studies of cardiovascular event outcomes in these populations do not exist. In addition, studies of cardiovascular complications of transgender hormone therapy are limited in number and complicated by poor contr

School refusal by patients with gender identity disorder.

OBJECTIVE: The accumulating evidence suggests that school refusal behavior is associated with severe negative outcomes. However, previous research has not addressed school refusal by patients with gender identity disorder (GID). In this study, we tried to clarify the prevalence of school refusal among GID patients and the relationship of school refusal to demographic characteristics. METHODS: A total of 579 consecutive Japanese GID patients at the outpatient GID Clinic of Okayama University Hospital between April 1997 and October 2005 were evaluated. RESULTS: The prevalence of school refusal

Suicidal ideation among patients with gender identity disorder.

In this study, we tried to clarify the prevalence of suicidal ideation and self-mutilation including suicide attempts among patients with gender identity disorder (GID) and the relationship of those behaviors to demographic characteristics. A total of 500 consecutive Japanese GID patients without any other psychiatric comorbidity were evaluated at the outpatient GID Clinic of Okayama University Hospital. The lifetime rate of suicidal ideation was 72.0% of the total sample. There were no significant differences in the prevalence of suicidal ideation among groups divided by sex, age, age at onse

Psychiatric comorbidity among patients with gender identity disorder.

AIMS: Psychiatric comorbidity and mental instability seem to be important unfavorable prognostic factors for long-term psychosocial adjustment in gender identity disorder (GID). However, psychiatric comorbidity in patients with GID has rarely been assessed. In this study, we investigated the psychiatric comorbidity and life events of patients with GID in Japan. METHODS: A total of 603 consecutive patients were evaluated independently by at least two senior psychiatrists at the GID clinic using clinical information and results of examinations. RESULTS: Using DSM-IV criteria, 579 patients (96.

Schizophrenia modifying the expression of gender identity disorder.

INTRODUCTION: According to the Brazilian Federal Medical Association, transsexualism is recognized as a gender identity disorder if a long-term diagnostic therapeutic process has demonstrated that the transposition of gender roles is irreversible, and if only hormonal and surgical procedures are appropriate to relieve the stress associated with the gender identity. Although such treatment will only be initiated with caution and after a long phase of intense diagnostic screening, the differentiation between pure identity disorders and transsexual feelings secondary to an ongoing psychopathologi

Comorbidity of Asperger syndrome and gender identity disorder.

The case of a 35-year-old biological woman with Asperger syndrome (AS) and gender identity disorder (GID) fulfilling DSM-IV criteria is reported. Against the background of recently emerging theories of cognitive male pattern underlying autism we present additional psychological assessments in order to discuss any possible interaction or discrimination between AS and GID. Whilst we explain GID as a secondary feature of AS, we examine the assumption of the necessity of treating GID in AS as a primary GID in accordance with international standards. We consider the treatment of GID as compelling,

Birth order in girls with gender identity disorder.

This study examined the birth order of girls with gender identity disorder (N = 22). Each proband was matched to 3-7 clinical control girls for age at assessment and number of siblings (the mode number of controls per proband was 7) (total N = 147). The number of older brothers, older sisters, younger brothers, and younger sisters was recorded. Slater's birth order index showed that the probands were significantly more likely to be early born than were the controls. A modified Slater's index also compared the birth order of the probands and the controls only to their brothers (when they had on
