Transgender and gender-diverse (TGD) people are disproportionately impacted by incarceration, interpersonal violence, HIV and other sexually transmitted infections, substance use disorders, and suicidality. Little is known about successful approaches to improve health outcomes for TGD individuals impacted by incarceration. We review the barriers to providing gender-affirming clinical care in correctional systems in the United States, identify key knowledge gaps regarding the provision of gender-affirming care to incarcerated TGD populations, and highlight necessary steps to improve the health …

BACKGROUND: Transgender people report increased prevalence of mental distress and suicide attempts compared to the general population. Gender-based minority stressors such as discrimination and victimization are positively associated with mental health problems while social and medical gender transition is inversely associated with mental health problems. Barriers to accessing gender-affirming health care is reported by transgender people in many countries, including Norway which has a state-funded public health care system. The aim of the current study was to examine gender-affirming health c

Political debate regarding trans youth's access to gender-affirming care (GAC) has pushed many to advocate for GAC by pointing to tragic, pathological outcomes of non-treatment, namely suicide. However, these pathologized arguments are a harmful ethical "shortcut" which should be replaced by a meaningful engagement with the ethics of providing GAC to youth.

Over the past decade, access to and insurance coverage for gender-affirming medical and surgical treatment for transgender (trans) individuals in the U.S. has improved. Despite this, trans individuals continue to experience insurance-related barriers to gender-affirming care (GAC)-and in particular, transmasculine individuals may face gender-specific barriers resulting from regulations on masculinizing hormones. In this study, we interviewed transmasculine individuals from across the U.S. about their gender-affirming care experiences with a focus on insurance, payment and policy-related barrie…

The transgender community makes up a significant portion of the American population, yet there is a paucity of data available to surgeons and anesthesiologists. The purpose of this article is to provide information to surgeons, anesthesiologists, and other medical professionals. This narrative review explores the importance of medical information that healthcare professionals should be aware of ahead of gender-affirming surgeries. This review paper's purpose is to explore perioperative considerations for physicians before gender-affirming surgeries. Physicians should be aware of the hormone th

BACKGROUND: Gender Dysphoria (GD, formerly known as transsexualism) describes distress and impairment in an individual caused by the incongruence between the experienced gender and the sex assigned at birth (Gender Incongruence: GI). Transgender health care focusses on gender-affirmative treatments (e. g., hormone therapy) and associated needs (e. g., psychotherapy). Moreover, genital surgery is considered an effective treatment to reduce GI/GD and improve mental health and quality of life. Interdisciplinary cooperation between the associated medical facilities is regarded as evidence-based he

The provision of gender affirming hormone therapy for transgender and non-binary people is a rapidly developing area of gender affirming healthcare. While research indicates the benefits of providing gender affirming hormone therapy through interdisciplinary primary care-based models, less is known about how service users and providers construct their understandings of affirmative approaches. In this paper, we present findings from a discourse analysis of four service users' and four healthcare professionals' talk about a primary care-based pilot clinic providing gender affirming hormone thera

In order to access gender-affirming care, transgender individuals were historically required by international guidelines to undergo mental health provider assessment (Coleman et al., 2012). This requirement for universal mental health provider involvement, initially formulated via professional expert opinion, has not been retained in the most recent World Professional Association for Transgender Health Standards of Care (WPATH SOC 8; Coleman et al., 2022). In the present analysis, I sought to examine the historical and cultural contexts of these expert opinions codified in the first version of…

BACKGROUND: Transgender and gender-diverse (TGD) people face significant discrimination in the South African (SA) health system, limiting their access to HIV services and gender-affirming care, which supports an individual's gender identity when it does not align with their sex assigned at birth. Despite the critical role of these services for TGD people, access to care remains understudied in SA.
OBJECTIVES: To describe TGD people and their access to and need for social, legal and medical transition, including psychosocial care, hormone therapy and surgery, as well as HIV services, in the Ea…

Research has identified discrimination and a lack of knowledgeable providers as major barriers for transgender and gender diverse (TGD) individuals seeking care, which contributes to greater stress and significant health disparities affecting this population. However, research involving TGD youth is limited. The aim of this study, therefore, was to describe TGD adolescents' experiences, concerns and needs in healthcare settings, including their feedback on themes previously identified by healthcare providers (i.e. discomfort with gender-related topics, reasons for not asking patients about gen

Transgender and gender-diverse (TGD) individuals experience profound inequities in sexual health due to intersecting social, structural, and clinical barriers, including limited provider competency. Using a nursing lens, we examine clinical practices for delivering sexual health care to TGD populations. Specifically, we address sexual identity, sexual function, gender-affirming care considerations, and clinical guidance for comprehensive sexual health assessment. We also discuss screening, prevention, and treatment of sexually transmitted infections and HIV. Emphasizing person-centered and tra…

BACKGROUND: The World Professional Association for Transgender Health (WPATH) recommends referral letters from two mental health providers within one year of gender-affirming genital surgery (gGAS) to ensure patient readiness before primary surgeries. Many U.S. health insurance plans will not authorize second- and third-stage surgeries or revision surgeries without two referral letters. Such requirements are not supported by WPATH guidelines.
OBJECTIVES: This study investigates insurance requirements for referral letters and their negative impact on care.
