Facial plastic surgery & aesthetic medicine2020eng
Background: Gender-affirming surgery may be pursued by individuals experiencing gender dysphoria. Although genital and chest procedures are classified as medically necessary, facial feminization surgeries (FFSs) are often considered cosmetic. Insurance companies may limit coverage of these procedures, especially in states less supportive of transgender individuals. Objectives: To determine insurance coverage and ease of finding policy information for FFSs, and to analyze differences based on state advocacy. Methods: Insurance policies for the top three commercial health plans per state were re

Introduction Gender affirmation surgery includes procedures of the face, larynx, chest, reproductive system, external genitalia, and adipose tissue performed to ameliorate incongruence between gender identity and phenotype. The annual number of gender affirmation surgeries performed in the United States has increased significantly. There have been no investigations into the frequency of peripheral neuropathy after gender affirmation surgery, which is an important topic, given transgender individuals are at increased risk for delaying necessary medical care. After appreciating a number of cases

PURPOSE OF REVIEW: Urethral complications remain a major source of morbidity following gender-affirming genital surgery, particularly in the setting of masculinizing procedures requiring urethral lengthening. This review summarizes recent advances in the epidemiology, prevention, and reconstructive management of urethral complications, with a focus on principles relevant to reconstructive urologists.
RECENT FINDINGS: Urethral complications are uncommon after feminizing genital surgery but occur frequently following masculinizing surgery, with pooled rates of strictures and fistulae approachin

Feminine facial features may be a source of significant gender dysphoria in certain patients. A masculine appearance includes an M-shaped hairline, wider and taller forehead, supraorbital bossing, larger and wider nose with a large dorsal hump, broader chin, wider mandible with a sharper angle, thinner lips, presence of facial hair, and prominent thyroid cartilage. Given these critical differences between feminine and masculine facial features, a comprehensive and systematic way of evaluating these distinct structures is necessary before facial gender-affirmation surgery. While no single appro

For patients with gender dysphoria, gender-affirming surgery of the face has been shown to vastly improve quality of life. The mandible is one area of the face that has distinct feminine and masculine presentations. This article will review gender-affirming surgery of the lower jaw, both for feminization and masculinization. Techniques for bony contouring and soft tissue manipulation will be discussed.

BACKGROUND: Facial gender-affirming surgery (FGAS) is described as a set of surgical procedures done to feminize the soft tissue and the facial skeleton, allowing for transfeminine individuals to be recognizable as women to others. It is established in the literature that the most significant facial area for determination of gender is the forehead (Spiegel in Laryngoscope 121:250-261, 2011). This article describes the author's three main surgical techniques used in forehead feminization and reports on the results.
METHODS: The type of surgery performed is based on the patient's anatomy. Type

Background: Gender-affirming facial surgery (GFS) is becoming more widely available for transgender individuals, but data on surgical approaches and outcomes remain limited. Methods: Retrospective analysis of surgical outcomes among consecutive GFS cases performed at a tertiary care academic center between March 2016 and August 2020. Results: Seventy-seven patients underwent 109 surgeries, including 478 individual procedures. The median age was 42 years. Ninety-five percent of patients had public health insurance. Two-stage GFS was often used in older patients (p = 0.001), with the first stage

OBJECTIVES/HYPOTHESIS: To determine insurance coverage for gender-affirming surgery and voice therapy for individuals who seek to align their voice with their gender identity, and to analyze differences based on state-by-state transgender equality.
STUDY DESIGN: Cross-sectional study.
METHODS: Policies from the top three commercial insurers per state in 2019 were reviewed. Coverage status was determined by web-based search, telephone interviews, and email inquiries. A state-by-state equality score was calculated based on the number of laws and policies relating to the transgender community.

BACKGROUND: Gender-affirming surgery is a growing field to address gender dysphoria, which is an increasingly recognized condition. The surgical robot is an excellent tool for performing some gender-affirming procedures and has been utilized extensively in both feminizing and masculinizing surgery.
OBJECTIVES: To provide an overview for the use of the surgical robot in gender-affirming surgeries.
DESIGN: This is a scoping review.
METHODS: A literature search was conducted by the authors via PubMed and Google Scholar.
RESULTS: The ability of the robot to operate within the pelvis makes it a

BACKGROUND: It can be argued that individuals in the transgender and gender nonbinary (TGNB) community suffer from a similar impact on quality of life in the face of gender dysphoria, with psychosocial and physical consequences. Indications for penile allotransplantation for patients wishing to undergo gender affirmation surgery have yet to be elucidated, but technical lessons for feasibility can be gleaned from penis transplants that have already been performed on cisgender males to date.
METHODS: This study investigates the theoretical feasibility for penile-to-clitoral transplantation, pri

IMPORTANCE: Requests for gender-affirming surgeries are rapidly increasing among transgender and gender diverse (TGD) people. However, there is limited evidence regarding the mental health benefits of these surgeries.
OBJECTIVE: To evaluate associations between gender-affirming surgeries and mental health outcomes, including psychological distress, substance use, and suicide risk.
DESIGN, SETTING, AND PARTICIPANTS: In this study, we performed a secondary analysis of data from the 2015 US Transgender Survey, the largest existing data set containing comprehensive information on the surgical an

BACKGROUND: The optimal approach for vaginectomy in transmasculinizing surgery is not well-defined, with options for sharp excision of vaginal epithelium or electrocautery fulguration of the vaginal mucosa.
AIM: This study compares the outcomes of excisional and fulguration vaginectomy approaches.
METHODS: A retrospective review of patients who underwent transmasculine surgery from 2021 to 2023 at a single institution was performed. All patients had vaginectomy with metoidioplasty.
OUTCOMES: Post-operative complications, estimated blood loss (EBL), and operating room (OR) time were compared

Introduction With the growing acceptance of transgender individuals, the number of gender affirmation surgeries has increased. Transgender individuals face elevated depression rates, leading to an increase in suicide ideation and attempts. This study evaluates the risk of suicide or self-harm associated with gender affirmation procedures. Methods This retrospective study utilized de-identified patient data from the TriNetX (TriNetX, LLC, Cambridge, MA) database, involving 56 United States healthcare organizations and over 90 million patients. The study involved four cohorts: cohort A, adults a
