Hip fractures in the geriatric population are associated with high morbidity and mortality rate. Early surgical fixation is of major importance, as it is one of the factors that contribute to patient optimization. However, these patients usually present a high burden of comorbidities, including aortic stenosis that could affect their treatment. Despite major advances in anesthetic and surgical techniques, severe aortic stenosis remains an independent, important risk factor for patients undergoing anesthesia for noncardiac surgery. In these patients, general and/or neuraxial anesthesia should b

INTRODUCTION: The number of older adults living with frailty undergoing gastrointestinal cancer surgery is increasing. To address the unique needs of the population, a whole pathway perioperative geriatrics strategy-PRIME-was developed to integrate geriatric principles into surgical care. The objective of this study was to evaluate the implementation of PRIME using validated structural, process, and outcome quality indicators.
MATERIALS AND METHODS: This retrospective cohort study included 106 consecutive patients aged 70 years and older who underwent gastrointestinal surgery for cancer or pr

We report the case of a 77-year-old, American Society of Anesthesiologists (ASA) physical status IV man who developed profound and sustained electrocerebral silence (ECS) during a hemicolectomy. This critical electroencephalogram (EEG)-detected event, occurring in the absence of hypotension, prompted the immediate discontinuation of all anesthetic agents. The procedure, converted to an open laparotomy, was completed without hypnotic or analgesic support, guided solely by the persistent neurophysiological suppression. The patient emerged hours later, developing postoperative delirium and object

Hip fractures in the elderly population are associated with considerable complications such as morbidity, mortality, and enduring functional deterioration. Thus, they render perioperative management an essential element of strategies aimed at fostering healthy aging. Regional anesthesia (RA) and general anesthesia (GA) constitute the two predominant methods for the surgical intervention of hip fractures; however, the most effective technique remains a subject of ongoing scholarly debate. This research review seeks to systematically assess and compare regional anesthesia to general anesthesia i

BACKGROUND: Geriatric patients are susceptible to respiratory and hemodynamic adverse events during endotracheal intubation and extubation due to anatomic and physiological changes with aging. Supraglottic airway devices (SADs) provide reduced airway morbidity and increased hemodynamic stability in adults. However, studies that have compared the clinical performance of SADs in geriatric patients are limited. Therefore, we evaluated the clinical performance of airway management with i-gel® and laryngeal mask airway Supreme (LMA SupremeTM) in geriatric patients.
METHODS: The subjects were Ameri

Neuraxial anesthesia is widely practiced for lower limb, abdominal, and pelvic surgeries due to its favorable risk-benefit profile compared with general anesthesia (GA). However, patients with previous lumbar spine surgery and instrumentation present significant challenges for conventional lumbar spinal anesthesia. Altered anatomy, epidural scarring, and the presence of metallic implants often preclude successful needle placement or result in patchy anesthesia. In such patients, GA may be considered, but it carries substantial risks in elderly individuals with significant comorbidities. Thorac

Burr hole craniotomy for the evacuation of subdural hematoma (SDH) can be performed under general anesthesia, surgical site local anesthesia, or scalp block. The prevalence of SDH is higher in the geriatric age group who often suffers from multiple comorbidities. The use of general anesthesia with polypharmacy and airway manipulation may adversely affect the systemic physiology in these patients with substantially decreased reserve. Relief of pain is often inadequate with surgical site local anesthesia leading to tachycardia, hypertension, adverse cardiovascular events, and poor patient satisf

Anesthesia is an essential component of dermatologic procedures, influencing pain management and patient outcomes, including wound healing, infection control, and cosmetic appearance. This review examines the impact of various anesthetic techniques, topical, local, regional, and general, on dermatological outcomes. The findings reveal that while local anesthesia is preferred due to its efficacy and safety, specialized considerations are necessary for pediatric, geriatric, and high-risk patients. Anesthesia-related complications, such as allergic reactions, systemic toxicity, and delayed healin

OBJECTIVE: The aim of the study is to determine the appropriate level of anaesthesia for elderly patients when using Narcotrend for maintaining anaesthesia during surgery.
MATERIAL AND METHODS: A total of 135 elderly patients with intestinal tumours were randomly divided into 3 groups: A, B and C (during the surgery, the anaesthesia levels were maintained at D0, D2, and E1, respectively). Heart rate (HR) and Mean arterial pressure (MAP) were monitored at the following time points: T1 - before anaesthesia, T2 - before intubation, T3 - after intubation, T4 - during surgery, T5 - at the end of s

AIM: Postoperative pulmonary complications (PPCs) can increase the risk of postoperative mortality, and the geriatric population has high incidence of PPCs. Early identification of high-risk geriatric patients is of great value for clinical decision making and prognosis improvement. Existing prediction models are based purely on structured data, and they lack predictive accuracy in geriatric patients. We aimed to develop and validate a deep neural network model based on combined natural language data and structured data for improving the prediction of PPCs in geriatric patients.
METHODS: We c

In a five year period, 62 patients aged 70 to 91 years underwent operative inguinal hernial repair. Other significant ailments were present in 46 of the 51 men and six of the 11 women. Postoperatively, complications occurred in 16 patients, four of whom died. In 44 patients with reducible hernia undergoing elective repair, there were no deaths and complications occurred in eight, while, in contrast, of the 18 with incarcerated or strangulated hernia, complications occurred in ten, and four died. The differences statistically significant. Local anesthesia was associated with the lowest complica

AIM: To determine whether carrying out the Comprehensive Geriatric Assessment before operations would be useful for predicting complications, particularly postoperative delirium (POD), in old-old patients.
METHODS: A total of 517 patients aged 75 years and older, who underwent radical surgery for gastrointestinal cancer at Osaka University Hospital, were recruited for this observational study. The Comprehensive Geriatric Assessment components and assessment of performance status were carried out before surgery, and a record of postoperative complications including POD was made prospectively u

The results of treatment of a group of severely ill and geriatric patients with chronic or subacute subdural hematoma using the twist drill craniostomy (TDC) with closed system drainage are presented. Of the 22 patients selected for this procedure because of the high risk of general anesthesia, 18 had a total removal of the hematoma with the first procecure, 1 required a second TDC, 1 required craniotomy, and the procedure failed in 2 patients. There was no mortality or significant morbidity associated with this procedure performed under local anesthesia, and there were no infections. The abse

RATIONALE: Cardio-Cerebral Infarction (CCI), defined by the simultaneous occurrence of acute myocardial infarction (AMI) and acute ischemic stroke (AIS), is a rare clinical entity associated with high mortality. Currently, no consensus exists regarding its optimal management strategy or the timing of surgical intervention.
PATIENT'S CONCERNS: We present the case of a 79-year-old female admitted for a hip fracture who subsequently developed Cardio-Cerebral Infarction. Following multidisciplinary evaluation and medical stabilization, she underwent femoral head replacement under general anesthes

In geriatric patients scheduled for hip or knee surgery, neuraxial anesthesia is a safe and effective anesthesia method and may be a better option than general anesthesia. Unfortunately, establishing neuraxial anesthesia is not always easy in this group of patients. Anatomical abnormalities, such as spinal stenosis, scoliosis, and narrowed interspaces, contribute to the difficulties that anesthesiologists face while performing these procedures. The classic Taylor's approach targets the widest interspace, L5-S1, as the needle insertion site and accordingly has an increased success rate in diffi
