Graves' disease in clinical perspective.

Graves' disease (GD) is the most common cause for hyperthyroidism in iodine-replete areas. The disease is caused by the appearance of stimulating TSH receptor autoantibodies (TRAb) leading to hyperthyroidism. Blocking and neutral TRAb have, however, also been described. TRAb can be measured either by competition assays, assays using a bridge technology or bioassays (for discriminating stimulating vs. blocking antibodies). Therapy of GD with antithyroid drugs belonging to the group of thionamides is the first-line treatment to be continued for 12 up to 18 months. In case of relapse, thyroid abl

Treatment of adult Graves' disease.

Treatment strategy in Graves' disease firstly requires recovery of euthyroid status by antithyroid therapy. Treatment modalities, precautions, advantages and side-effects are to be discussed with the patient. No particular treatment modality has demonstrated superiority. Pregnancy or pregnancy project affects choice of treatment and monitoring. Graves' orbitopathy is liable to be aggravated by iodine-131 treatment and requires pre-treatment assessment. Iodine-131 treatment aims at achieving hypothyroidism. Thyroid surgery for Graves' disease should preferably be performed by an expert team. In

Thymic hyperplasia in Graves' disease.

Graves' disease is an autoimmune thyroid condition characterized by the production of autoantibodies against the thyrotropin receptor. It is known to be associated with autoimmune conditions such as myasthenia gravis, Addison's disease, type 1 diabetes mellitus, and vitiligo. We present a case of rare autoimmune association of Graves' disease with thymic hyperplasia which regressed after treatment with antithyroid drugs. Exact pathophysiology of thymic hyperplasia in Graves' is not well understood; it is likely to be the result of rather than the cause of Graves' disease.

Graves' ophthalmopathy: a preventable disease?

Most patients with Graves' disease have some degree of ocular involvement, but only 3-5% of them develop severe ophthalmopathy (1). The reasons why only such a minority of patients with Graves' disease have severe expression of the ophthalmopathy remain to be elucidated. One possible explanation is that non-severe ophthalmopathy and severe ophthalmopathy are two different disorders with different genetic backgrounds; alternatively, they might be part of a spectrum of different conditions ranging from absent ocular involvement to most severe ophthalmopathy. In this case, external variables (i.e

Genetic determinants of Graves disease.

Basedow-Graves disease is an autoimmune thyroid syndrome. Genetic factors contribute to the pathogenesis of Graves disease, and current findings confirm that a number of genes may be involved in the development of autoimmune thyrotoxicosis. At present three loci, namely human leukocyte antigen (HLA, 6p21.3), cytotoxic T-lymphocyte-associated esterase-4 (CTLA4, 2q33), and thyroid-stimulating hormone receptor (TSHR, 14q31), are the only well-known genetic determinants for Graves disease. It is difficult to determine clearly the contribution of large multifunctional proteasome genes and transport

Thyroid carcinoma and Graves' disease.

OBJECTIVE: To describe an unusual patient with thyroid carcinoma who had metastatic pulmonary nodules that regressed transiently during the active phase of Graves' disease. METHODS: The clinical, pathologic, and laboratory findings in a female patient with thyroid carcinoma, pulmonary lesions, and Graves' ophthalmopathy are presented. In addition, the patient's clinical course and results of serial studies are reviewed. RESULTS: A 62-year-old woman underwent excision of a follicular carcinoma of the thyroid and then total thyroidectomy in 1984. Two years later, chest radiography disclosed mu

[Surgical treatment of Graves' disease].

In the surgical treatment of Graves' disease, there are several concepts established. The methods of bilateral subtotal resection, hemithyreoidectomy combined with contralateral subtotal resection, near-total resection and total thyreoidectomy all have there rightful placement in the surgical treatment of Graves' disease. The method of choice should be selected according to the activity of the disease. To avoid a recurrence of the disease, the more aggressive resections have proven to be more successful and there's evidence of preference of these techniques lately. Despite the more refined sur

[Surgical treatment of Graves' disease].

Surgery has a definite role in the management of patients with Graves' disease, although it may be followed by perioperative complications and postoperative thyroid dysfunction. Since alternative treatments such as antithyroid drugs or radioiodine are also available, the decision making in the management should be based on clear assessment of advantages and limitations of each strategy by both patients and physicians. As clinicians caring for patients, we have to make efforts to obtain valid and reliable evidence regarding clinical outcomes of the treatment options so that the information can

Total thyroidectomy for Graves' disease.

BACKGROUND: Therapeutic options for treatment of hyperthyroidism caused by Graves' disease remain controversial. There are three main options: thiourea drugs, radioactive iodine ablation, and thyroidectomy. Each treatment has significant advantages and potential problems. METHODS: The present study is a retrospective analysis of our experience with total thyroidectomy in Graves' disease. Sixty-two patients underwent this procedure in 11 years' time and were followed for a minimum of 2 years after surgery. All had measurement of total thyroxine, T3 uptake, and radioactive iodine (RAI) uptake a

Extrathyroidal complications of Graves' disease.

Ophthalmopathy, pretibial myxoedema and acropachy are progressively less common features of Graves' disease with unknown but probably related aetiologies (Table 1). Treatment for affected eyes and skin is unsatisfactory and will only improve when we understand the pathogenesis of these complications and their arcane relationship with thyroid autoimmunity. This brief review will highlight recent research which suggests that fibroblast activation is critical to the appearance of ophthalmopathy and dermopathy; more general overviews can be found elsewhere.

Graves' thyrotoxicosis and Moyamoya disease.

Cerebrovascular accidents due to Moyamoya disease, a disorder characterized by arterial stenosis at the base of the brain accompanied by typical net-like collateral vessels, occurred in two young Japanese women with Graves' disease when they were in thyrotoxicosis. In one patient, a second attack of cerebral infarction occurred with the recurrence of thyrotoxicosis. Association of Moyamoya disease and Graves' thyrotoxicosis is rare and the pathegenetic relationship is discussed.

Graves' disease and Down's syndrome.

A case is described of severe thyrotoxicosis occurring in a patient with Down's syndrome, in which behavioural disturbance was a marked but reversible phenomenon. In addition, we report that antibodies capable of stimulating the thyroid were detectable in the sputum of both the patient and his mother.

Thyrotoxicosis factitia complicating Graves' disease.

A patient with Graves' disease remained clinically and biochemically hyperthyroid after intravenous therapeutic(131)I, although thyroidal uptake fell to normal. Previously she had taken antithyroid drugs irregularly and vomited twice after oral(131)I. She was treated with carbimazole and thyroxine 0.2 mg daily because of severe exophthalmos. However, she maintained her hypermetabolic state by surreptitiously taking thyroxine in excess of the prescribed dose. When drug treatment was stopped under supervision she rapidly became euthyroid. By tracing prescriptions supplied to the patient it was s

Coronavirus as a Trigger Of Graves' Disease.

CONTEXT: SARS-CoV-2 infection was declared a pandemic in 2020 and affected millions of people worldwide. Angiotensin-converting enzyme-2 receptors, through which coronavirus enters the cells of different organs, have been detected in the thyroid gland. The most common cause of thyrotoxicosis is Graves' disease in which thyroid-receptors antibodies (TRAb) stimulate the TSH receptor, increasing thyroid hormone production and release. CASE PRESENTATION: A 22-year-old woman had symptoms of palpitation, tremor, muscle weakness, anxiety and sleep disturbance. 3 weeks before the onset of these sympt
