Barriers and enablers towards benzodiazepine-receptor agonists deprescribing in nursing homes: A qualitative study of stakeholder groups.

BACKGROUND: Despite recommendations to deprescribe chronic benzodiazepine receptor agonists (BZRA) among older adults, the prevalence of their use in Belgian nursing homes (NHs) remains above 50%. The use of a behavioral science approach, starting with the evaluation of barriers and enablers for BZRA deprescribing, has the potential to decrease BZRA prescribing. OBJECTIVES: To identify barriers and enablers for BZRA deprescribing perceived by the different stakeholders involved in nursing home care in Belgium. METHODS: In a purposive sample of 6 NHs, we conducted face-to-face interviews with

Home infusion program with enzyme replacement therapy for Fabry disease: The experience of a large Italian collaborative group.

Fabry disease (FD) [OMIM 301500] is an X-linked lysosomal storage disorder caused by a deficiency of the lysosomal enzyme alpha-galactosidase A, resulting in progressive multisystem accumulation of globotriaosylceramide (Gb3). Although the introduction of Enzyme Replacement Therapy (ERT) resulted in a variety of clinical benefits, life-long intravenous (IV) treatment with ERT with an every other week schedule, may interfere with daily life activities and impact on QoL. We report here a multicentric, observational, longitudinal data analysis on a large cohort of 85 Italian FD patients (45 males

Protection against radon-222 at home and at work. A report of a task group of the International Commission on Radiological Protection.

The Commission has used an epidemiological basis for the assessment and control of radon exposure in this report. Since all the available epidemiological studies use the quantity inhaled potential alpha energy, this has been used as the primary quantity in this report. The Commission does not recommend the use of the dosimetric human respiratory model (ICRP, 1994) for the assessment and control of radon exposures. The Commission sees practical advantages in the delineation of radon-prone areas where more buildings than usual have elevated radon levels. For dwellings, it is suggested that areas

Home-based geriatric rehabilitation after shortened inpatient rehabilitation: cost-effectiveness analysis of the "Better@Home" multicentre prospective cohort study with a matched historical control group.

PURPOSE: Geriatric rehabilitation (GR) is facing increased demand due to population ageing. Home-based GR could reduce reliance on resource-intensive inpatient care. However, evidence is limited regarding the cost-effectiveness of home-based GR. The aim of this study is to assess the cost-effectiveness of home-based GR following shortened inpatient GR, compared to conventional inpatient GR. METHODS: Eight GR facilities in the Netherlands implemented the Better@Home program, in which home-based GR following shortened inpatient GR is compared to a historical control group receiving conventional

Evaluation of outcomes, costs, and feasibility of home-based geriatric rehabilitation after inpatient rehabilitation: study protocol of the "Better@Home" multicentre prospective cohort study with historical control group.

BACKGROUND: The increasing demands of an aging population, healthcare workforce shortages, financial constraints, and a shift in care perspectives call for rethinking geriatric rehabilitation (GR). To ensure GR remains sustainable, a transition towards home-based GR is proposed, reducing the need for prolonged inpatient GR. This study assesses the outcomes, costs and feasibility of the "Better@Home" program, in which home-based GR replaces part of inpatient GR. METHODS: This multicenter cohort study is conducted in eight GR facilities in the Netherlands, implementing the Better@Home program.

Outbreak of invasive Group A streptococcus disease in a nursing home in Ireland in February 2023 caused by emm type 18.

An out-of-season increase in cases of invasive Group A streptococcus (iGAS) was observed in Ireland between October 2022 and August 2023. We describe the management of an iGAS outbreak involving three nursing home residents in Ireland in early 2023. A regional Department of Public Health was notified of an iGAS case in a nursing home resident in January 2023. When two further cases among residents were notified 7 days later, an outbreak was declared. Surveillance for GAS/iGAS infection in residents and staff was undertaken. The site was visited to provide infection prevention and control (IPC)

Effectiveness of a combined home visiting and group intervention for low income African American mothers: the pride in parenting program.

Intervention strategies are needed to improve maternal and infant outcomes in minority populations living in poverty. Home visiting by nurses has improved outcomes for mothers and young children, but use of professional staff makes these programs expensive. Pride in Parenting was a randomized controlled trial of paraprofessional home visitation to provide health and developmental intervention for high-risk African American mothers in Washington, DC. This study proposed to test whether paraprofessional visitors drawn from the community could effectively influence health and mothers' parenting b

The effectiveness of group and home-based exercise on psychological status in people with ankylosing spondylitis: A systematic review and meta-analysis.

INTRODUCTION: Ankylosing spondylitis (AS) is an inflammatory rheumatic disease in which the physical impact has been evaluated; however, the psychological consequences are less well explored. The primary aim of this review was to determine the effectiveness of group versus home-based exercises on psychological status of patients with AS. METHODS: Six databases were searched until January 2020. Eligible studies were randomised controlled trials including group or home-based exercise interventions. Risk of bias (RoB) was evaluated using the Cochrane RoB 2.0 tool. Relative percentage difference

Teaching family carers about home-based palliative care: final results from a group education program.

Without the considerable support provided by family carers, many patients receiving palliative care would be unable to remain at home. However, family carers typically lack the required information and skills to prepare them for such a role. Pilot work has demonstrated that group education programs for family carers can be readily developed; they are feasible, accessible, and useful. This project sought to build on our pilot research to further examine the effectiveness of a group education program by evaluating the outcomes with a larger number of participants. The program aimed to prepare pr

Prevention and treatment of influenza in high-risk groups: children, pregnant women, immunocompromised hosts, and nursing home residents.

The pediatric population experiences preventable hospitalizations and serves as a reservoir for influenza and its transmission to other children as well as adults. As a consequence, the Advisory Committee on Immunization Practices has recommended initiating influenza immunization of children as young as 6 months of age through 23 months of age and, recently, up to 5 years of age. However, immunization of older children has not yet become a priority of the US Public Health Service. As a consequence, the importance of antiviral agents, particularly neuraminidase (NA) inhibitors, cannot be overem

Family Caregivers' Perspectives on the Potential of Drone-Based Medication Delivery in Palliative Home Care: Qualitative Focus Group Study.

BACKGROUND: Palliative care supports individuals with incurable, life-threatening illnesses, focusing on symptom management and quality of life. Access to timely care, including essential medications, is often limited, particularly in rural areas, leading to gaps in home-based care. Digital health technologies, including drone-based delivery systems, have the potential to address such logistical challenges. For these technologies to be effective, they must be adapted to the specific needs of patients and caregivers, which often differ from general health care contexts, especially in remote are
